Kapalbati should always be done on empty stomach. Hours or time difference does not really apply as some people digest fast and some not. As for making a video, every pranayama is different, so whenever I make a video on pranayama I usually mention about the hours in it. One rule does not apply on all the pranayamas.

No, Kapalbati and Breath of Fire are two different Pranayamas.

If lunch is raken late ...say 3..or 4...can it be done in the afternoon before lunch?
Kapalbati should not be done in the afternoon, only in the morning or evening. It should be done on empty stomach. Every person's digestive system is different, so I can't give you a specific time to wait after eating. Type of food you eat is also different, sometimes you feel empty after 2 hours, sometimes it takes 5 hours.
So, observe your body, make sure you are empty stomach.
As fas as after is concerned, wait at least 20 minutes after your Pranayama practice to eat.

Hello. I'm wondering where kapalbati best fits into a morning routine. For example, if in the morning, I generally start with a japa practice, then sit quietly, then do asana. Where would I best insert kapalbati into a such a routine?

Can I do this pranayama in Vajrasana
Kapalbati pranayama increases bodily/digestive heat and vajrasana also increases heat. So, usually it is not recommended to do it in vajrasana.
Mam while exhalation where we need to focus on our breath or in belly area
If you are not able to move belly along with the breath, keep your focus on the belly. Once you master those muscles then stay with your breath.
Is there any restrictions that we need to do only at this time or we should not eat anything before doing it.

After a time a get pain I my back. Am I doing same thing wrong ? Can you advise me please
It depends, how long you are doing it. Add some core strengthening exercised to your routine. That will strengthen your lower back. Make sure to engage the lower right when you workout.
