DETAILS:
1st Responders were called to a Van vs Pedestrian accident. When the Police and firefighters arrived, they found a male down in the driveway in front of the Presidio Hills Golf Course Pro Shop. They immediately started CPR and did it for 20 minutes before pronouncing the male deceased.
The female driver of the van stayed at the scene.
She apparently hit the victim with the front of the van, and she knew the victim. Due to inconsistent statements by the female and the suspicious circumstances of the accident, the SDPD Homicide Detectives have been requested to come and investigate. The van belonged to the victim.
NOTE*
To all of the armchair medics out there making comments about the CPR attempt. The victim had no rib cage left to do proper CPR techniques. They did the best they could for over 20 minutes. Stop with the bullshit comments, you could not have done any better.
You're absolutely correct. I'm a firefighter out of NY. Sometimes it is a special situation. I've personally done CPR on an elderly woman, knowing she had been gone for hours. It was just for the benefit of the family. In my 16 years, my volunteer department is roughly 2-125-1 when it comes to CPR. It's a bad record, but take into affect, we're volunteers. Time is a factor. Only once did I perform it on a person that wasn't cold. Each situation is different. I commend these EMT's for realizing the victim had a shattered ribcage. Keep up the good work.
Holy crap, I can't imagine trying to do CPR on someone whose rib cage was crushed. I'd be so afraid of crushing or puncturing the heart. Bless them for trying, though.
When you do CPR majority of times if they aren't already broken, you will break them doing CPR

Michelle W the CPR trainers actually instruct you to not care about breaking ribs. Matter fact they say as a trained CPR providers performing CPR if the paramedics show up after you've been performing a rescue CPR and they find no broken ribs they will assume you were either trained wrong or didn't do your chest compressions hard enough.
If you ain't breaking ribs you're doing it wrong!
I took an EMT class and I asked my instructor “what if the person has broken ribs before doing CPR?” And he told me “if you do good CPR, you’re going to break ribs regardless. If the person is losing oxygen, they’re going to die, so might as well give them a chance to fight” and I believe what he said because he was a Navy Seal Corpsman

You can't perform CPR forever. They know when the patient is gone. They do their best. I'd like to see how these "armchair medics" would do under that level of stress.
They could start by doing proper CPR. This was NOT proper CPR.

@@katden220 for an adult, if you don't hear ribs or sternum crack and pop under your hands than you aren't doing it right. And there are too many people there to do sloppy CPR, if they got tired then you switch out.
It is not hard to lean all the way over the chest, and lock your elbows to deliver good vertical compressions.

yes your right when i got trained to do it the instructor said i would break a rib how i was doing it asked him would you prefer to wake up with a busted rib or not wake up at all he had to think about it. the fire fighter was like slap wake up lol stiff arms one hand on top of each other and use your own weight is the usual advice and you can keep it up longer with out tiring

I learned if you are doing it right, you should not be breaking ribs. Not to say it can't happen. But if this guy did not have any ribs left, it was due to being run over by the van or the males attempt at compressions, which was grossly wrong and he should have been corrected right then and there.

Yes, I will agree, they were probably just going thru the motions on a deceased person. But when I hear someone say they should be breaking ribs, then I know they were trained incorrectly or just didn't understand.

But what really has me wondering now is your need to point out your work history as a FF/EMT and "paid service" at that. I hope your work ethics would not have changed based on that fact. Unfortunately it seems I've come across a few arrogant paramedics over the years.
FYI: I have worked in the medical field from 1973 to 2014, most of it paid,but a good amount unpaid. That did not change my care and treatment of patients and the public. So in the end here, is the fact a person unfortunately died 😕

4:41 That was my first impression, I think to do that kind of damage to that particular bumper, your looking at 50+ mph speeds, and I believe there would have been damage to the grill with a lot of blood splatter. There is no damage to the license plate, so im thinking the plate was put on after that bumper was damaged.

Looking at the front of that van tells me it was a HARD hit, maybe with some speed involved.

Why is her shoe missing off her foot? Looks like she was fighting with him and in such a rage she ignored putting in her shoe. Maybe she threw it at him. If her shoe is still at home you knew there was a fight there. Maybe the shoe was found in the parking lot. So sad, it was a bad idea for him to walk in the open space. Did she follow him and hit him? Pedestrian like road rage? Lots of questions.

I got my CPR/First Aid certificate about 6 years ago. Even I who has less to no experience can see that technique of the first guy isnt even the correct way to compress. WTF!??

That man was NOT doing chest compressions correctly, that in its self could have caused this death. When seconds matter & a life is in danger every one of those first responders should have immediately stopped that man and corrected what he was doing or replaced him with someone that will do CPR correctly immediately!

Rhonda Clark ... telling you how to do it won’t help. You need to physically practice doing it to learn it properly. I’m sure there’s probably available CPR courses in your area. They are very beneficial to take.

I'll add - Old Town, and the Midway district, are total dumps because of the Trolley/Bus transit center located there. It's full of homeless transients, drug addicts, drunks, people w ankle bracelets on going to their court ordered work release programs and mentally ill people. I've worked down there for 20 years and you have to step over human feces on the sidewalk, garbage everywhere, broken glass, used needles, and get crazy people hitting you up for money every block or trying to run some stupid scam on you. I highly recommend checking it out if you'd like to feel better about the neighborhood you currently live in.
They are still humans of which many fell down on their luck and got stuck. Take time and talk to some of those who you've described. I promise you will be shocked to find doctors, lawyers, business owners, teachers...somebody who was once just like you!👍🙏

At one point in my life I couldn't use an ATM because it only gives out $20 bills but I didn't have $20 in my bank account so I had to go inside the bank. Kind of embarrassing. I hand out cash to people that are in need. I just wish they could clean the place up. The city seems to not care.

I have worked in EMS as an EMT for 15 years and the way CPR is being done by that first guy is absolutely disgusting. I have never seen anyone in EMS lift their hand off the chest and only leave his finger tips on the chest. What kind of CPR is that?
Yes, the hands should be 1 on top the other, interlocked and should not lift off the chest.

Just to point out. Being cpr trained, no matter how many years you've done it. doing it on a resusi Anne dummy in a class is totally and utterly different than doing it on a human being, in a street, after the patient has been run over by a van so his chest has been pretty much destroyed! Why do it? Because when we tell a patients family "we gave our best efforts but unfortunately your loved one died" we mean it. It's not something we say for fun. No one does cpr for fun it's really hard. Stop judging a situation you were not personally at. Be grateful someone is willing to do such a traumatic job. Be thankful instead of judgemental.

@@InobuZ I've literally seen this method in the field and in a CPR class. He's compressing with the "heel" of his hands while keeping his fingertips on the chest. The point is compressions that are the right depth and with good rhythm and he's doing that.

Only the female paramedic was giving the victim the correct CPR. The female homicide detective was intelligent and well spoken. The victim and the perpetrator should have separated long ago, before things got out of hand.

I've done many first aid, cpr courses, and I'm completely disgusted with the way these first responders are doing CPR- chest compressions. Its Hand over hand, arms, shoulders, wrists locked shoulders straight over hands, using your full body to push, directly over the heart.
Pushing on someone's lungs stomach doesnt do shit.

@patrick dean looks like good CPR to me. I went into cardiac arrest last year and two police officers showed up and beat the hell out of my chest while AED machine arrived. Hurting chest for about two months or more but here I am alive and well thanks to all of them who helped. Sometimes ribs break, lucky for me mine didn't.

Did you hold your portable MRI up to the computer monitor to take a picture of the chest? The second and third providers provided quality CPR, but the first needs to go back for training. And before you start whining about have I ever done compressions on someone in traumatic arrest, the answer is many times. I'm an active paramedic and AHA instructor.

I know they hate to lose one, but sometimes they don’t get enough to work with. These guys worked there asses off trying to get this man back with us, but in many vehicular accidents, the internal injuries are simply too excessive.

I was wondering while watching this whether there's training given on how to do CPR on someone whose rib cage is so badly damaged. It's that something they teach? Are there recognised techniques? Or would they just have to kind of wing it based on the knowledge and experience they do have? Seems like a very difficult situation.

I think it’ll take a thorough investigation to determine whether or not a crime has been committed. If so, I hope she is held responsible. I’m still waiting for the lady who killed my friend and took off to face charges. It’s been over a year and the detectives are telling me to file a request for the records. I can’t remember the proper name. My friend was hit by her vehicle and it would have been just an accident but she took off making it a hit and run. She basically called the police and confessed.

I felt she was giving some sort of performance? I've been around a LOT of accidents, and quite a few deaths. Her behavior was certainly interesting.

Your "NOTE" is irrelevant. There is no excuse for the poor CPR. NONE. And I'm no "armchair medic".
I was a paramedic (and a certified AHA CPR instructor) for 35 years (retired 2020), and that's the WORST CPR I've ever seen performed.
Shoulders should be directly above and in-line with the sternum, elbows should be locked, heels of the hand should never leave the chest wall....he's bouncing up and down, all over the place, like a trampoline.
Broken ribs are NO excuse. Poor CPR is no CPR. That's NOT to say the patient would have survived otherwise (less than 2% of prehospital trauma codes survive), but that's still no excuse.
If there are so many people commenting on the poor quality of the CPR that you felt the need to add a note providing an excuse, then maybe you should just accept the fact that the CPR sucked. The fact that they're noticing at all shows they at least know the proper way to do compressions. The few who are criticizing the comments obviously know NOTHING about the dynamics and goals of CPR.

It already breaks ribs to do CPR. As a nurse, I've broken a few ribs doing CPR. If there was no rib cage, Jesus. Kudos to the responders for trying.
