I was doing OMAD and eating a pound of beef and a couple of eggs in a sitting and couldn't understand why I was starting to feel exhausted. Finally figured out it was the old sugar crash from too much protein being converted. Up with the fat, down with the protein, couple of days and butter fasting, and feeling a lot better.

Exactly... same here! I always had to eat meat/protein even before starting a walk and after finishing I felt so exausted nevertheless and needed more protein, but after the meal I felt much worse. I also wasn't able to get into Ketosis and wondered why, so frustrated. All of the carnivore Docs say, the amount of protein doesn't matter. I stopped believing that because, like you, I had the feeling of this old sugar crash. So I tried out and a couple of days later I reached 1,5 values of Ketosis and felt great. I wonder if there may be a difference between people with an healthy metabolism and those who are pretty insulin resistant like me (and you maybe?)

I listened to many other low-carb channels and I could not lose a lb. if my life depended on it due to the high protein recommendations. In the past week, I have done some intermittent fasting which decreased my protein intake in half. I lost 4 lbs. in one week. I feel great! Thank you, sir.

I don't do well with plants (fiber, fodmaps, resistant starches), so that leaves animal products. I simply eat generously from them and don't register the grams of protein consumed. My decades long IBS is thankful for this long-overdue discovery.

Folks before you will panic and stop eating meat contemplate on the fact that 4oz of ribeye is only 20g of protein. 1 egg is only 7g of protein, you can eat a lot of it and still be within range Dr Cywes talks about.

The generally rule of thumb is about 1 gram of protein per pound of your estimated lean body weight. So, for someone who is 180 pounds and 140 of that is lean body weight, that person will require a maximum of 140 grams of protein and a minimum of about 90 grams of protein

Thank you Dr. Cywes!! I am so tired of hearing: "gluconeogenesis is a demand driven process...". I really appreciate Ben Bikman, Ted Naiman, Shawn Baker et. al. but when I switched to the carnivore diet my blood glucose never, and I mean NEVER went under 100 g/dl. I never saw ketosis for 3 1/2 months. Now I'm careful not to overdo the protein, do the occasional OMAD and am in steady ketosis which really helps my Hashi's.

    • @JDRatka
      @JDRatka 3 роки тому +4

Ted Naimans range for protein is 1g of protein per 1 pound of lean body mass.
That's within Dr Cywes range. No contradiction there.

This is a timely video for me Dr., Cywes. I am a lean, fit, strong female who's recent A1c was back in the prediabetic range. 5.7 after 4 years keto. I up my protein at the beginning of this year and even though I am very active I think that is why my blood sugar was staying elevated throughout the day. I now have a CGM to see what is really going on. I am hopeful that I can get things back in the normal range. Thanks again

On top of this another doc from the diet doc podcast says that body cannot absorb more than around 30-40g of protein in one meal… think, if you exercise and want to improve your body composition you need to get around 1.5-2g of protein per 1kg of your body mass. Simple answer, eat 3 meals in a span of 6-8 hr eating window.

    • @williamdennis1537

Yeah, ditto that. After about 5 years of eating Keto, pretty much all my lab numbers were optimal-glucose low, insulin low, A1c low, weight 155-down from 210, BP 100-105 over 60 ish, etc. I eventually followed my impulses for more protein, switched to Carnivore, and all the aforementioned markers eventually tanked and inched back up- and suddenly, waaaay up. Glucose baseline 125/130, and most disturbingly, insulin rose from 1.8 to 7. Now back on Keto and not too concerned about eating more protein to address Sarcopenia issues, because I suspect regular exercise may be more impactful than wishful copious meat eating. Clearly, for me, there’s consequences pretending to be wolfman. I’m 73, and one thing has become quite clear to me-I have no idea how long I will live, eating “healthy” or not. Another month? A year-5 years? I do know, however, I have no intention of lying on my deathbed counting regrets and wishing I had eaten a few Snickers on occasion. I don’t see myself thinking, “At least I’m dying healthy”....I find great amusement at that oxymoron. Do what works for you, cheers!

    • @ernestjahn6676
      @ernestjahn6676 3 роки тому +9

Yes, sarcopenia is very real. In fact, when I tried extended fasting I experienced muscle wasting, pretty severe. I have gotten some back with exercise, but not all of it. You have to listen to your own body I feel. It is smarter than any doctor or fad about you, if you "listen" to it.

I really appreciate the knowledge shared by Dr. Cywes. I have been in the process of turning my carb addicted life around. I am eagerly awaiting my appointment with him later this year! I am really at a crossroads though, I am almost to the point of one meal a day, the 18/6 IF is easier by the day, but my struggle is with what to eat. No sugar, no carbs, no starchy veggies, and now I am being told 3oz of protein a day! I am down almost 30 pounds in 30 days, but I am still an obese but muscular 6'3", 390 lb man (all belly fat) and I just don't know how I am going to get by on 3 oz of protein a day, no matter how much fat I add, to say nothing of the gastrointestinal consequences/misery of the added fat in my diet. I guess I will keep be content in knowing that whatever my protein intake is, I am still in a better place than when I lived on pasta and pizza 7 days a week. I guess if it was easy, we would all have a swimmer's body! LOL

I've been keto and doing IF for a few years now. Hit a major stall with COVID and having a hard time getting the added weight back off. Also noticed over the course of the past several months that I was suffering from depression that was REALLY starting to get to me. It would start the minute I opened my eyes in the morning. I chalked it off to the state of the world, etc. Then I went back and I started looking at how I was eating when I started keto in 2018 (yes, I track everything and have been for many years). My macros were different, a little less fat, more carbs and more protein. I decided to try increasing the protein a bit ( i was only consuming about 30 grams a day back then). Ok, so the weight is still not pouring off like it did back then but after only 1or 2 days of the increased protein I noticed a MARKED decrease in the depression! Started reading some articles that state protein can increase dopamine and norepinephrine. So until my keto friendly primary care doc tells me otherwise, I will keep my protein levels up a bit. Better than antidepressants!

I think you need to learn all you can, and then try out what works for you. Your goals will be different. Mine are weight loss right now, that is very different than people who use carnivore as an elimation diet. You need to judge your own goals, needs and abilities.
They are still doctors (people), doing their best to GUESS at these really complex biological issues and convoluted social diet confusion.
Start trying it and see how you feel.
I like dr Cywes because he makes it simple. I will say for myself to fast (OMAD), I have to stuff like dr berry says. If my eating window is bigger I can do what dr Cywes says. So just see what makes you feel best, physically and emotionally

I was confused, too, until just a day or so ago, when I realized that, much like keto, each person has to eat just until they're full---not "stuffed," which is very hard on the stomach's digestion process---and use common sense as to how much to eat.
None of the doctors are stating as a command that people MUST eat XX amount of protein (or anything else), but that one should eat as they're body needs (whatever kind of real food one is eating).
Common sense is key, and everyone should be doing further research than just listening to only one person, something I've heard every keto specialist and carnivore advocate on YT state.
      Hope that helps.

Didn't seem to matter what I did with low carb and plenty of fat. Ability to stay in Ketosis was frustratingly random. Stopped eating cheese/cream/yogurt and voila! Your body will give you cues when you’re consuming too much, the trick is learning to listen to your body. I have learned that if I eat too much protein, I have high morning blood sugar. If I eat too much fat in one sitting, I have digestive upset. There is no one formula for everyone because we are all different. Hope that helped.

One of the things I am/was thinking about is if the process of Gluconeogensis is demand driven or a product of simply eating too many proteins itself? In my own personal experience, fully carnivore, I find that I pretty much need to do an extended fast routinely to deplete glycogen stores even though my carb consumption is pretty much nil. I typically see my blood glucose levels slowly rise over several weeks. And an extended fast seems to reset me back lower. I really, REALLY need to exercise more. lol 63 years old, 160-165 lbs, 6 ft. I had the same problem with rising fasting glucose on carnivore. I found that I needed to carb cycle every few days to bring it back down. Dr Saladino warned us that this would happen if we didn't. It's called pathophysiiological insulin resistance. Cells would become insulin resistant on purpose to spare the small amount of glucose from gluconeogenesis so that only the brain and red blood cells can uptake the glucose. This type of insulin resistance causes a bit too much glucose in the blood.
I usually fat fortify my steaks and seafood with tallow, butter, or swiss cheese to make sure I get in more fat. That's just works for me. When I do carb cycle I eat an avocado, some broccoli or a few bites of rice. I know it's more keto-carnivore but I find that it's more beneficial to me personally that way than 100% carnivore

I initially disagreed with him like how many people in the community did. But then as he explained it and I having experienced elevated fasting glucose it all made sense to me. His choice is honey but that's his choice of carbs. Mine is rice bc I rather not take in fructose. After I tried carb cycling I could not believe it myself that Saladino was right. Prior to carb cycling my glucose would be 110-120 and could never get it any lower. Once I began to carb cycle it went down to 70s-80s which is exactly where I need to be. Not only that but I benefit from more magnesium, vitaC, and other nutrients that are more rich in plants than in animal foods. We are omnivore after all right

Just came across this video but timely for me. My BUN has increased from 21 to 31 since starting a Carnivore diet in April 2023.
Am 67yo and 6’2”. My BUN in Feb 2020 was 17 with no special diet and was doing strength training 3xWeek.
In april 2023 blood work and med checkup my BUN was 21 and was diagnosed with NAFL. My hbA1C was 5.8 and fasting TRIG were 125 and body weight 226lb.
Immediately cut out sugars and sugary drinks and over the next 3 months began cutting out more and more carbs and replaced those calories with more meat and eggs.
In early aug, A1C had dropped from 5.8 to 5.6 but BUN had increased from 21 to 27. TRIG dropped from 125 to 51. At that point i had pretty much cut out all carbs and was basically carnivore with some mild cheat days. Also started to incorporate more and more restricted feeding window down to a 6-8 hours. Checked blood work again in early october, A1C was still 5.6 and BUN still 27. TRIG 65.
Continued with carnivore diet until now. Checked blood work again in early Dec and A1C had dropped to 5.3 but BUN increased to 31 so seems like my liver function is going in the wrong direction. Also somewhat concerned with kidney function as my eGFR is 65-67.
My diet isn’t all that crazy. 4 scrambled eggs and 8-10 thick slices of bacon in early afternoon and then 4-6 slices of smoked salmon and a 300gr ribeye around 6pm.
My body weight has dropped 50 lbs down to 176lb and my waist size has dropped from 40-41” down to 30”. So am really happy with that transformation but my liver and kidney blood markers have me a little freaked out.
The nitrate preservatives in the bacon is one concern and i eat a lot of restaurant prepared steaks and never know for sure what the meat is cooked in (marinated) in terms of oils and sugars. Have been in Thailand since early October which explains all the restaurant steaks.

    I've always wondered about this.
When first getting into ketosis, people see high ketone wasting in urine, breath etc, higher weight loss, and from faster fat burning.
Is this due to the body still running on glucose, and the best source of glucose being the glycerol backbone of triglycerides?

    • @Bonussime
      @Bonussime 3 роки тому +2

      Hello :)
It is mainly because the body is not used to utilizing ketones efficiently, hence the losses and wastes. As the body becomes more fat adapted, with sometimes longer periods of fasting or a LCHF diet, it also becomes more efficient in utilizing ketones, either by burning it more efficiently and producing less waste and thus losing fat more slower, or by producing more energy for the same rate of fat burning.
A shift in the mitochondria "memory muscle" is needed, and takes time for them to switch from glucose burning to ketones/fatty acids burning.
For your hypothesis, glucose is quickly expended in case of fasting (24-36h at best), and only the liver's glycogen counts, not the muscles'. Also, the only tissues/organs requiring mandatory glucose are the brain (20-30%, check Sokoloff (1973)), the retina and the lenses (100%, which are a projection of the brain and neural tissue), the renal medulla (100%, despite having mitochondria), and the red cells/haemoglobin (100%, no nucleus nor mitochondria).
Also, glycerol is only one part of GNG in the liver : lactate and pyruvate, the latter in the form of alanine, are needed. Check the Cori and Cahill cycles for more information.
If you check the study on the medical supervision of Angus Barbieri's 382 day fast, you can see that his blood glucose went below 20mg/dL, down to 17 ! (while "healthy" average is today between 70-100 and 80-110 mg/dL) without any adverse effects. The only time he fainted was when the doctors injected him with glucose, hence rising insulin, lowering glucagon and shutting down momentarily ketones production.
I hope this small explanation helps !

    • @Unsensitive
      @Unsensitive 3 роки тому

      @@Bonussime
I ment before fat adaption, by "first getting into ketosis".
      Perhaps that clarifies my meaning.
I was more looking at "why the body isn't 'efficient at using ketones until fat adaptation' ", to word it better. In that perhaps it's not an efficiency issue, but a biological need.
Since you have a greater need for glucose, and to your lack of fat adaption.

Wow, I eat 10 to 15 eggs a day and try to eat 4 to 8 oz of protein a day while doing carnivore. I can't eat much because I get full fast. Recently, I went back to eating green veggies at my 2nd meal. Trying to get BG under control.

Dr. Robert Cywes, do you know Dr. Eric Berg DC?
He advocates the healthy ketogenic diet, with plenty of veggies, mainly cruciferous veggies and leafy greens.

I’m a caregiver (and advocate) for elderly family members and have been to many seniors for several years. I have found that as they age, it’s difficult for them to get their needed protein in. 1 reason is lack of appetite and another reason is being able to chew due to dental issues. How can I be sure they get in their protein and meet their caloric requirements if not via protein shakes? This has become an ever growing problem that I see getting worse yearly.

Perhaps cooking soft meats, such as pot roast? Cooking meats in an InstantPot or CrockPot until they are very mushy?
I have two cousins who live in assisted living facilities, and the staff feed them absolute garbage. It's horrifying. Nothing I say or do makes a difference though. If you have any suggestions for me, please let me know. Thanks for your post. :)

@@pdxoregon1 hi, thanks for the reply. Yes soft meat works well, as does eggs 2737 different ways lol
However that brings us back to problem number 1 which is lack of appetite. There’s only so much actual food they can eat in a give day. That’s why we always resort to protein shakes. It’s hard to get them to even get their calorie requirements just to get through a day without adding anything physical.
My stepdad had stage 4 kidney disease and on home dialysis. There was never a time that he was in his protein range on his monthly labs. It was so frustrating. It’s a battle that saddens me. Many times, even those with no underlying illnesses, their minds are top notch, but their lack of nutrition eventually catches up with them.
I always gave protein shakes and all sources of protein at any meals I could get them to eat.
Bottom line, to me, life is better with quality instead of quantity.
      Best wishes to you.

My wife just died from stage 4 breast cancer. She lost her appetite and taste for meat and eggs, so I made a high fat, moderate protein, low carb shake for her, which she enjoyed. This recipe is about 2100 calories and would last her about two days:
1 scoop plant based chocolate protein powder
2 scoops egg white protein powder
      1/4 cup unsweetened cocoa
      1/4 cup MCT oil
1/4 cup orange flavored fish oil
      1/2 cup heavy cream
      1/4 cup sour cream
      2oz cream cheese
      one cup water
      ice
Tip: Mix the cocoa and oil in a separate bowl, then blend with the other ingredients.
She survived 29 months after diagnosis. Her oncologist said her low carb diet helped.

The theory makes sense but not really what I see in the real world working with clients. Especially for weight loss, protein should always be the focus.
