Hi Dr Barkley, i just got diagnosed last month and started on medication. you really helped me figuring out what might be the cause of the problems i was having, which were bothering me a lot for years, and took action to solve them. now with the treatment and some effort i've started doing much much better, functionally and emotionally. thanks a lot for sharing your knowledge and insights on the topic, best regards from Türkiye.
How did you manage that? I got diagnosed last February and I’m still waiting to be titrated, because of global drug shortages.
@@Indiekid-1976 i went to a pharmacy next to a big hospital, they usually have more stock. at least in my city. i also got lucky probably

The ADHD diagnosis has to be very thorough and rigorous in my view. The following have to be ruled out before it is made.
Drug abuse
Alcohol abuse
Depression
Anxiety
Stress
Trauma
Family problems
Sensory processing disorder
Borderline Personality Disorder
Learning disabilities
Bipolar
Hearing problems
Family history has to be checked and school reports and interview with the parents and teachers etc. You can't just go on TV programs and say to somebody after 6 questions welcome to the illustrious world of the race car brain. Somebody you know absolutely nothing about. A 15 minute well-baby check up saying you have it or don't is malpractice! It is dangerous to say somebody has ADHD in that fashion and also irresponsible to say they haven't. Anybody who does is not fit or suitable to be in the profession.

Oh! I have a good dad joke. I make them for my sister with ADHD / autism. Normal babies are delivered by storks. How are extra heavy babies delivered? By crane.
Thanks for the books and channel, I got my sister diagnosed for ADHD, and it's helped my a lot for finding resources about her condition, when lots of mental health professionals, teachers, and doctors didn't notice it.
The study about jaw clenching made me suspect that the reason I don't like ADHD medication is that it makes my body tense. Before I had difficulties to explain what was the over all bodily feeling, but now a sort of holistic muscle tension rings a bell

Oh, that's interesting. Since I started taking methylphenidate my jaw clenching’s gone. But I was diagnosed in my 40s… probably when I gradually started taking control over my life I also managed to reduce stress.

You are the best, Dr. Barkley! Your videos helped me so much with parenting skills for my son. He will be 14 soon. One thing I haven't been able to do is to convince him to accept his condition and follow treatment plan, either that be medicine or therapy. It seems to me the doctors "avoid" explaining the condition to younger children. ADHD is not what most people think it is. Unfortunately, not many people bother to understand. This channel also helps me with research methods, believe it or not. :)

I was hoping the doctors could "convince" him ADHD is a condition that needs to be treated/addressed. Maybe he will have to grow into the realization, and that will be his first step in treatment. Anyway, I am so glad you are having fun running this informative and helpful channel! Can't do without those dead jokes. 😀

Every time you tell us about one of these studies, I feel as though I see the answers as to why, eg the jaw pain in ADHD people is bound to be driven in large part by the dental problems that come from mismanaged ADHD. I was always forgetting to brush my teeth, especially as a kid and the known link to poor economic outcomes for ADHD people means they can’t afford the dental work that they need often, plus they are more likely to drink , smoke, take drugs and have a bad diet, all of which will be a contributing factor.

Thanks! Fascinating, as always. Sadly I cannot take stimulant medication because of jaw clenching - even though for the short time I tried, before the side effects became overwhelming, Ritalin was *amazing*. It was like I was seeing a whole new, beautiful world. But the jaw pain became unbearable and after a few weeks I had to stop the Ritalin. I pine badly, for the world I could be experiencing, if it wasn't for jaw pain. I tried a few different stimulants, but they all had the same effect.
Some people get relief from the clenching if they take magnesium
With the japanese study about DAT-availability. They mentioned that people with adhd had LESS DAT availability. If I understand correctly, would that not mean that with less transporters you would tend to have Dopmine stay longer in the synapse and hence people with adhd have more dopaminergic activity? Cause that would contradict the hypothesis that people with adhd have too little dopaminergic activity?
As far as I understand it, it's the case that people with ADHD have more receptors that capture the dopamine and lock it up within cells, so dopamine is less freely available to do its work. Stimulant medication either tends to block the uptake or even stimulate the cells to release dopamine. Either way more dopamine is available to fuel the neurochemical processes that underpin motivation.
I hope I've understood and explained this correctly! I hope I'll be corrected if not.

This actually answers my curiosity 😮 see, I had finally been officially diagnosed adhd as of last year but had encephalitis back on 2018. I had always have problems with paying attention which was reflected on my notes and friends referring as weird during my childhood time tho,,, and often not so well aware of my surrounding 😅 Anyway, i was wondering if my virus induced encephalitis played a role in my destructive habits so yeah um thank you for making this quite nicely and entertainingly
Im feeling motivated to keep writing my papers and hopefully finished as a PhD too, aameen..

MS is an autoimmune disease, where body attacks to its own neurons. Seems like this is another confounding factor that shows ADHD and other neurological illnesses has strong connection with metabolism (where diet, excercise, sleep etc can help a lot)

Can you please help me with something? It's a big ask but please I'm helpless. My father doesn't understand what ADHD is. He found videos online that ADHD is not real. I'm capable. I'm smart. Until 12 I had aced all the tests in school. I'm in a rut now. My father thinks stimulant medications have side effects. I want to go abroad, use medication and study. Can you please make a video explaining why medication is important and how ADHD symptoms affect someone's life? Please Dr Russell.

hello lovely doctor Barkley, I recalled something you said about intelligence between ADHD people, you mentioned their intelligence is not different from other people, but you said they can't communicate verbally, so if we have multiple intelligence including linguistic intelligence, you're saying their linguistic intelligence is low? and their intelligences might be high, minus( or except?) linguistic intelligence?
When we say "activates" what does that mean here? Stimulant mediation stimulates the part of the brain creating dopamine? As in, makes more? Or something else?
It means that the stimulants make more dopamine available by either blocking the reuptake transporter and, in some drugs, increasing production and release from the nerve cell. In both instances, there is more dopamine available outside the cell to do its job.

I was curious, because I wasn't sure what was actually perceivable: that there is electrical or chemical activity happening, or if it's possible to specifically see what that activity is? Or maybe one entails the other, as in, if this thing is on, this is what it's doing.
Like, if the donut making machine is on, then it follows that it must be making donuts, because that's all it does. What I wondered is whether the imaging shows us the donuts that have been produced...or just that the machine is on? (Donut machines are not my usual choice of metaphor, but it is lunchtime!) 😀
As I understand it the scan measures the amount of dopamine in the cell synaptic spaces, known as availability. Be well. @@bencassidybc
