She clenches her fist behind her back in the flashback because she is in pain but she is hiding her pain and putting on a front. Although she’s not in physical pain at the end of the episode she’s experiencing pain but knows that she can’t show it. So she uses the same method she used as a child to hide her pain.

Someone pointed out how that when Annie first joined the 7 that she did something uncomfortable and unwanted with the Deep and in the scene with The Deep eating the Octopus he did something uncomfortable and unwanted like shoving an octopus in his mouth and eating it as he was rejoining the 7. I thought Homelander made him do it because he was mad at the Deep for taking the first slot with the interviewer, but maybe he did for Annie's sake maybe not.
