  • Been told you have hip impingement (FAI) and worried about bone on bone hip pain? In this video we'll dive into questions you should be asking about bone on bone hip pain and exercises you can do to beat your hip pain and improve your hip mobility
    The current conventional medical perspective on bone on bone hip pain can you leave you feeling helpless and ready for hip surgery. However, when you look at the science of hip impingement and bone on bone hip pain, you quickly discover that the medical approach is not backed on rigorous research.
    Matt presents two exercises that he's seen help people with alleged bone on bone hip pain and femoracetabular impingement (FAI). These exercises address the muscles around the hip joint. The focus on muscles is important to restore function and relieve pain. The bones themselves from a practical standpoint, are highly unlikely to be the cause of pain. Muscles, on the other hand, are well-known to generate pain and discomfort and are the most likely source of hip pain - whether your diagnosis is arthritis, FAI, or a hip labral tear.

      It’s not called misaligned hips, that’s the same type of misleading language that doctors affect their patients negatively with. Most of injuries that need the right type and amount of work and that’s the right language to use and other not misleading expressions.

    I was diagnosed with hip impingement by a world renown doctor... this happened after I ran a marathon on an over trained hip (tendonitis). After the doc showed me the xray, he said that I'd be the perfect candidate for surgery, which I knew I wanted no part of. I'm a huge believer in self-healing and allowing the body to do what it has to do to mend, no medication, no surgery. Knowing I'd have to help my body cope with the injury, I searched on YT for hip exercises and this channel popped up. I religiously viewed all their hip videos and committed to their exercises and stretches. I took it easy because the pain was immense, but I stuck to it and after 2 months came off crutches. The doctor said I'd need about 4-6 months to heal post surgery (if I had gone that route), but I started walking on my on in half the time, without surgery or medication. Was I in pain while recovering? YES... a lot of pain, but I saw progress every day, albeit minimal, but it added up at the end. The doctor said if I didn't go through surgery that I'd never be back to being as active as I was pre-marathon, and that I'd be risking getting arthritis and eventually needing surgery on both hips. It's been a little over a year and I've gone back to doing my normal activities, eased into them at first, then came back a little stronger because I allowed my body to heal on its own. I do recommend that people stretch and exercise both hips/legs while going through recovery so as to avoid overworking just one.       I was told by the assistant of a surgeon after taking x-rays that I needed surgery. He really surprised me with this I didn’t believe him so I went on to have a second and third opinion and both those surgeons said the same thing. by the time I got to the third one you can really tell he was a salesman. He literally was selling me surgery so I stay in his building. He kept on calling me a tough SOB for walking on this. of course I did research and found out that unnecessary procedures are up big time in the United States. Two reasons why Doctors . quickly suggest surgery is obviously for financial gain and the other one is for status. I will be doing physical therapy, changing my diet, taking supplements, etc. I pray that this does wonders for me and if it does, I am going to send in a complaint to the first doctor and take it from there

      My self got discharged from the army for severe cam joint impingement. Seen the xrays its bad. Broken my ankle and dislocated my knee thanks to my hips mobility locking during soccer and that hurt like a mf. Got more xrays then and definitely got fucked up hip bones here badly. Ive been told many occasions years apart only surgery. Lately i am in extreme aching pain like dull pain. Nonstop. I do all this have been for years still getting worse here. Now my back is being affected badly too. 😢

    First I want to say that this channel has been an inspiration to me in showing me that surgery is often avoidable by strengthening the body but it often takes longer than the typical PT stint. I came across the channel after I had labral repair and the bone debridement surgery on my left hip. My surgical experience was not bad. I've had previous injuries and surgeries and this hip procedure was a quicker and easier recovery. I dont regret the surgery. I made the best decision I could at the time and it did solve the problem. Unfortunately, my right hip started to flare up similarly while I was recovering. I knew I didn't want to have another surgery. I inconsistently did some of these exercises for my right hip. Then I just started back working out, I started slowly and nervously. But I gradually got stronger. My confidence grew and I continued to move and strengthen. The worst thing is to remain sedentary. FAI can be fixed without surgery. I can squat deeper, kick higher, and sit longer without pain. Thanks for helping me see that it was possible!

    I actually do have “bone on bone” no cartilage, but in my ankle, not hip. At 1 month shy of my 19th birthday I was a passenger on a motorcycle that was broadsided by a big ted station wagon. The impact site was my left ankle. My amazing surgeon and team saved my leg. I did lose 2” and lots of issues along with surgeries. They said I would be in a wheel chair by 30. I’m 67 this year…I hike, mountain bike, bodyboard, and just SUPed a couple hours two days ago. I learned how to snow ski and water ski after the accident. After numerous X-rays and consults over the years with orthopedic doctors who did not believe that I could walk let alone walk unassisted and without pain, I stopped going to the doctors and sought my own therapy and workouts. Your information is so valuable for us to understand our bodies and to take responsibility for our OWN health journey. I did radically change my diet when I had a couple of scary periods…it’s been 18 years of a pain free very active life! I am so very grateful to have been sent my first surgeon who would not listen to the team of doomsayer orthopedists and took a leap of faith. I am also blessed to find my way to practitioners such as yourself, Matt, to teach me more of how our bodies work and help me live the active lifestyle I promised myself that I would so many many years ago.🙏🏻❤️

    I'm delaying your course because so often I have signed up for something before I'm mentally ready and then I reject it, for that very reason.
    I'm seeing gradual improvement though and with that comes increased confidence in my body's healing abilities. So, soon I'll be checking out the hip fix program.

    Not commented for a while but now I've returned, yet again I want to say 'Thanks' for such a useful and inspiring video. Always based on moving forward with the difficulty rather than leave things vague and suggest that medical intervention...ie. an operation is the only answer. I'm similar to many of your clients and I know that bit by bit, day by day is the answer rather than one fix and I've tried these exercises out and found them possible and useful. Previously I wouldn't have been able to try them but now I'm in a place where I can and they are yet one more useful option for me to take up.

    I've been listening to off and on for a year or 2... This video seems to track my own lonely struggle. In particular your comment about the medical advice not to move until you get your surgery and possibly still have the problem.

    My doctor wants to operate and I know that would not solve my pain problems because my muscles are not balanced around my hip or my pelvis so how in the world would an operation be helpful. It would make things Much worse, so no thanks, I’ll focus on the muscles and facia that surround these bones and find relief.

    That last stretch hits the spot! It’s the spot that I target with my massage gun. Right side of my hip sometimes pop and now I have lower/mid back pains and right glute pain. Thanks.

    The problem with that analogy is that it's not bone on bone. There's a cartlidge between the bones. That has to wear before it becomes bone on bone. That is called arthritis or an impingement. This more often than not, is painful. However, when you strengthen your surroundings in the hip, you can become more flexible and pain-free. Love the video because it provides information on how to start correcting some of the problems.

      There is another approach which is to avoid stretching the tendons, muscles and ligaments that stabilize the hip. I have an approach that works globally in the whole body to recalibrate tension rather than trying to stretch parts. Many of my client had over stretched hip stabilizing muscles or lots of forward bends that made the hamstrings and deep glutes lax and long.. ua-cam.com/video/mYKHC4GFMuc/v-deo.html

      So, so true. I have a high threshold for pain but 2 years of awful continual pain in both knees ( bone on bone OA) and hip have made me so weary, fearful and lacking in confidence. I have started doing the recommended exercises and although I am in more pain I can see a slight improvement in range of motion of the hip and have established that I especially have a problem with internal rotation of the hip so have up'd the stretches for that. It hard, painful work but the alternative is eventually being housebound and THAT is my dread.     My right hip cracks, snaps and hurts already some years.
    I don’t want a replacement (yet) and therefore tried already many different exercises, but these two simple ones make really a difference! Thanks Matt!

      @@jonathanhadley2555to be honest, I gave up fighting against the pain recently (my hip was stuck and did not see any further improvement) and last month I got a hip replacement. The picture of my hip showed severe damage, so I guess it was the right thing to do the surgery. So far things really improved regarding pain and mobility. The first month the doctor did not recommend any heavy training or therapy , only light exercise and walking but now I am good to restart the heavy work.

    Excellent. Great change of camera view by rotating easily on the mat. Internal rotation on all fours is difficult to comprehend, so even more time with setup and progression could help, although this is better than other vids.

    Very limited internal rotation and knee to chest are exactly the problems I’m having. I never even noticed these problems until I got into a sport where I need to make those types of moves.

    This is what I’ve been telling people I feel is the real issue! I’ve been unwinding my own muscles/tissue issues for over a year now to release my hip pains …getting there …but it takes steady incremental work.

    Thank you! I'm certain this is my experience...if I use a massager on the muscles around my upper thigh, flexors, and psoas...and stretch also ...my hip stops hurting.

    Hey Matt! My hip pain is all on the posterior hip and kills me most in the mornings, specifically on the drive to work and standing up after getting out of my car.
    These stretches feel great. Takes away that posterior hip pain. Can I do them first thing in the morning, or should I allow my body to be awake for a certain amount of time first?
    If you could recommend any sort of stretch prior or post to driving for posterior hip pain, please don’t hesitate.
    I’d also like to add that throughout these two months of pain I’ve had and researching constantly, your videos show up the most often, and I’ve been watching videos spanning back 6 years.
    I’ve noticed you’ve improved greatly on your speech skills in that time. Even in the old videos, the demeanor is very enjoyable and easy to learn. Feels like a simple conversation.

      Sounds like you have short piriformis syndrome, he most likely has a video for this specific injury. I know squat university also has a good video on this too with tests you can do to confirm this injury and rehab exercises.

    Hi Matt, can you guys please make a video on SI joint pain at one point? I have SI joint dysfunction from a motorbike accident and the pain is on only the left side deep in the QL and sometimes it feels sensitive around the sacrum. I haven’t found the best exercises that could help relief pain longer than a day yet.
    Anyway, I’ve also been doing the exercises like what you have shown on the videos on hip impingement as well because I started developing pinchy sensation in front of my left hip after 6 months of being diagnosed with SI joint dysfunction.
    Kindly guide me on this and looking forward to more helpful content. Thank you for putting in your effort to help people.

    Love all your videos! But can you also address the opposite? Dancers, yogis who become stuck in their hips, sharp pains, feel joint locking, you name it, without obvious injury. You know the muscles were flexible before ...so what is the best approach in this case?

      @@Uprighthealth I would love a see a video of these exercises without getting on the floor. Of course the strap I can do in bed, but the other I have difficulty getting down on all fours, or is it the getting up that's difficult? Is there an alternative using a kitchen counter or desk for support, like I use to do the Plank vs getting on the floor? I love your videos and find yours are the most helpful!!     Hi Matt, I have watched so many of your video's. I suffer from 'FAI' going on 3 years now from a roller-skating incident. Every time I try and stretch like you advise, next day I'm so stiff and sore. I don't feel like I'm over doing it but really not sure. How can I stop that sudden pinchy feeling all the time? I can only get the right knee while standing to maybe 60-65 degrees. Left hip is totally fine. Thank you for all you do. I feel like you're the answer! eric

    This is so important regardless of type of pain. Couldn’t walk for 2 years because of a stabbing pain in my knee. I’m the end I just needed to release tense muscles on the side of the knee cap pulling the patella to the side (most likely, cant know for sure ofc). Pain was gone after 2 weeks. 🤦🏻‍♀️😅
    Hopefully I will find a similar solution to my hip now.

    While I have been a follower of Upright Health for a couple of years now, and agree with so much of what you "teach," I have to say that I now have a different perspective. You may or may not remember some of my comments from earlier videos, where I agree with "least" treatments first before or in lieu of going under the knife. When my hip pain would just not get better no matter what I did, sleeping became unbearable as I couldn't lie down flat anymore) I finally had a very well-known and trusted hip guy diagnose me. There were actually several things going on (as I'm certain most people do). Hip impingement, arthritis and most important, leg-length discrepancy. The combination of the three finally brought me to a place of "no longer can function." So I had an anterior hip replacement, with my ortho adding a bit of length to my leg. Not only did all of my pain go away, but much of the back pain I had been experiencing for 30 years also vanished (I had tried lifts and inserts to no avail). So YES, try the least invasive treatments first (exercises for hip mobility and strength), but when nothing works...go for the new hip. Thanks for all you do to bring new awareness to functionality of our incredible bodies. :)

    I have a labrum tear and “hip” pain. Which is really all the muscles around it usually. We’re getting my glutes stronger and other muscles that are doing too much to try not to. I definitely need to stretch more. Some of my pain may be hip a lot seems to be muscles though. Haven’t done his program though. Thanks. I saved this to try later when my shoulders are strong enough to do so. Have some nagging pain from injuries there.

    I've been on this journey for 4+ years. Completely degenerated hip. Muscles had atrophied to the point that I could not activate them. I have progressed dramatically. Had to relearn how to walk. And I still practice that everyday because it is not natural yet. At this point there are two muscles that "pop" when I walk. One in the glut. Probably piriformis. The other in inner thigh. I suspect that is breaking up myofacia. Seem to slowly be getting through it but it still stops me in my tracks. Stabilizer muscles are weak still and I can feel them come into play as I force myself to walk correctly. I'm an FAI customer.

    I tried these and it worked after the first session to minimize almost entirely my FAI. Gonna do it everyday as well as strengthening my hip with hip bands

    Bone-on-bone is the total loss of cartilage in the hip joint. It's not a lack of space between the bones. He's either misinformed or is trying to downplay a serious condition.

      I think it is true that surgery is not always the answer and then one must heal from the cutting of bone or tissues. However there are other approaches as well. Like a car that needs the front end aligned, your cartilage ( tires) can wear out if you do not work on global posture issues. Alignment is key and stretching can be counterproductive. Pain means STOP. There are no parts in the body. I created a breath based neuromuscular method based on global exercises that get your muscles working in balanced tension all at once in the whole body rather than working on stretching out parts of the body..ua-cam.com/video/mYKHC4GFMuc/v-deo.html

    Hi - nice video - could you tell me if the TFL muscle plays a significant role in FAI? My therapist says I should not be touching any glute muscles at present, only releasing TFL, but I have the same symptoms that you are describing in your video. Many thanks.

    Hi Matt, the more I loosen my tight hip muscles, the worse my neck and shoulder on the opposite side hurts. I know it sounds crazy. How do I deal with both? Help! Thanks for all your great videos. Been doing them for years with you!

    Hi, thank you, the stretch your colleague shows is easier to position and do than..maybe the way your showing that stretch. Also, yes your way is better for me b/c I can't get my leg to bend up against the wall... but, for me it's a little awkward. I don't know about anyone else. 🙏

    Hi Matt! I'm a FAI Fixer and thanks for your amazing videos :)
    I have "the same"(!!) poor internal rotation you had, that pushes my leg to externally rotate when I move it up. But I also have to say that all your exercise that forces the hip to internal rotates, always cause me an increasing pain in the following days. Do you think this is normal?

      You gotta reduce the range you're trying to get and also remember to use contractions to strengthen the muscles you're trying to lengthen.

    When trying to stretch your hamstrings while laying on your back on the floor, would it help to place a very small pillow or something like that under your lower back to help keep you from “cheating” and over rotating the hip? Or would this not really help?

      Keep working on your body. Kit Laughlin has great information. Emmet Louis. Tons of things on UA-cam are super helpful. I did NOT study physiotherapy in college. Constant chronic pain is what pushed me to start learning.
      A body wracked with constant pain is BIG motivation to learn how to treat it right. 💪

    Do you have any bow legged stuff going on? I do and just curious how many people with labrum tears have that also. I know bodies are different shapes and mine seem fine, so I was told (nothing to worry about).

    Hey Matt, I noticed that when you do the hamstring stretch your opposite leg is in a bent position. For me this allows the leg I am stretching to stretch further towards 90 degrees and it causes pain in my hip because of this. I wonder if doing the stretch with the non stretching leg straight on the ground means the stretch does not have to be pushed so far (More like 70degrees) for the same stretch result but with less pressure on the hip?

      Bent leg is usually the easier way to ease into the stretch. If it's causing you hip pain, you're going too far and not paying close enough attention to the sensations as you are going into more hip flexion.

    Hey Matt, will stretching my stiff, shortened hamstrings and my glutes help the femoral head move posteriorly in the acetabulum to relieve the impingement? I was thinking it was more of a priority for me to stretch my tight hip flexors and that then the hamstrings and glutes would pull the femoral head back. I experience 'bone-on-bone' pain' from what's been diagnosed as 'severe' OA in the hip, but it's probably not surprising since all my muscles surrounding the hip joint are tight and probably squeezing the hip joint all the time.

    My glute medius is almost paralyzed. It will barely even lift the weight of my leg. I had some novocaine injected into my SI joints and the muscles immediately began to fire again within one minute. They went from 1/5 initiation to 4/5. 🤔 6 years post FAI surgery. Surgeon said it might fix the nagging low back/above the hip pain. It actually made the glute pain worse. But at least the impingement issue was repaired... I had bone spurs and a torn/debrided labrum that needed fixed due to a lifetime of instability and misdiagnoses. I'd say dont not get the surgery if you cant walk. Just know that the surgery is not really the fix. It just repairs the damage caused by the instability issue and gets you well enough that you can go deeper into rehab. It's a bandaid and if you dont fix the root cause you'll need a revision since the same instability is still making the hip track improperly. Did I get it right?!?!
