I think you used the wrong formula. A vertical ellipse formula is (x-h)^2/b^2 + (y-k)^2/a^2 according to my textbook and www.clausentech.com/lchs/dclausen/algebra2/ellipses.htm

the key point here is that when you plot the points (h,K) its RELATIVE to coordinates (0,0) and not the center of (3,1)

Yes it happens if the equation is not equal to 1. For example if it's all equal to 16, you have to divide through the equation by 16
