Chris/Laraine - Caught your comment on the recent Live about do I know about any maintenance clinical trials (btw, I can’t watch every Live or vid, so expect cricket chirping from me much of the time 🦗). Short answer, there doesn’t seem to be much that’s helpful. There’s two types of maintenance, far as Lilly and Novo are concerned: Maintenance WITH a GLP-1 (aka still taking it), and maintenance WITHOUT a GLP-1 (tapering off and eventually discontinuing). The first kind of maintenance, Lilly and Novo are HAPPY to run trials on (and have), ‘cuz the results show the same (and good for their business) thing: Most ppl who go OFF of a GLP-1 gain most of the weight back. 😢 But, trials about tapering and discontinuing? There’s little financial incentive for them to run those. The few such trials I’ve heard about are things like one run on discontinuing Saxenda (not sure if Novo actually ran it). It measured what happened if you exercised during maintenance vs if you didn’t. The Captain Obvious Result: Yeah, ppl who exercised after discontinuing Saxenda regained a lot less weight (but still some). There’s also an interesting trial Weight Watches just began. It’s studying if going to Contrave (or Contrave plus Metformin) after disco’ing a GLP-1 can be effective at stopping the weight regain. Sorta makes sense, as Contrave is far cheaper…. but hey, you’re still on a weight-loss drug. 🤷🏽 Finally, our friend Alli over at FitFlavorFun has talked about how there’s evidence that ppl who decrease dose very slowly and take 6-12 months (minimum) to taper off a GLP-1 have much better success rates (aka avoid catastrophic weight regain). If you’re interested in pursuing what is obviously a hot and relevant corner of the topic, maybe you could invite her over for another discussion. 😊

I took four 2.5 mg Mounjaro shots and now today I took my fourth 5 mg shot. I've cut out carbohydrates and sugar almost entirely. I'm drinking coffee, water, and tea. Today I'm down 38.8 lb! I had something cool happen. My doctor wrote me a 3-month prescription for 5 mg and I picked it up last week. CVS asked me if I wanted them to contact the doctor to refill the prescription, because it had 0 fills left. I said yes, of course. My doctor's office called an hour later or so and asked if I wanted to try moving up to a 7.5 mg dose. I said yes, I'll try that after I finish the 3 months of 5 mg I just picked up. And then they ended up sending in the prescription for the 7.5 mg minutes later. So on the same day from the same CVS I picked up three 5 mg boxes and one 7.5 mg box! Now I can pick up another box of 7.5 mg on November 11th and potentially a 3-month supply of 7.5 mg in December. We met our out-of-pocket maximum so I've been paying $0 for all of these boxes.

I think picking a goal weight is a very interesting and difficult topic. I'm actually in the process of rethinking mine. When I started with 208 lbs I picked 148 lbs as my goal weight. One reason was that once in my adult life I actually reached that goal and felt really good (even if for me this means a BMI of 27.2 which still means overweight). The other reason was that I -in no way- could imagine to possibly loose more than 60 lbs. Now I've lost 50 lbs and keep thinking: Should I go for a "normal" BMI? Or should I go for a "normal" abdominal circumference? Or at least for 140 to be save to stay under 148 in maintenance...? Well, right now, I decided to go till 148 lbs and then maintain for some weeks to see how I feel... Is it just me or or do others also know these thoughts once one realizes that one is close to goal weight?

It’s my first one, am NOT lookin’ forward to it. 😬 Took my last shot 10 days before Procedure Day. My doc’s guidance, like yours, was no shot for 7 days before, but I worry that’s insufficient, esp. if one is on a higher dose (I’m currently on 10). But the interesting thing that no one ever seems to talk about - How many days AFTER the procedure ‘til you should start taking your GLP-1 again? 🤷🏻♂️ I mean, c’mon… the colonoscopy kinda beats up on your GI-tract, especially if they find and remove polyps (IOW, there’s some bleeding). And, what else can be a bit rough on the GI-tract? GLP-1s. Fortunately I have one weaker pen left in my arsenal (a 7.5), but it’d be nice to know how long to hold my fire. Guess that’s a message to send to my doc real soon. 🤔
CD: excellent question - I had one a few months ago on Tuesday and took my shot on Friday. I had a polyp removed so there was a bit of blood. Not sure if I did it right but everything was fine

We don’t earthquakes, tornadoes or volcanoes and are three -4 hours off the coast so we should not have hurricane issues. But of late they have been showing up. The last one travelled 9 hours over land to get to western North Carolina
I get mine from lavender sky. They have different tiers. I ordered tier one which was 2 vials of 20mg for $440. They say I can order again in a month. Their BUD is several months away. My order was taking a while so I called the pharmacy direct (South Lake) and the lady said that it was still processing because they were so busy. But she said if I was running low she could put me at the top of the list. I said yes. That was on a Friday and I got it that Monday morning.
So you just spoke about the weather in Phoenix. It’s been hot here in Central Texas, too. We are in a severe drought and lots of warmer climate issues. So I agree. Who wants to live in heat? 😂 You live in a beautiful part of the country!
Hi Laraine, I was wondering if you lost a significant amount of weight when you went down in dose after being on a higher dose? I am on 12.5 but wanted to use up a box of 7.5 my first week I lost almost 5 pounds which was the most I’ve ever lost on this medication in a week. Also, I’d be interested to know how you do 2 weeks without the medication because I’m thinking about moving to every two weeks, I have been able to hold on to my health insurance but it will definitely end Nov 1st. I will have enough to last me until January, longer if I go every two weeks.
When i lowered my dose I lost less than 1 #/ week. I’ll be off for 2 weeks, but that involves not eating solid food for 2 days, and colonoscopy prep🫣. I’ll let you know!
In my weight loss history which is every diet in the planet, the body wants to gain back what it lost at some point In time , no matter what type of diet , and I am curious what your opinion will be , if at some point the weight loss with a glp 1 will be the same story. Do you also feel that this is a lifetime medication? What if the compounding will never be available or unattainable? And private insurance is unattainable for almost everyone as the price is not reasonable . I read 1st lie wins too!
CD: the interesting thing about these meds are they are a treatment for the disease of obesity they are not a diet. The goal in this process is two fold: to become healthier with weight-loss and to build healthy eating habit’s that carry us forward. We do believe these meds are a lifelong treatment similar to the blood pressure and cholesterol meds that I have taken for thirty years (which I no longer have to take) Nothing is guaranteed in this life and as you know no diet was ever guaranteed. So if we gain weight we will deal with it - make a video about it and go on. We are at the very beginning of this science, it will change grow and evolve and inevitably become cheaper and more accessible.
