The toothpicks, apparently are explain better in the book. Beaver is a former smoker, and the toothpicks were a substitute. Basically, the situation was so stressful that he needed a smoke, but used toothpicks. What ultimately led to his death was a lack of impulse control.

    The toothpicks date the movie.
In the late 90s and early 00s there was a big anti-smoking movement and many smokers used toothpicks to control the oral fixation. One of the reasons smoking is so hard to quit is because it is a fast acting stress reliever.
So he's stressed, needs a cigarette, doesn't have one, and desperately goes for his alternative - his box of toothpicks.
Wild how it didn't need explaining when the movie was made.
