All You Need To Know About Xylazine - The Most Dangerous Drug in US History

- 1. #xylazine discovered in 1962 in Leverkusen, Germany by Farbenfabriken #bayer . Was used as an #anti-hypertensive agent. It is an alpha-2 adrenergic receptor agonist similar to #bp medicine clonidine. Xylazine is a non-narcotic #sedative used for #analgesia & #muscle #relaxation only in animals like rabbits, deer, cows, horses etc. Xylazine has been termed as 'anestesia de caballo' (horse anesthetic)
2. Used illicitly first time in #puertorico in 2000 & now in the US esp the #kensingtonphiladelphia . Xylazine has been mixed in #fentanyl to cut the cost of fentanyl & also to prolong fentanyl’s euphoric effect. Also mixed with other drugs like heroin & cocaine . The Drug Enforcement Agency #DEA has seized xylazine-fentanyl dope in 48/50 states. In 2022, 23% of fentanyl powder & 7% of fentanyl tablets recovered by the DEA contained Xylazine
3. This drug is popularly termed as "tranq" & illicit drugs, heroin & fentanyl cut with xylazine, have been referred to as 'tranq dope', “street cut” or “zombie drug”
4. In the US Xylazine is not a scheduled medication. Although it is approved for veterinary use #fda has not approved it for humans
5. Online 1kg of Xylazine is available from Chinese markets for just 20 USD
6. From 2000-2006 cocaine was the leading drug associated with overdose deaths, which was replaced by prescription opioids (2007-2013), heroin (2014-2015), & illicitly-manufactured fentanyl (2016-present). Now a high prevalence (up to 78%) of xylazine in fentanyl screen-positive urine samples in Puerto Rico & Philadelphia & also a steep increase in fatalities from xylazine overdose
7. Many people exposed to xylazine often knowingly or unknowingly use it in combination with drugs particularly fentanyl
8. The severity of addiction : Fentanyl is a synthetic opioid that is up to 50 times stronger than heroin and 100 times stronger than morphine
9. Xylazine is a central nervous system depressant that can cause drowsiness & amnesia & slow breathing, heart rate, & BP to dangerously low levels. Taking opioids in combination with xylazine & other CNS depressants-like alcohol or benzodiazepines-increases the risk of life-threatening overdose. Tranq causes shaking violently, salivating, & vomiting
10. Polysubstance use is common. Xylazine can be swallowed, inhaled, smoked, snorted, or injected into the muscle or vein.
11. The effects on the human body or how to treat intense withdrawal symptoms is not known. Opioid overdose reversal medication naloxone can help because xylazine is frequently combined with opioids. However naloxone does not address the impact of xylazine on breathing
12. In 2021 at least 434 fatal overdoses cases were reported
13. Young boys & girls & middle aged people indulge in this addiction. Mostly these people are #poor #jobless & #homeless . They typically inject into their veins on hands/legs. Most develop #drowsiness #confusion #muscle_weakness . Also #ataxia #blurred_vision #Disorientation #dizziness #dysarthria #dysmetria #fainting #hyporeflexia #slurredspeech #Somnolence #staggering #miosis #drymouth #apnea & #shallow #breathing . Many develop problems of the #heart like #premature_ventricular_ contraction #tachycardia #cardiac_muscle_necrosis & #heartfailure . Some lose consciousness & go into #coma . Some #hyperglycemia & #diabetes
Equally dangerous is scaling of skin, ulcers on upper & lower extremities even at places where the drug has not been injected. Reason: Xylazine leads to constriction of blood vessels & hypotension & bradycardia causing lack of oxygen to the tissues. Healing is delayed & infection sets in. Usually there is copious purulent drainage & a foul smell from the wounds. Extensive necrosis of the subcutaneous tissues, abscesses, & the underlying bones are infected causing osteomyelitis. This leads to multiple hospitalizations with bacteremia from Strep pyogenes, methicillin-resistant Staphylococcus aureus, methicillin-sensitive S. aureus, Enterococcus faecalis, Escherichia coli, & Proteus requiring intravenous antibiotics, multiple debridement of the wounds & dressings. The gaping wounds can be so large that the flesh starts to corrode & attract maggots. Can lead to amputation . The Kensington street is full of such dead looking people - therefore many refer to Xylazine as Zombie Drug. These are the Xylazine Zombies
14. Management problems: a. The addiction of Xylazine not taken seriously enough. Lack of awareness. b. Treatment options hardly any. Doctors & hospitals don’t know how to treat this & don’t have the medicine against it. c. At present the traditional drug screen to detect xylazine in urine or blood samples not available freely. Many feel ignored by society
15. If you’re not willing to take time & look at them like human beings you’ll never be able to treat them or rehabilitate them
