Literally during the World War 3 match, he changed his elimination over the top rope at the hands of The Giant (Paul Wight/Big Show) to him being thrown under the ropes, so when Macho Man won the match and the belt Hogan overshadowed it with a promo saying "Enjoy it while it lasts because I was never eliminated and all the Hulkamaniacs know that that's still my title". He wasn't heel at this point. At least, not in front of the camera.
One thing you should have added, WCW owed Flair $$ related to the belt. That's why he kept it. He was required to have a deposit with WCW while he held the belt, and Hurd wouldn't pay Flair his $$ back. He kept the belt over $$$, not because of who he would or wouldn't drop it to.

In 1991, Flair and Jim Herd had a contract dispute, which resulted in Herd firing Flair 2 weeks before the Great American Bash, where he was scheduled to defend the title against Lex Luger. The title was declared vacant, and Barry Windham substituted for Flair. Luger won that match. Flair may have been scheduled to lose the belt to Luger. He didn't refuse to lose to him, but rather refused to go with the changes Herd proposed, such as shaving his hair, wearing a diamond earing, and calling himself "Spartacus"
