  • 1. Do you know that some people get symptoms of #hypoglycemia even at levels which are higher than normal #sugar levels. This happens in the early stage of treatment of very poorly controlled #diabetes .The reason is simple: the symptoms of hypos like #sweating #tremor #nervousness & #palpitations is caused by the drastic fall in sugar level. Not the absolute value of sugar but the fall itself. The glucostat in the #hypothalamus senses the sudden fall rather than the actual level. And leads to symptoms.
    2. The #neuroglycopenic #symptoms like drowsiness, slurred speech, coma which can lead to death are due to the actual very low levels of glucose in the blood. Less than 30 mg can be fatal. Remember a low sugar level of 30 mg can kill suddenly but a sugar level of 300 will do nothing immediately. But poor control for months to years can lead to severe irreversible complications involving the eyes, heart and the kidney.
    3. If you get hypos during the night while you are asleep, you get #nightmares You get up tired in the morning. A 3 AM test done with a #glucometer to check the sugar level clinches the diagnosis.
    4. Do you know if the sugar drops low in the middle of night, there is a rebound hyperglycemia in the hours following that. So in the morning you can have high sugar levels. Here you don’t need to increase the nighttime dose of tablets or insulin but to actually decrease it.
    5. If your sugar levels are going down without any reason, when you were well controlled on the same dose of insulin or tablets, and no change in the diet intake, suspect the onset of nephropathy. So the apparent ‘good control’ that time is a marker of seriousness. Your kidney is getting damaged and its reflecting now.
    6. Do not use nuts or milk when you have hypoglycemia. It will take longer time to correct low sugar levels. This is because these food items are carbohydrate base carrying proteins which lower their glycemic index and do not raise sugar levels fast. Simple sugar water, juice, or candy is better.
    7. After you have tackled the problem of hypo, do consult your doctor so that he titrates your dose of insulin or tablets. If not then you would get another hypo which may be even worse. Never self-titrate.
    8. Do you know that the hypoglycemia caused by insulin is correctable much easily than that with oral diabetic drugs. This is because the tablets especially sulphonylureas have a prolong duration of action and may lead to more severe hypos and that too for longer duration and multiple times. So your doctor needs to keep you in hospital for a longer stay.
    9. With each episode of severe hypoglycemia a patient loses a lot of brain cells. So death of brain cells multiple times makes the unfortunate patient lose memory and become dull over a period of time.
    10. Some patients do not perceive symptoms of hypoglycemia. These are elderly patients; those on beta blockers for hypertension; those having autonomic neuropathy. This hypoglycemia unawareness can be dangerous because the patient can land directly into neuroglycopenic hypoglycemia leading to drowsiness and coma. This can be fatal too.
    So do take good care of your diabetes. Know more about this dangerous disease and tackle it well.

    आदरणीय सर सादर नमस्कार मुझे बी पी शुगर नयूरोपेथी रेटिनोपेथी कानिक पेनकियाटाइटिस है मुझे इन्सुलिन लगती है यह बीमारी 32 साल से है मुझे दो बार शुगर लो हुई है किंतु मुझे मालूम ही नही पता चल सका जबकि में साथ मै गलूकोज रखता हूँ कभी चक्कर पसीना आया तुरंत ग्लूकोज पावडर खा लेता हूँ भले ही शुगर कम न हो हमें जानकारी देवे कि शुगर कम होने पर एहसास हो जावे कष्ट हेतु माफी चाहता हूँ
