Surgical (and all extraction effects, for that matter) make for some of most memorable games I've ever had. You'll figure out if you have enough win conditions or if you're deck is just stone cold beat.
I think my favorite surgical play to date was: Modern grixis shadow vs azorius control, game 2 T1: I thoughtseize and see 3 cantrips, an absorb, and 3 flooded strands. Take the absorb, pass. Their T1: m1 they fetch. I fire off the surgical on the strand. They reveal and they drew the 4th. I watched his soul leave his body.
I think you still had the win in round 4 final game when you drew Fractured Sanity. Activate the Field of Ruin on the Arena of Glory, fetching a basic Island. They'd be milled down to 12. Pass the turn, taking 3 from DRC, and chumping the 2 creatures on the ground. They'd now be down to 11 or 10, depending on if they got a land off of Field. Next turn, you have 3 blue to cast Fractured for lethal, assuming that the new card that they top deck isn't Endurance or capable of finding Endurance
