Every time i think ive got my head around the low carb diet something comes up that adds complexity/confusion. To be honest I'm going to stick to eating fatty beef and eggs and not worrying about protein intake.

I'm with you Tom! My head is spinning after this video! 😵 I love you Doc but this is way to complicated for me! I'm with Tom everything seems to be going great eating my steak and eggs ! I can't get into trying to figuring out how many grams of protien a day by doing blood work every month or what ever! There has to be some sort of average a day for these numbers of fat and protien with out digging into blood work wich didn't even exist yrs ago and people lived to be over 100 so idk
Ever since I went keto 4 1/2 years ago I've had high fasting glucose. Last few days based on a video on Kelly Hogan's YT channel with Amber O'Hearn, I've lowered my protein intake per meal, upped the fat to 70 - 80% and do 3MAD instead of 2MAD. My fasting glucose dropped into the 70's. Amazing.

Same thing happened to me. Watched Stephanie keto person, lowered protein and raised fat and no more dawn phenomenon! Amazing!
If I eat beef at night I always have higher morning glucose. When I eat Chicken eggs bacon I get great morning glucose

I am not getting my brain in a twist about this. I eat carnivore and butter, one or two meals a day, to satiety. Period. I feel great, no meds, no problems.

But I come here to really understand what is going on metabolically. I am done with eat all you want steak Carnivore- my blood sugar never went below 100 doing that. Now I'm 82-100 grams protein to 145 grams fat/day and my glucose and ketones are in good shape, finally. (F, 5'6", 59, 140 lbs.).

Those influencers were right _ I cannot believe I just watched Dr. Cywes spewso much BS in a video. The excess protein will NOT be converted into carbs (and allegedly ultimately into fat) in FED STATE ! Gluconeogenisis depends on glocagon which gets sercreted only under hypoglycemia! And consequently gets stopped as soon as you are out of it !!!!!!!!
If you have (for example) 1000 gram "too much" protein lying around in your system and in a hypoglycemic condition the Gluconeogenisis were to start _ it would stop IMMEDIATELY when there is enough glucose to get you out of hypoglycemia ! Just because you had 1 KG "excess protein" in your system DOES NOT mean all of it will be used to make new glucose (which would then ultimately allegedly end up in your fat cells). NO THAT WILL NOT HAPPEN! If it did then eating a couple of KG of chicken breast (just as an example) would give bodybuilders/powerligter/strongmen seizures left right and centre because their body somehow mgically converted all to glucose 😵😵
You are spot on Doc! When I did carnivore/keto the first time, after the initial drop of water weight, my weight didn’t budge. Being insulin resistant, i was eating too much protein at about 1 lb of beef per day being a female. Now that I’m eating about 6 ozs of meat and some protein from eggs and/or cheese, my weight is dropping by 2 lbs a week and my BUN level is at 22 along with fasting glucose dropping to the low 90s. Never knew how too much meat could raise your glucose and stop all fat loss. I wasted years doing low carb and shoveling unlimited amounts of meat and getting nowhere.
I wonder if that's what my problem is too. I need an appointment with Dr C!! I sure can't get this kind of info from any Dr around here. Lol thanks K this helped me a lot.
@@groovy-momma418 but know that 'K' could be 24" taller than you, a different sex, with twice as much lean muscle mass as you and you PROBABLY have MUCH lower fasting glucose because low 90s isn't all that impressive and easy to beat Know your BUN

@@KJB0001 LOL You are confusing me. I'm not sure if you are being rude or not but I Don't know what all the numbers and other stuff mean. However, I checked and in Dec. 2022 and my BUN is 22 mg/dL Standard range says : 6 - 24 mg/dL

I’ve eaten no sugar, processed foods, fruits or veg, or basically any carbs to speak of for months, except dairy occasionally, but I do eat a LOT of protein in the form of beef. The problem is that there are so few people who can do the proper analysis, as you have now done. In that regard, I am streets ahead of my own GP, but will not claim for a moment that I am close to knowing it all. The other problem is that there are many good experts on UA-cam, but many disagree, especially around protein and gluconeogenesis.
This was a real eye opener for me. In reviewing just BUN and Creatinine levels in lab work for the past couple of years I have been "sick" metabolically for a while. Things have improved, with dietary changes in the past 6 months, but this has opened my eyes to how the process works and why it can take so many months to begin to rebound. Thanks!! I will certainly stuck with a low carb/carnivorious lifestyle.
Completely eye opening. My October video call with you was the same! 177 lbs down in 20 months! I've been carnivore for the last 10 months. All these influencers online shout 'MACRO COUNTING' Never counted macros.. not gonna start now. For someone who eats OMAD - meat, eggs and salt - my BUN is low at 13! I'm gonna break up my meals - try 2MAD and see if that makes a difference. Seems my large protein meals end up as stored sugar.. time to make some changes. Thank you!!

eat until you are full or for example meat not tasting good anymore. Thats the moment when your body has enough protein and it won't get metabolized into glucose. The body has nothing to waste!

Listened to this video 4 times. Can’t pin point the gold nugget. Can’t seem to focus.
Amazing info. My entire life I was told I have proteins in my urine but never with enough importance to explain why or future problems it may cause. Them gout happened. Only to be diagnosed with no real help from the Dr I researched the info myself and implemented what I knew to understand. I was a high carb high fat high protein (imagine the numerous ailments) so I backed off the protein but in research I found that carbs can also be a contributing factor but nonetheless I stuck with the gout control with food mainly less protein. I've been managing through baby steps my carb addiction. Then I found keto 5 years ago and oh my what a change that made but I still struggled with gout trying to sneak back so I'd cut back on protein. Then through trial and error I've realized that the non starchy veggies are causing problems and weight trying to sneak back on. The last 30 days I've been standard carnivore much to my surprise that my gout has not once inflamed. Which surprises people bc it doesn't make sense. But I know my experience and am grateful for the carb addiction doc speaking plainly.

Thank you again! Carnivore for 17 months. Finally metabolically healthy bloodwork. Did not work out during the process of transitioning my metabolism. Prior history as high class powerlifter (not for the past 8 years tho) and at 57 want to simply do kettlebells as my primary "until I die" weight fun (vice workout mentality). Whoa! Recovery time was awful - something I had never experienced doing heavy yard work throughout process. Not pain, but extreme muscular fatigue. Decided to track macros and was sitting at 45-50% fat. Wrote my own tracker program (I'm a geek). I had tried 80% before and know it is too much for my system (at this point in time). Settled in to a 70/30 with a starting caloric and protein range based on age and weight. Clearly back into ketosis. Just looked at my last labs and I was just slightly out of the ranges you recommended (.1 over on BUN and creatinine). Can't tell you how much I appreciate knowing HOW to actually track how well my body is processing protein! I am a medical professional -- so I am all about letting the tests tell me how my body is doing and NOT simply going by "do I feel good/bad." Yes, that is important, however overcoming addiction typically feels bad even though it is good for you, and you can feel good for a heck of a long time before the body fails because what you are doing is bad --- knowing how and what and when to MEASURE is so important to me. Your willingness to educate is beyond appreciated!

After hearing your explanation of protein with Coach Bronson I adjusted my fat to protein ratio and went from gaining weight to quickly losing weight again on my carnivore diet. Great info.

@@richardjohnston4682 mostly butter. Butter in my coffee or melted butter in some warm water. More butter on my meat. I still haven't managed to kick the coffee but I don't seem to have any problems with it so far
How much do you eat per day?
@@lichttv8302 I'm not super strict on tracking fully. But I typically get a quarter pound or more of butter in a day. I was eating 3 to 5 lb of beef a day. And that was clearly way too much protein. I was trying to do what was advised eat until I was full like most of the carnivore promoters suggest. I'm probably down to somewhere between a pound and 2 lb of beef a day now.

I’m 6ft1 90-95kg, 31y, bf% 13-18% fluctuates depending either i’m on a mini bulk/cut.
I usually eat:
1)10large eggs, sometimes with whole milk
2)1-1,3lbs of very fatty ground beef (I ask specifically to make me prepackaged).
3)espresso with cold whole milk (up to 4 or rarely 6 per day. I don’t have coffee side effects like some).
And all this keeps me in my “perfect” range. Can’t imagine how much muscle you need to be eating up to 5lbs of meat per day…

Everyone keeps saying to "just eat meat" when in reality, the fattiest cut of meat will always have more protein than fat. People forget our fuel source is not protein, it is fat. Every 'expert' will presume to tell you how much protein you need to eat when, in fact, we don't really know.
People also way over think shit this is something you have to figure out for yourself no 2 people are exactly alike.

Sausages have a high fat to protein ratio. The problem is finding ones that are not adulterated with phosphates or other dangerous chemicals.
@@1truemoose I found a good fat source at Costco in he meat department. THEY CUT AND SELL A LOT OF RIB STEAK SO THEY HAVE BIG PACKETS OF THE RIBS. TENDER RIB TYPE MEAT WITH LOTS OF FAT. THE PRICE IS MUCH MORE REASONABLE.

This is the most enlightening information I have heard from you so far and I have viewed almost all your videos-some of them multiple times!!! As I sit here looking at my most recent bloodwork panel, I am absolutely fascinated!!! I can see where my numbers are and can immediately see that I am eating a little too much protein and very likely not enough fat!!! As a side note, I intuitively already thought this to be my case…!!! I have been eating VERY FEW daily carbs and sometimes no daily carbs for over three years and have lost almost 100 pounds. Oh wow this is a great video. I never understood the blood urea meanings as you’ve explained til now. My tests show urea at 22 mg/dl which is high. My insulin is .6 m u/l and that’s normal , both were fasting labs so I’m sure that my ratios are off with protein and fats and why I’m having trouble burning my fat in my tummy area. This was super helpful! I've been seeing high BUN and low creatinine on my labs for awhile with fasting blood sugar still in the 90s and wondering if too much protein was a possibility. Time to experiment!

Thank you. My fasting glucose and creatinine are always elevated and “flagged” on blood work. I eat mostly protein and fat. Thought it was just the dawn effect and a possible UTI. My goal is 100g protein and 100g fats 2MAD. I have since lower protein per meal to no more than 30g with 1 additional protein snack. Hope this will change my blood #.

I have struggled mightily to get my diabetes under control. I thought that carnivore would be the key. It wasn’t, and I didn’t know why until your video about too much protein. After going from OMAD to 2MAD and reducing my total protein intake my sugars suddenly improved dramatically. Until my dermatologist prescribed a steroid cream for a skin issue. I knew that oral steroids were a massive problem for me, but I never thought about topical steroids having the same effect. After four days of stopping the steroid my sugars are finally on the way back down
Interesting, I got a steroid shot in my knee and my sugar and blood pressure have been high.😢

Would be interesting to see you and Don Lyman speak about protein. Because they prescribe certain grams of protein. Alot is in the context of building and retaining muscle. But they still prescibe between 1.1 and 0.8 per pound so I wonder where that comes from.

Looking forward to my appointment with you in a month. Carnivore over 10 months now, but struggling with having gained weight and cannot hack how to lose it. Just recently shifted my fat to protein ratio to higher protein, fat in around 60 to 70 percent protein. I workout and I've noticed I can go harder, longer and my recovery is supremely better. Before the protein upping, my glucose/ketones were pretty ideal even first thing in the morning. I've got to start testing them again now with this change I've made. I certainly don't want to make things worse for myself.

Hello Dr Cywes, at last something to go by rather than blind faith! I'm interested in what you said about liquid proteins such as whey etc. Does this apply to soya milk? I'll often have a no sugars soya milk if I don't want the carbs in whole dairy milk, and I'm wondering if the protein in this goes to glucose? All the best

I've been saying this for some time. Everybody is obsessed with protein maxing when however we should be fat maxing. I think Bart Kay touches on this too. This is probably one reason Dr. Shawn Baker had a high a1c from eating like 4 pound of steak a day. Even a 260 lb high intensity athlete like him can't assimilate all that protein. It gets converted to glucose and since he is eating fat as well, his glucose transporters on the cell are inhibited potentially leading to hyperglycemia?

Doesn't some BUN come from working out, maybe the day before the lab?
PS: Do if I take creatine before workouts, is there an easy way to predict the impact on creatinine? I absolutely love the idea of bringing down my BUN, which runs high normal (near the LabCorp spec.) BTW, my personal physician is Dr Gerber, so would be cool if you two are on the same page on this. I've been looking to calibrate my protein for a long time, particularly in combination with weekend fasting.

Never really checked my protein intake after going off grains, sugars, and fruits. I thought I would weigh everything today and the proportion was 47% protein, 47% fat, and 6% total carbs based on gram weights. I was surprised that the total protein and fat had virtually identical gram weights of 116 grams of each. This is my total for the day and I only eat once a day. Creates crazy satiety.

My BUN is 20 my Creatinine is 1.0 and my Uric Acid is 5.9 (with Allopurinol daily)...I will definitely need a consult...
Thank you doctor, you have solved the question I have always had. When I consume too much protein without adding enough fat, it feels like eating a lot of sugar and I will have a bad sleep at night.
Sounds like he knows enough to make informed judgements and medical advice and to most parts I agree. Without sounding like I am disagreeing, there are many contradictory comments and essentially mistakes in the information he provides. Surely the complexities of each individuals bloodwork would suggest these comments are not reasonable for mass consumption.

This video was saw helpful for me. My physician and I have been watching my BUN, Creatinine and Potassium levels because they are suddenly high. This coincides with my trying BBBE first followed by attempts to eat 100-140g of protein (even though I wasn’t hungry). Thank you!
You and me both…..I finally understood why I was successful losing a ton of weight when following the mighty EM and her Better half. I literally could not meet her protein goals. It overwhelmed me that I literally cheated by eating less protein because it made me sick. And her unrelentless insistence that excess protein does not increase glucose and literally jacks up our test outcomes.

Rob please talk about raised serum glucose on a carnivore diet. I have gone carnivore losing weight feel great but my glucose has gone from 80 to 100 on zero carbs. Persists for most of the morning. Never had so much energy, clear minded and good gut health so cant believe this is a problem? Have not yet repeated A1c which was 5.2

well, this may explain why I have gained weight after switching to clean keto (I love meat and have 2 big meat meals per day) or else it can be down to diary (heavy cream) that caused weight gain in my case. It is time to look at it. I will do. thanks Doc. You are brilliant.
As a theoretical upper limit of protein grams per day would be height converted to centimeters ( 72 inch tall male 182.88 cm) this breaks down tho if you are very tall or short. Also this is a very lose estimate that could be off for way to many variables to mention, also like i said earlier “ theoretical upper limit daily protein grams “ not how much you should have.

My BUN is 8 and creatinine 0.85. Both of these are within the normal ranges according to the lab report, but according to Dr Cywes, by BUN is very low and my creatinine is a little high, so I'm unsure what to think. However I'm inclined to trust Dr. Cywes more. Because they also flagged my glucose as being too low, it was 65. But our numbers on low carb are going to be different than those of people on SAD. Who knows how they come up with these reference ranges.
It is fairly easy to notice if you consuming insufficient amount of protein. The signs are thinning and losing hair, weak and brittle or chipping nails and bleeding gums. Upping meat in diet resolves the issues. Side note. Actual amount of protein in steak is high but steak contains also fat and water. Ratio protein to fat is about 60:40

Dr Cywes, Do you think that patients with type 2 diabetes on a low carbohydrate diet or a ketogenic lifestyle while taking drugs such as empagliflozin would have a chance to have elevated BUN due to increased plasma osmolality from the SGLT -2 inhibitor and to some extent high protein to fat ratio in their diet or the elevated BUN in this scenario is mainly attributed to hyperinsulinemia effect on inhibition of fat burning and stimulation of lipogenesis leading to the shift of dietary protein to gluconeogenesis and hence the elevated BUN?
I had some, pickled, herring and plain lettuce. I take Chitosen, before I eat red meat, Phytosterol, fish oil gel capsules, and vitamins. My doctor said, that my cholesterol was great and asked what I'm taking. He said, that he wished, that his was that good. My good cholesterol was almost 200 and my bad cholesterol was 40.
I do paleo strength training and HIIT AND donate plasma 2x week. Age 68.
So plasma protein >6.0 required, i was low then deferred. I added a 20 grams whey protein shake daily and I'm good to go. Now eating OMAD and plasma protein still >6.0. 💪🙏

This goes against what is said by other professionals in this area. One gram per lean kg of body weight is what I read. Also read that adults and older folks especially need a little more. Totally confusion here
This is very confusing. Instead of counting how much proteïne, fat or how much of this or that, we should learn to feel our bodies and it's needs. Besides on my opinion our intake of food depends on the climate we live, age, whether we live by the sea or Mountain, the state of our health and so many other factors.

@rob cywes What about the blanket statement of every adult getting in at least 30g protein per meal for muscle protein synthesis per Dr. Gabrielle Lyon/Don Layman? Just make sure the fat ratio is the 60-70% with it, or could that 30g even be too high for some people?

While I appreciate your efforts and attempts to advise us on a "better way" to eat; I question the need to examine the blood so frequently. How did we survive in 907, 1247 or 1950? Our bodies will allow a check of our numbers, but it was not designed for that activity. Again, how the hell did anyone eat "properly" without blood work being done 3-4 times a year? What about the vast majority of us who cannot afford those tests? It seems to me that if I can improve my health by eating 70% better I am gaining a vastly improved, healthier and longer life. The medical tech that allows this constant measuring is great, but... what about just eating better? Perfect is the enemy of good... I am happy with an improved diet combined with some regular exercise, working on resetting my circadian rhythm, and being thinner. It is not better eating that has caused longer lives in the recent century, it is the props offered by the medical industry with the medicines that treat symptoms not causes. Invasive surgeries remove and replace damaged parts are what holds up so many. What are the actual numbers for long life for those who never have surgery to "patch" together damaged organs? The approach using so many tests and working this balancing act prostitutes the joy of having lunch or dinner with family. It is good for you to share with me the latest about what works well to feed me better, but I choose to move forward as best I can without the tests that replace my own poor ability to judge how I feel when I eat various portions of the foods that I know are healthy for me. God designed us to eat and provided a marvelous system to process that food. Why not just use it after we feed it with unprocessed or modified foods? If apples make your stomach hurt, then stop, neh? First world peoples have morphed into some form of android filled with pumps, braces, stents and the odd valve or two. And I am not a gum flapper on the internet. Your approach to health still functions within the genre of "Hey I got tools and I need to use all of them to turn your life into a mind numbing numbers exercise." I shall never eat perfectly with or without your help. I am simple and want my adjustments to better health to be bearable and not burdensome... I know that you know that I can use the color of my urine to help me understand my health. There are many ways to monitor our health without blood tests. My life is so much more than a series of numbers and letters that I have to have some one explain to me. I am satisfied at 76 that my knees stopped hurting, that I can get solid REM sleep over about 7 hours a night, and that my digestive tract works at 90.7% instead of 65.2% and I still have my "vigor." Just a few thoughts from an old man who is living on a strict and limited income and who is healthier than he was at 25 without blood tests to "prove" anything... I am living healthy and I know it! Done is better than perfect!

Time period of protein intake before the excess gets converted to fat for storage? On a OMAD eating cycle will large meat portions risk being turned into fat stores due to the surge of protein?
From what you are saying, you would need urine uric acid levels and eGFR to go with BUN and creatinine in order to get a clearer picture of nitrogen waste management?

In the absence of sophisticated tests, the guideline I use is my LCHF diet ratio of 10/30/60, which equates to around 1.5 times my body weight; 1.5 x 67.5 kg = around 100g of protein every day.

Such interesting information. I am still attempting to figure out how much protein I actually need. My ketones were 0.8 this morning, but my glucose was 91. I ate nothing but meat yesterday......maybe too much meat.
Your glucose may be a little high in the morning due to the dawn effect. It sometimes lasts up to 4 hrs after waking.
