    AHA and Retin-A two game changers in topical skincare. My skin is bright tight and wrinkle free (almost) I'm 48 my skin still looks 38. People never guess my age, people are always shocked when I say my age ,they always think I'm in my 30's. I do have great genetics my 97 yr old grandma barely has a wrinkle it's amazing. I've been a skincare junkie since I was 20 and stay out of the direct sun. It all plays a part in having good skin.

      I am line free too and people are shocked by my age as well. The two biggest reasons are genes and staying out of the sun. A good skin care routine is important, but you can't overcome sun damage with creams and serums. I use retina a as a preventative, but I have only worked up to every other day use. My skin rebels when I use it every day, so I don't know how much it's doing for me. I love the new S. Korean face masks that are everywhere now. They are aawesome and I use two a week. I also like red light therapy. It makes your skin radiant. By the way, I have dry skin and it hasn't made any difference in aging. A lot of people think dry skin ages faster, but it isn't true.

    I think I have temporary brain freeze and need to learn to play the mandolin to get my mind working ;) Thank you very much indeed for this presentation, which was stellar. I still think I will get referred to a determatologist who can confirm the efficacy of the products I may choose (the fewer the better). In the evening I use a popular Amazon promoted serum containing Hyaluronic acid and vits \c and e on wet skin; wait 10 minutes and use another french product touted as reducing redness; dog and cat so HEPA filter in bedroom with minor humidifying function. AM sunscreen; recommended by hospital pharmacy seems to contain everything you stipulate in layer 1. So I think 1 and 2 are sort of covered; retinoids and peptides are another subject. Getting there and thank you again for deconstructing the glamour of skin care products!!! Very much appreciated indeed!

    Great video!
    Finally someone you suggests a "pyramid" to direct people's choice.
    because we should use different products, can you suggest a "schedule".
    For example, "put this as first thing in the morning. Put that in the afternoon.
    You can use this and that tighter" etc.

      Raffaele Corrente sure yes! Please note though this is only if you have skin like mine and is very very basic, and if you have eczema or skin barrier function problems then this isn’t for you!

      Dr sarah...
      a) If I have PRP, do I need peptides/stem cells?
      b) wich kind of acid in the morning? hyaluronic?
      I always use acids (trotinoine and AHA) at night, because I heard that I should avoid sun exposure the hours immediately following the application.
      I have a mixed skin (my forehead is greasy, my cheeks are dry!)...which kind of toner should I use?

    6 years later and I still come back to this video EVERY time I need to redo my skincare routine. No one else has ever been able to explain to me how to build a routine properly (and most are just trying to sell you products or get tiktok views).

    Thank you for making a video about this! I can find skincare to be quite exhausting sometimes (knowing what to use and when to use it , and so on...) :( But I have to ask : what about BHAs ? Yes or no? I use the Paulas choice BHA liquid exfoliator. Thanks!

    Great video, thanks for posting. Would just like to note Alpha Hydroxy Acid (AHA) is not the same as Hyaluronic Acid but it sounds like you're referring to both in the middle tier of the pyramid.

    Sarah,
    I suffer from mostly dry skin, with some lightly oily areas. My forehead, temples, and side pads of my nose/under eyes flake all the time and get very dry. Weirdly i also get very dry and flakey in my mustache area and basically anywhere where there is facial hair. I also get very dry skin in my eyebrows. I’ve used tons of moisturizers and it seems the best brand that works for me is Garnier. I still tend to get dry flakey skin, so my results aren’t perfect but they are better. Any thoughts or opinions on this specific brand? Also I’ve tried multiple exfoliates and even after exfoliating I still have flakey skin that just doesn’t seem to go away. Any tips or suggestions? I’m desperate and I’m not sure what other brands to try or products to purchase. Thank you 💛

    PRETTY PLEASE, a humble request for a deep dive on DNA 🧬 repair enzymes, like Plankton in skincare.
    What ingredients to look for.
    Actual product recommendations, where the repair enzyme is very high on the inci list. Believe it or not Paris Hilton has a skincare line, imagine the chemists she has access to & with her longtime interest & enthusiasm in skincare , tech & chemistry its not that surprising really. I noticed that almost all of her products have some form of DNA repair enzyme within the formulations, particularly stood out to me was the serum with the plankton being approximately 3rd on the ingredient deck. Looks quite promising, would like your thoughts very much , thank you.

    Hello 😊 I was wondering what do you think of collagen etc.. that comes in pills is it beneficial ? And is it more beneficial than topical skincare ? Because I tried taking my approach more towards pills than topical and I do feel I’ve seen a better difference in my skin but I am asking for the long run ? Thank you

    I use Obaji medical grade skincare including tretinoin. My skin looks incredible. Been on it for 2 years and my skin has been transformed. Also sunblock Factor 50 La Roche Posey is the best !

    Great info but I’m wondering what order to put all three layers of the pyramid on. I’m assuming vit c and sunscreen last, but what about a peptide moisturizer, and an AHA exfoliant. Should the leave on aha/bha be before or after peptides, and does it matter if the peptides are in a serum or cream form

      md w ok so you generally go from thin to thick. Vitamin c, any other serum, then any cream products then sunscreen. Simple!

    So I'm guessing you don't recommend occlusive moisturizers at all for people with normal skin? For those with compromised skin barriers, how long should they continue to use occlusives? Is there a tell tale sign that your skin barrier has been repaired?

    You are so fabulous. You should have your own TV show. Especially love the celeb critiques. you mention telomere length. Do you plan to also venture into other anti ageing treatments such as Bio identical HRT, or growth hormone treatments etc? Ie; if we can slow down the ageing process then we won't lose so much volume, collagen etc?

      Mary Hanover thank you!! Yes I do but I don’t offer it at the clinic. I’m starting a Masters at George Washington in Integrate Medicine in January and I’m involved in a metabolic endocrinology fellowship right now in the US. I was thinking to maybe set up another channel for all the stuff but there are only so many hours in the day!! I don’t know though, do you think it would be interesting?

      Yes, I personally find all the integrated anti ageing medicine interesting. I've seen some pretty radical doctors speak at The Anti Ageing Conference like Dr. Thierry Hertoghe www.hertoghe.eu/en/ who basically uses himself as a guinea pig taking growth hormones, cortisol, testosterone...you name it! (I went a few years ago to research a TV doc) And I think they are ahead of the game in the US compared to here in the UK. But like you say, only so many hours in a day and you can't spread yourself too thin!
      As for a TV series I absolutely love love hearing you review the celebs and the work they've had done from the comfort of your sofa! It's better than a dose of Hello Magazine with a sprinkling of goggle box! 👍@@DrSarahTonks

    Thanks so much for this great video Doc! I'm in my mid 20s and recently started a preventative anti-aging skincare routine. I'm using everything you mentioned except DNA Repair Enzymes and Growth Factors. I can't afford Growth Factors, but I could afford DNA Repair Enzymes. My question is, are DNA Repair Enzymes necessary if you don't have any sun damage (I've lived in a cold, sunless country my whole life) and use SPF every day? Or are they still useful because of the effects they have on telomeres?

    Don't most sunscreens act like a moisturizer, so if we wear loads of sunscreen, then we are getting this negative effect of skin not making it's own moisture anymore.. Thoughts on this?

    Hi Sarah, I found a video on the pyramid yay! I was just wondering about what you thought of the new Paula's choice peptide booster? I cant use a copper peptide as I have pyrole disorder so my copper levels are through the roof. Could you recommend a peptide serum with matrixyl if not Paula's choice? Also is the medik8 tetra C a good vitamin C ? I'm having trouble with L Ascorbic ones as they seem to sting and I have a sneaking suspicion it's also making the capillaries on my cheek get worse.
    One more the ,about growth factors. Is that the same as stem cell factors? I have Osmosis stem cell serum,is that a good one sorry for all the questions. I'm loving getting to finally watch your videos. 💖💖

      Natalie Brenk phew I missed this comment! I like the copper peptides as a concept, I think it’s quite interesting. Not enough for me to throw away my vitamin c and retinol quite yet though! There are LOADS of products that contain Matrixyl, I’m using it right now in a retinol from Alumier! The Medik8 range is very good yes. Growth factors and stem cells are not the same no. So stem cells are actual cells and can be differentiated into any type of cell in the future and growth factors are just like signalling chemicals. I do feel like the terms are used pretty interchangeably in general though

      what is the benefits of the copper. I am getting a bit jowley and porelss around my mouth now. Doc would this work. I just look so drawn and dried out but do all the sunscreens and vit c and retin a.buffet ,hyrolic 2 per cent, arbutin and niacinamide.can you name a good peptide also.       You could try using a thermal water must with any humectant. Glycerin is another one!

      Gardenia in bloom usually goes away after a couple of weeks- there are some retinoids with hydrating products in them like Alumier retinol which I really like

    Suggestion: perhaps a caption of ingredients or terms mentioned. Some are easily understood ("vitamin C") and/or have been around long enough that we are familiar with the word ("Retinoids"). Some, not so much! When you mention DNA Repair Enzymes (7:00), I can't for the life of me catch what comes tripping off your tongue, Dr. Tonks, despite slowing playback speed down, 50% and I also can't determine if you're listing a category of enzymes or a specific one/ingredient. Thanks for your consideration of this suggestion.

    Is it possible to be allergic to topical hyaluronic acid? I use 20% vit c and just started with retinol at .02% with no problems but always feel my skin easily becomes dehydrated. However, whenever I have used products with hyaluronic acid it ruins my skin, with the worst reactions happening with more highly concentrated serums that contain several molecular weights (weeks on steroid cream! ). Also, if I am sensitive to hyaluronic acid would this cause a reaction if I tried to get fillers containing it?

      Adelaide Hamilton I’ve heard hyaluronic acid can actually dehydrate you more if you are using it in a dry climate like many of us are during the winter! Squalene is then recommended instead. Hope this helps!

      Adelaide Hamilton it’s possible you could be allergic to the other things in the serum, and yes very helpful advice here ⬆️ you can be allergic to filler but very unusual, more likely to be to the lidocaine in it, or any small amount of bacterial by products from the HA production, or it can be an immune response from an underlying illness

      Some of the really low weights can penetrate deeper, but that may cause some issues. I would say anything less than 80,000 daltons can cause redness and inflammation. Stick with higher weights, and also be sure to follow immediately after with moisturizer or balm to prevent hydration in your skin from evaporating into the air in your environment.

      ​@@DrSarahTonks Thank you for the responses, I live in a humid climate and have tried all the tricks such as applying to a wet face with an emollient and sealing with an occlusive moisturiser/oil...
      However, your comment about the immune response prompted me to check my blood and I found out I have low counts, and in particular neutropenia. I am not sure if this is related to hyaluronic acid sensitivity but just wanted to thank you as otherwise I may not have picked up on this.

    I've added benzac to my skink care routine because few but big and deep pimples.
    I'm using BP in the morning and retain a at night but BP in the morning is a mess...
    BP is a free radical, increases wrinkles and worsen skin appearance, so I try to use it only where I have pimples (cheek bones and temple). The problem is when I put other creams and I use Sunscreen, BO spreads all over my face!
    this is why I would use it at night but I read some researches showing that BO deactivates TRETINOINE,
    How long should I wait from BP application before using TRETINOINE?
    Sorry if I'm bothering you, but I spent hours on pubmed and I can't find anything!
    Some people say to wait three hours but can't bring any data to support this statement.
    Can you help me please?

      It lists a few trials in which certain formulations are more resistant to degradation than others. Use of tretinoin 0.1% microsphere gel or adapalene with BPO has been shown to minimise the potential degradation of the retinoid it says. Hope that helps!

      My absolute all time favourite has to be Skinceuticals' CE ferrulic serum, but it's a bit pricey! If you're on a budget, The Ordinary's Vitamin C Suspension 23% is effective, but the texture is a little odd. You might also want to look into Vichy's LiftActiv Vitamin C serum. Hope that helps!

    I'm very sceptical about the need of Dna repair enzyme. Especially because the issue with the lengths of telomeres, after a long period of scientific consensus, has been proven to be a myth
