  • Hello Friends! In this video, I break down what Botox is and how it's used for TMJ Disorders (TMJD). Botox is used for TMJD to help relieve headaches, jaw pain, tension and/or pain at the temples, and pain at the base of the skull (the occiput). I am not strongly pro-Botox; nor am I strongly anti-Botox. I've tried to present the Pros and Cons in an objective manner. Please watch and let me know if you have any questions!

    I use Botox and it is the only thing that has helped me with the pain and headaches. All your pros and cons are true and if I didn't need it, I wouldn't use it.

    After your treatment, I no longer need to get expensive Botox injections at Stanford. My last injection was in November and I have not needed since. Thanks you Dr. Mistry.

    Botox was the only thing my dentist offered for my TMJ. That and slowly replacing my damaged teeth with crowns. Obviously I'm not ok with that (your info has been super helpful, btw!) and am searching for a new dentist...

    You’re heaven sent. Just yesterday I broke down telling my husband I wanted to try the Botox treatment. I’m exhausted of the chronic daily pain for years now. Your tips on pain management have given me relief like no other. I am in Florida do you have virtual consults?

      I live in Ireland 🇮🇪 Dublin i have been diagnosed with tmj disfunction. I don't no what to do for this 😕 I don't have much pain in my jaws only wen I touch them. I have dislocated my jaw twice from grinding. I use a night guard. But I am in a lot of pain with my teeth but also burning or inflammation in my gums can u help 🙏🙏

    Thank you for your video and the information provided. I’ve had a constant sensation of ‘tightness’ just above my left jaw joint for the last 19 years and that feeling has never gone away (excessive jaw movement can exacerbate that ‘tightness’ feeling above the left jaw joint).
    I’ve been wearing a dental splint for almost two years and that has minimised the tightness I feel in the back of my neck, but the constant ‘tightness’ just above my left jaw joint is still there. I had a left TMJ arthrocentesis late last year but that didn’t achieve any results.
    I’ve recently been thinking of getting Botox to address the constant tightness above my left jaw joint. Even if it doesn’t completely resolve that tightness, if it can significantly minimise it it will help to restore more quality of life, so I don’t feel like I constantly have a headache above my left ear/ jaw joint all the time.

    I've had this done and putting it in the masseter was a waste - started to wear off after only 2 months (probs cause it moves so much) and I never had pain there to begin with. Was great in the temporalis though!! If I were to go it again, I would only do it in the temporalis and see if there are secondary injection sites for that muscle, like closer to the ear or something. I did also read a medical paper about electrode-guided Botox injections into the lateral pterygoids in a patient with facial dystonia so it has been done, I bet that would feel so nice.

    I'm in the UK and I have tried botox twice now, not only has it not helped its actually made my symptoms worse. 10/10 DO NOT RECOMMEND! 😩😩😩

    Thank you so much for this. I have been struggling for 10+ years. I had double sided jaw surgery 6 years ago but the jaw pain came back pretty quickly and is now worse than ever. Doctors have said Botox is my only option left. I am nowhere near Oregon but I wish I was 🥺

      Girl I feel for you. I've had Botox and it's ok, it didn't fully eliminate the jaw pain but it did wonders for the temporalis muscle. The huge downside is that Health Canada doesn't recognize Botox for TMJ treatment (only hyperhidrosis and migarines ii believe) so you have to pay for both the drug and the injection out of pocket and Botox is like $200 CAD per vial and I needed 2 for just 1 application. Thankfully my dentist filled out the forms for off-label use approval and the insurance company approved it. But I still had to pay the injection fee and it only lasted like 2 months in the masseter and closer to 3 months in the temples.

      Thank you. I have to see a surgeon on Monday and am afraid of what he’ll suggest. I was thinking of Botox to help discs get back into place but I doubt it will work. Apparently there’s a tool ‘mitak’ that pulls them back into place. I have night orthotic and asked if they could do a day one. I have no pain, I just haven’t been able to speak or chew normally since accident 10 months ago.

    Great explanation. I wish I knew that Botox injections don't help the lateral pterygoid muscle, before spending money on Botox.

    I used dysport in the massaster muscle & effects lasted only one month. Had two tmj surgeries (each joint) but now seeking a tmj orthotic. That would hopefully help from grinding & unaligned joints..

    One quick question
    So for months have been dealing with a lot of TMJ complications mainly to do with ear, dizziness most likely caused by clenching
    Tried so many things ..couldn't take it anymore so got botox in masseters yesterday
    Too soon to see results obviously but will the weak muscles promote sleep apnea (I strongly suspect I have bad sleep apnea and is the cause of all this)
    And what can I do about it short term? CPAP? I have one at home but don't use it.
    I've tried going to doctors but their knowledge of TMJ is 0 and here in Canada seeing a TMJ dentist is cost prohibitive as I don't have coverage :/

    Love the video, so unfortunate that most questions you get are about Botox, even though that’s contradictory to your point. I know that there is a huge connection between the gut health and teeth grinding. You could add a recommendation to check gut health for those that seem to not be helped by your therapy. For some an anti parasitic program with a good holistic practitioner can make a world of difference in bruxism. Just so sad that people resort to shortcuts so easily, without even attempting to understand the root cause.

    Can TMJ cause teeth pain? I grind my teeth at night and wake up with extreme tooth pain, I can barely eat anything. Does that happpen normally? It hurts so bad, and my jaw gets stiff and pops.

      Yes it can. There are muscles that can refer pain to teeth!

    unfortunately here in egypt we don't have doctors who treat tmj, i've been to so many and all just recommended pain killers which i know it can do more harm than good on the long term, and with the fact that i need braces i'm actually thinking about getting botox injections for the period of the braces so that i can handle the pain of the frequent dentist visits, Do you think that it's a good idea or that i shouldn't get the braces even though my teeth are getting worse every year
    i would love to hear your take on it

    Hello I have some questions if you don't mind answering. Im looking for botox for TMJ for a short term treatment. Im worried about if there will be an exaggerated facial difference? Also, wouldn't any TMJ reduce jaw muscle size, because that's part of the problem, the overuse of the jaw muscles.
    Also was wondering if you have had cases where fixing TMJ also fixes sleep quality issues, in your clinic? Thanks

      There shouldn't be an exaggerated facial difference if you get Botox 1-2 times. More than that can result in facial slimming where the masseters are. Yes, addressing TMJD can also help with sleep quality issues.

    I'm have so much pain and tension but the only doc that both treats tmjd and takes my insurance is boomed til the end of November. Considering if botox could kind of "tide me over?"

    Thank you so much for the Video, I actually have Disk displacement without reduction with limited open, tried therapy and also I did
    arthrocentesis last month, however with no improvement and pain when open mouth My TMJ Dr recommending BOTOX! So please is it treating my case, and there is any side effect!? And if it’s treating for only few month so it means I have to re do it again and again!? And means it’s not really treatment !?

      Hi there - I don't use Botox to treat disc displacement without reduction so I can't really answer if it will work or if you'll have to do it repeatedly.

    Can botox injections be used as a diagnosis, to help determine if a person's headaches are related to muscle tension, etc..?

      No. I don't recommend using Botox for diagnostic purposes.

    I have tmj that had me in partial dentures at 36 (10 years ago) and it has seriously affected my life. My dentist told me after MULTIPLE dentures (I break them) that botox was the only way. I need 100 units or cc whatever in both cheeks, I tried to keep up, I can't. Went another route with dysport. Cheaper but I still need so much it's not doable. WTF can I do??m

    What is a second choice instead of botox?
    I been having tmj, not pain, not tension, just popping/cracking. Not sure if its the socket out of place, how can it get back without using botox?

    The video mentions botox is for short-term relief. But what if the occipital nerves are irriated and not healing well because of the constant muscle tension. Wouldn't relaxing those muscles for a couple months allow the nerves to heal better/faster? Thanks

      I hear you! I’ve never seen Botox to give long lasting relief. I think the reason is that the muscles are trapped in a pain/spasm cycle and Botox is not enough to snap them out of it.

      I did read where it can train you not to clench anymore n if u doing other treatments to handle stress sleeping issues or whatever triggers it. Mayb it won't come back as long as u not working them muscles again idk lol but couple months would relax n help lot faster then if not done.

What are the risks for lateral pterygoid injection please ?

    I'm curious if maybe the botox treatments I'm receiving are being done incorrectly? My doc injects in the lateral pterygoid muscle right in front of my ear, the occipital muscles and a little around my eyes, He never injects in my masseter because I have zero pain in that area. Should he be focusing on that area anyway? Why can't the lateral pterygoid muscle be treated with botox? Because he definitely injects there. I'm not getting much relief despite spending hundreds of dollars.
    Also, are steroid injections something that might be a good option?

      I was under the impression that the lateral pterygoid cannot be injected with Botox. I have heard that Botox doesn’t work for some people so it may not be the best treatment modality for you.

      I would recommend seeing a TMJ Dentist or TMJ Physical Therapist.

    Is it possible that botox can work once? I suspect that my TMJ is a problem of bad habits that I've worked to fix. But even if the habits are gone the damage is still there. Can botox undo the muscle tightness and a fixed posture can keep it from returning?

      I haven’t seen Botox work effectively very often but there are some cases where it can help.

    I am a 27 year old guy, my tmj doesn’t necessarily hurt but I Am so sure that over the last few years it has realllly changed my face shape. I think I need to get Botox to restore my face shape as the bottom looks more masculine/wide etc but my upper face always is more soft and slimmer so it is now out of balance.
    Can I reduce the massater muscle without Botox? Really need help please fml

    My dad get Botox in his lateral pterigoid for open mouth Dystonia. Why do you not believe in injecting that muscle? He gets very minimal relief from injections. Is it worth getting them done?

      I’ve never heard of injecting the lateral pterygoid with Botox. I was under the impression that it’s risky to do so because it’s so deep and difficult to access. If he gets minimal relief I don’t think it’s worth getting it done.

    I press on my jaw and my ringing changes in sound. Does this mean Botox will help my tinitus since mine is due to clenching?

      Tinnitus does not respond predictably to any treatment that I’m aware of... sorry :/

    Hi Doc.. Sometimes when I speak, I notice involuntary clicking of the Soft palate in my mouth.. How to stop it?

      It must be the tensor veli palatini muscle which is probably spammed.. I would find a tmj physiotherapist who can work intraorally on that muscle, or better off a PNT (precision neuromuscular therapist) who has done the extra TMJ course. Are you in the US? They're there. I have learned to do it myself however, it's best if a profesional does it.

      And by the way they do inject Botox in the tensor valli palatini muscle to help with tonic tensor tympani syndrome (ttts), at least in public hospitals in Scotland (for free). I read this in a fb group of TTTS.

    I heard that Botox is grown in a protein that comes from cow's milk, and as someone who follows a vegan diet, that is an ethical issue for me. Are there any alternatives? I've had TMJ issues for years now with no relief from mouthguards etc and anti-inflammatories have no effect on me.

      You can research Dysport but it’s likely similar to Botox

    Hi Doctor. Just wanted to know is trigger point injections for TMJ the same as Botox injections for TMJ?

      Thanks for the reply Doctor 😊 I’ve been following you for some time and all your advice have been helping me deal with my TMJD.
      In what situations would a TMJ specialist use trigger point injections on their patients? My issue is I suffer with jaw tightness on my left side when I open my mouth. I have orthotics day and night time and at my recent appointment, my doctor gave me trigger point injections to help with the tightness in my jaw whenever I open my mouth.

    Hi Dr. Mistry. I seem to be a patient whose jaw recessed a bit after getting botox injections in the deep masseters a year ago. Also, the muscles haven't gone back to near their size after a year has gone by, leaving my face with a bit of a loose skin look. Is normal for things to not go back to the way they were and if not, is there anything that can be done about it in hour opinion? Thank you for your awesome content =)

      Botox can certainly atrophy the masseter muscles within a year if administered every 3 months. I’m not sure what can be done about the loose skin. Sorry..

      @@theTMJdoc mine as well. The one variable in my case is that my muscles were extremely hypertrophic, the masseters were about an inch larger than normal. So maybe it just means mine will take a little longer to come online than usual. Thank you!

    Hi I was wondering I had all top of my teeth pulled and on the bottom I have had all of them pulled but the last 11 teeth on my bottom. 2 months into healing. I’ve started to heal and now every time I swallow I hear a clicking noise and im just wondering have you ever heard of such a thing ?

    I was just suggested the botox treatment and going in for a consultation... It didn't sit well with me that we haven't discussed anything else yet - other than getting a night guard made. What are the other solutions to chronic bruxism and TMJ issues? Mine has wreaked havoc and I only just started to realize it all stems from my jaw! Migraines and vision issues.

Honestly, I just needed the one where you explained how the jaw should be held while resting because that was the ticket! Here I was thinking my teeth should be together - just constantly clenching. I wish I found your videos so much earlier ---- I haven't had any pain or headache since mastering letting my jaw relax. I cancelled that botox appointment and just going to see how well this goes.     Thank you so much for this information. I know that you do not offer botox at your practice, but how many units do you suggest that someone gets on each side (if they were to). I've seen people get as little as 3 units to over 30 each side.

      20 units in each masseter, 10 units in each anterior temporalis. That’s a good starting point in my opinion :)

    I was many times offered the Botox for my pains but I refused . I am looking for a permanent solution and Botox isn’t.

      The botox didnt work for anterior disc displacement. It made jaw muscles less flexible and more stiff. Botox sucks and my tmj got much worse with if

    Hi Dr,
    I have a question, can TMJ cause any neurological symptoms like arm or leg tingling? I ask because I have read about this in other mediums as it might press on the trigamenal nerve which is in direct contact with the spinal cord.

    I have tmj in my right side more than 3 years. I want to know can tmj cause sciatica pain because recently it starts and I cannot walk properly. I have neck pain, hand pain, back shoulder pain at first. now i am using night guard.

      Doctor prescribed me a upper teeth mouth guard said that I have to use it at least 6 month, before going to sleep I wear it everyday. I am using it last 1 month but don't see any difference.😓😓

    Thx you for your informative videos. I have been diagnosed with TMD. I have tried physiotherapy, RMT, chiropractic, ultrasound, laser, CBD oil, nerve block injections, mouth guard. It's been 8 months of stabbing, pain, headaches, ear pain. What can I do? How can you help?

      @@theTMJdoc the dentist that has some training in Tmj scared me for the orthotics as he said it can permanently change my bite as it moves my jaw forward. My teeth are in perfect condition as I had braces. So I'm terrified to do this. He was aiming for 65 to 85 % pain relief. He also said there is no cure. It's $3500 for phase 1 and I may need several phases. So I was afraid and didn't do it. Myofacial release was not offered to me. What are your thoughts please.

    Hi i got 22 units each side masseters a month ago and now my face looks like a shriveled up wrinkly almond. I look a decade older. When will my masseters grow back? Im not a teeth grinder.

      I really cannot predict if or when your masseters will “grow back”. I do not use Botox commonly in my practice.

      @@theTMJdoc with EDS it takes longer for the Botox to kick in? Or you have to do it more times? I’ve been in so much pain I broke down last week and got Botox in my masseter and temporalis but it hasn’t helped yet. I’m still hoping I’ll get relief because it wasn’t cheap 😅

      If you wear the nighttime orthotic indefinitely and you don’t have any serious head, neck, jaw or bodily injuries then yes.

    Pain every day for 2+ years, been to 40+ specialists. Got worse. Can I do botox to get better quality of life for a few months and continue to find a permanent solution? Or will botox for sure mean I can only do that onwards?

      Botox will do this if administered every 3 months over a 1-2 year span. However, Botox is a neurotoxin and it can also atrophy the masseter so much that the mandible falls farther back into the airway when sleeping, which can make existing sleep apnea worse OR potentially even cause sleep apnea.
