Pair all that with From Software having an penchant to use previously scrapped data and work to fit into another project… all evidence points towards her being originally Tomoe. For a while now, I had this thought that Malenia may use leftover animations from a scrapped Tomoe boss fight.
For those unaware, Tomoe is a big character in Sekiro's background lore, a master swordswoman, whose abilities were described to be elegant, like dancing.
Malenia always struck me as too ridiculous for Elden Ring, definitely more fitting into a Sekiro setting, this paired with the fact that we never got to actually meet Tomoe (except her corpse) and the way the animations behave like her fighting style was described, kinda settles it for me.

It's pretty much confirmed that she is a scrapped boss from Sekiro (namely Lady Tomoe, which you would likely find in the never made DLC)
There are just too many similarities for it not to be the case.
*Her waterfowl dance in Elden Ring is extremely similar to the secret Spiral Cloud Passage weapon art from Sekiro.
*She only has one arm, mirroring Wolf.
*She is rotting, just like Wolf is.
*She dedicates her life to protecting an important and holy child that is on a mission to undo a godly ailment affecting their protector (Sekiro and Kuro)
*Her black spots on her body were probably originally supposed to be burn marks from the lightning of Tomoe everyone in Sekiro talked about. Her attacks seem out of place for a game like Elden Ring, hence why she is so controversial. She even uses a katana, I mean come on.

Since focus on Elden Ring was the reason Sekiro didn't get DLC when dataminers have found that there were originally plans for it, people can't convince me Malenia wasn't intended to be a Sekiro boss before being remodelled into Malenia for Elden Ring. That old version of ''waterfowl dance'' she has in the 1.0 of Elden Ring we see at 0:37 looks sooooo much like how Tomoe's watery flowing fighting style was described by Isshin. I think she was meant to be Tomoe.

For example, in this clip, you see how well you can parry her attacks but you can’t do that in Elden ring which is why people have a hard time with her just like how in sekiro the demon of hatred, you can’t Parry its attacks only dodge it, but if you were to put him in blood-borne it would probably be a more enjoyable fight because of how aggressive he is

*For anyone wondering,* the reason this fits so well (and the reason there's no DLC for Sekiro) is because Malenia and Radahn were originally going to be a DLC for Sekiro with their own areas, etc. Malenia was "Tomoe". There are several reasons why this is obvious, besides the blatant fact that Malenia is legit a Sekiro enemy (she even has a prosthetic like Sekiro and uses both the Sabimaru and Floating Cloud Passage move) - Sekrio is almost entirely inspired by Hokuto No Ken (yes, the ATATATA you're already dead anime) and each character in the game has a very clear inspiration and story arc similar to Hokuto No Ken's manga.
*Sekiro* = Kenshiro, The Fist Of The North Star
*Tomoe (Malenia)* = Rei, The Waterfowl Master
*Radahn, Conqueror Of The Stars* = Raoh, The Conqueror
*Genichiro* = Shin, The Lone Eagle
    *The Owl* = Ryūken, Kenshiro's Father/Master
*Kuro, Divine Heir* = Yuria, The Last Holy Nanto General
*To elaborate further (if you're interested), see if this rings any bells:*
Hokuto No Ken is a story that begins with Kenshiro (Sekiro) protecting Yuria (Kuro), but Shin (Genichiro) comes and steals her, believing he should be her protector. Shin (Genichiro) mutilated Ken (Sekiro) in the process, permanently scarring him. After his near death, Kenshiro (Sekiro) comes back even stronger and goes straight to Shin's (Genichiro)'s tower and defeats Shin (Genichiro) who throws himself off the tower after suffering a killing blow. Later in the story, a new, distant ally to Kenshiro (Sekiro) who goes by the name Rei (Malenia/Tomoe) and is the heir to the Waterfowl Style of the South Star battles Raoh The Conqueror (Radahn, Conqueror of the Stars) in a very dramatic battle. Raoh (Radahn) refuses to dismount from his beloved horse and Rei (Malenia/Tomoe) is forced to face him at a distinct disadvantage. Raoh (Radahn) makes a show of his power, blowing the wind in both direction and using the stars to his advantage. Rei (Malenia/Tomoe) flies in the air after and uses a devastating sacrificial move to neutralize both of them. The only difference is that in the manga/anime, Raoh (Radahn) uses the stars to see this move coming and nullfies the threat, defeating Rei (Malenia/Tomoe). This make Rei (Malenia/Tomoe) very weak and kills him eventually. However, if you played Elden Ring, you obviously know this outcomes is reversed, making Radahn weak and eventually defeated, not Malenia.
So yeah, it's clear for more than just the obvious visual and gameplay design that Malenia and Radahn belonged to Sekiro. My best guess is that it was taking them a while and they thought more players would get to experience these two characters if they put it into their latest game, Elden Ring. Which overall, was a good move. While I would have liked them in Sekiro to complete the metaphor, I don't mind it at all.

    I saw the rumours about Sekiro DLC "Lightning of Tomoe" a few years ago here and there. My guess is;
-From Software Originally did not planned a DLC for Sekiro becuase they did not believe it would be popular. After it became GOTY, they wanted to prepare a DLC but as you might guess, if it's not decided to be a DLC from early on, you need a very loooong time to finish the making of DLC or it wont be up par with the usual standarts or it wont be out at least 2 years.
-Malenia was supposed to be Lady Tomoe but after looking at all the content, they must have decided the scrap the SekiroDLC idea since there isn't much to go on but story wise or content. Instead they used her for Elden Ring.
It's interesting that a scrapped character from Sekiro becoming the most memorable boss in Elden Ring.

For anyone wondering why this is possible, it is because FromSoftware use the same game engine for most of their big games. So models, animations, and other stuff port over between their games more easily than normal. Not saying it is easy, just that it is easier than porting from a completely different franchise or media. This is also why Dark Souls 2 mods are so rare, because it is the only game among the big titles that use a different engine while Dark Souls 1, 3, Sekiro, and Elden Ring all use the same/some variation of their proprietary engine.

This working as well as it does kinda lends credence to that theory.
Her mechanics also are pretty fair in the context of Sekiro, her regeneration does basically nothing since you can just break her stance and deliver a deathblow and with Sekiro's abilitys you could deal with waterfoul and her regular attacks fairely easily.
