    I wouldn’t get them, too risky. Your face changes as you age anyway so you could end up looking odd. Thanks for the video!

      Exactly. The problem is most doctors inject directly in the skin, so when the skin descends so does the filler. It would be better if they injected on bone so that the tissues on top glide over them.

    No, never have and never will. For the reasons you state - fashion plus aging gracefully. What looks appropriate on a 30 year old face, may look totally ridiculous on a 60 year old face. Dr Gavin, just wondering if you'd consider doing a video on thread lifts if you haven't already. I ask because they're intriguing to me. I'm not convinced they really work and need an experts opinion. Thank you.

    When I first started Botox and fillers I was the demo patient for a prominent PS here and one day he called and asked if I would be available for lip filler training he was doing for some docs. I said of course! I arrived, he proceeded to do a full dental block, not questioning him at all. I was a young nursing student, he is a revered PS, who was I to question him?🙄 Then I see his Nurse bring in the tray and there was a brand (I’d rather not say which brand as it is a good product for other issues and I don’t want to turn someone away from it should it be what is the best answer for their particular challenge) of PERMANENT lip filler that he was going to use in my vermillion boarder (top and bottom) hence the dental block. As he was explaining to the PS Residents that the only way to extract this was to surgically remove it, I completely panicked and declined PERMANENT anything at my age back then. I’m glad I did as I’ve seen it surgically removed in women that have had it done and I’ve never been so happy to have said NO to a permanent ANYTHING in my life! Needless to say, he used a non-permanent lip filler, which I was pleased with but I wasn’t his “go to demo student nurse” anymore! I was 24, I’m 53 now so I can’t imagine what it would look like. I’ve only had filler in my lips twice since then as I’ve been blessed with great genetics but that was a close call I know I would have been kicking myself for had I not said no. I recently did the lip flip with fantastic results and I think I’ll stick with this (for me, it’s all I need) but there is definitely something out there for everyone and I have seen some fantastic permanent filler work done here with great results (on friends that are going on year 5 & 6) but it’s definitely a very serious discussion with your PS. I can’t stress enough to people that if you see a “sale” or a “coupon”, that’s NOT the Doctor for you. The louder the advertising, the worse the doctor and I’ve yet to be proven wrong on that. Don’t be afraid of permanent fillers, just do your own due diligence and have a VERY candid and serious talk with YOUR doctor. Read the waiver you’re signing. Only after all of that can you make a truly informed decision. (A ticket to Australia would also be a great start, erhem)😉👍🏻

      I got permanent chin fillers It's been 3 years now. I'm quite satisfied so far, Hopefully it wont look weird or something in the following more years to come

      I had rhinoplasty a couple of years ago and my surgeon told me that permanent filler would be a good option to fix the defect from the surgery (it's a small dent where my dorsal hump was) I'm happy with the outcome after surgery except for that dent, it's not huge but I always notice it, it's pretty much what you explained. Is this something you could do for me? I live near Melbourne and would love to see you!

    Permanent filler sounds terrifying to me due to the potential risks of occlusion with absolutely no remedy. 😱 If Dr. Chan is that cautious and apprehensive about permanent dermal fillers, the rest of us less educated suckers should cower at the thought of it.

      Hyaluronidase dissolve the HA found on fillers but not that in your tissue. I could not find paper/ journals to explain how this works but presumably there is a slightly different structure found in artificial fillers that is complimentary to the enzyme (Hyaluronidase) active site compared to the natural HA.

      Search on Realself.com to see reviews about the hyaluronidase, it seems to dissolve your original skin collagen tissue as well. No matter what doctors say, trust the reviews of people who underwent that. Also doctors say that botox is safe, instead it is a poison, I tried it and it will make your hair fall, but if you don't make you inject botox again, hair will regrow again luckily. So my recommendation is to leave the ha filler to dissolve on his own during the months or the 2-3 years completely.

    I've had 3 sessions of Silikon™ 1000 in my lips and it made a difference since the temp fillers never quite took, but I was also just looking for a more natural look since I have smaller tighter lips. I think it depends on the doctor and being patient and not going crazy all at once. I truly just wanted lips where I could rock lipstick, but I did do a ton of research and went to a doctor who really understood the face and how to slowly inject me. But I have seen some scary Bellafill fillers on people.

    No matter how good your doctor is, or how highly recommended they are, complications do sometimes happen. The one time I ever had any filler was just a tiny bit in the "11s" between my eyebrows, which went into a blood vessel and had to be reversed. Also, when I was diagnosed with breast cancer the (male) plastic surgeon insisted that agree to implants. They became infected, which then necessitated another 6 hour emergency surgery 5 days later. Since then I've had another half dozen major surgeries with 2 more plastic surgeons to try to correct the painful scarring it caused, which also limited movement in my arm. The point is, complications do happen, and sometimes it's just luck of the draw.

    Yesterday I was about to book an appointment for lip filler for the first time, since finding your channel I've learned one thing, I'm NEVER getting filler. What's the point? They migrate after a few months but stays in your body for years. I wanted more symmetrical, plumper lips but even a 0.001% chance of going blind is not worth it, temporary or permanent, oh hell no

I just started this botox and fillers journey a month ago and I was considering the permanent fillers for my nasolabial folds...definitely going to just suck it up and use the temporary fillers, even though it will be more expensive.     In my mid sixties I’m more secure in who I am. That’s not to suggest I would prefer fewer signs of ageing, but I don’t lose sleep over it, nor do I feel less attractive. Ageing gracefully is alluring 🥰

    You say that permanent filler is good for nose deficits. Are they for structural changes, or are they as flexible as HA filler and able to be used in high motion areas?
    Are there different consistencies like HA filler?

      Bellafill is 100% permanent. I see a lot of doctors claiming longevity of 5 years but this is misleading. The clinical studies terminated at 5 years so legally they can only say that it lasts that long. The reality is that the acrylic microspheres will remain lodged in the tissues forever.

    I got Silikon-1000 injected into my lips and it’s been wonderful. I had lip fillers in the past that from what was explained to me my body metabolized almost immediately. After 4 attempts. I was then introduced to Silikon-1000. What are your thoughts on this in the lips ?

    I have heard that HA fillers leave around a 10% change due to natural collagen production after each treatment and this why you need less and less over the years. Anecdotal reports state for a small defect in the keystone and rhinestone areas of the nose that even one treatment maybe enough for a permanent outcome. Many people forget how they looked before HA fillers hence why many believe they are temporary. Yes you lose a lot of the product but as Gavin states HA fillers may last a lot longer than previously thought, couple this with the collagen production they create and you have to make sure you are not experiencing body dysmorphia before going for top ups. Before and after pictures should always be taken so you do not forget what you looked like!

    So far Juvederm has consistently been pushed to me by multiple injectors but I have also hated the results each time by multiple injectors the results seem permanent. I love radiesse. How do you feel about that and have you noticed the longevity of that as opposed to Juvederm which supposedly doesn't last as long?

    Would you recommend permanent filler for revision rhinoplasty? I got indents on my nasal bridge from cortisone shots to reduce healing and my surgeon suggested filler. I don’t want to because filler is temporary and I’ve seen the side effects of patients losing their nose. Will this do the same effect? Also, what is the name of this permanent filler?

    Dr. Chan, is Sculptra considered a permanent filler in that it enhances your own collagen development? I have considered something called plasmasculpt using my blood and sculptra injections. And what are the contraindications if you have presented with Graves disease but is now in remission? Are any autoimmune disorders a contraindication for any dermal fillers?       I think most of the dermal fillers are also 'collagen stimulators,' meaning your body reacts to the filler by building your own collagen.

    Hi Dr. Chan, I wish I had found you before I had permanent filler. I had bellafill in my temporal areas and under my eyes (very expensive). Not very happy with the filler. I think the filler under my eyes was too much and I wouldn't recommend it. It's not awful but I'm now trying to find a procedure to tighten my skin. Thank you for being a honest doctor!

    By permanent fillers do you mean bellafill? Are you considering Sculptra a collagen stimulator? Do you think that's okay for global volume loss?

      We don't have bellafill in Australia, but we do have a permanent filler with a similar constituent called Aquamid. Both have PMMA. We occasionally use Sculptra as a collagen stimulator in those who have a significant degree of volume loss over the whole face as you suggested.

    I beg and warn anyone reading this to please NEVER have permanent fillers. I had my lips injected with Aquamid which is a hydrogel ( polyacrylamide) and it has destroyed my self confidence and lips. They are lumpy and uneven plus no surgeon wants to risk removing the filler. My only hope is to find out if I can have a non permanent filler to disguise the mess.

    I’m kinda interested in getting temp fillers for my cheeks, nasal fold, chin, and jawline. I’m dying to know WHERE exactly are the fillers injected (dermis layer, hyperdermis, adipose, etc)? And where does it go over time? Does it dissolve? does the gel/filler harden over time? And does it float over time? Like if I get filler in my jawline, can it float or move down towards my neck for an example? 🤔

      lizitis Filler are injected quite superficially into the dermis and do not tend to migrate but can if it is injected into an area where filler is already present - it will then move down a pressure gradient to a “more empty” area. It can also move if it’s manipulated.

    You say normal fillers last way much longer than supposed to. Well, I had them in my nasiolabio area and after 1 year the wrinkles returned. Maybe there was still some left, but I don't think the effect lasts so long. Can you make a video on that?

      There is a video on how long fillers last for on our channel. We use MRI scans to prove the longevity of the filler. I recently actually saw some nasolabial fold filler on an MRI after 7 years btw!

      Actually based on that video, it could be likely that some filler remains but that it has migrated out of the fold and into the surrounding area.

    Dear Doctor. Following the scans youve shown whereby filler lasts far longer than what is understood by most, is there a way to prevent migration/dispersion of the filler? Or would it be best to do individual scans, locate the filler and dissolve it, before admimistering more filler? Hope to hear from you ☺

      Thank you for your question. I don't know if there is a way to prevent migration or dispersion of the filler. It just happens. Also I would not advocate doing MRI scans every time prior to filler. We can just look and assess the face and see what needs to be done each time. We must have an aesthetic goal for each treatment, and not just do treatments for the sake of putting in more filler.

    Hello, I've had temporary fillers more than 5times to my nose and I want to have a nose job. Is it possible to not use the dissolver? The fillers are very close to my eyes and I'm pretty scared. What do you say?

    I have seen several of your videos including the one about the mri and that temporary fillers last 4 years. What confuses me is that even though the filler is still left in the face or in the lips, it has moved. Therefore it does not give you the benefit of why you put it in your lips or face. Or am I mistaken and it's just because we get used to the filler and think that it has deflated?

      We have found temporary fillers (hyaluronic acid fillers) to last often beyond ten years in many cases. This is based on MRI evidence. I like to use the analogy of a pizza dough with fillers. You start with a ball of pizza dough and you press/roll on it and it becomes flatter and spreads out. It's still there in its entirety, but it's not giving the 'lift' that it gave initially. We see this problem with areas such as the lips, where filler migrates out of the vermillion or red part of the lips. Also we see it with nose filler, where it doesn't give height to the nose after some time, but instead makes the nose wider.

    Doctor my face is so skinny even if i gained weight, it makes me look unhealthy or older and i was considering cheek fillers but i don't know anything about it or what is the best option.     I did temporary filler on my cheeks and lips and I got delay reaction after 5 months. I was getting lumps on my lips twice a week and I had big lump on my cheek and was painful. I was on antibiotics and cortisone pills for two weeks and after finishing my medication I was getting the lumps back. I was getting tired of it and I decided to dissolve it. I will never do it again.

    Dr.Chan, I had lip augmentation 18y ago with Aquamid and Bioalcamid permanent filler, 4times. Nothing happened there, but I need correction after years- lips looses volume, its uneven. Can we fill it with hyaluronic acid? Can you help

      Personally, I wouldn't do it as introduction of another filler may cause an infection or reaction with the existing filler.

      Sounds like you may need removal of the filler. After removal, you can consider getting more filler. But I agree with Dr. Chan in not combining fillers.

      sorry, can't help you as we don't have prx-t33. Chemical peels are only for resurfacing. That is, they will improve skin texture and tone, but not tighten skin

    Dr Chan, could you please advice? I have tried several lip fillers in the past such as Juvederm, Teosyal, Belotero, Radiesse; as well as my own body fat and PRP. I always got a very natural looking and satisfying result in the first place. But unfortunately, after only +/- 7-14 days (yes, that's right) the fillers seems to vanish completely. My surgeon says I am really an exception as far as the longevity of the fillers is concerned. My question: did you have cases like mine, and are there other options (perhaps not hyaluron based ones) or new techniques/products that could work for me? I've Googled a lot about it, but didn't find any good advice here in Europe. many thanks in advance!

    I had two syringes of Bellafill injected prior to knowing it was permanent and the horrible side effects. My dr told me it would dissolve into my body but I’ve read it never does. She still stands by what she said. I’m terrified now and wish I had seen this before she talked me into it. She didn’t even do the allergy test which would have given me more time to investigate. Did it same day of consult.

    After many, many years of having temporary fillers, my body rejected them JUST LIKE THAT! I developed infection and horrible red blisters at the sites (cheeks). My doctor did use the filler dissolver but I was left with scarring and indentations which I am still having treated today FIVE YEARS LATER!

      I can't tell you for sure, but it sounds like that your 'infection' was actually a vascular complication - that is the filler may have been injected into an artery and blocked off the blood supply. Which area was it injected into? When did the redness start appearing?

      It's safe, but the problem is when you inject HA into your breasts, it could interfere with the imaging on mammograms. So heaven forbid the filler causes the doctor to miss a tumor.

    I've had multiple sessions and many ml of permanent fillers in my face, since I'm HIV-positive (30 years now) and had a lot of facial lipoatrophy and butt lipoatrophy. I had a lot of Bio-Alcamid product, as well as New Plastic, and other PMMA fillers in many sessions since 2001. Despite it being "permanent", it's "gone away" every few years, but I think it migrated a bit to over-fill the "apples" of my cheeks. Had it in my temples, cheeks, and jawline. I'm over-corrected in my cheeks. I'm a 55 year old white male. Wish I could consult with you!

      they are not considered to be permanent fillers. They are temporary hyaluronic acid fillers. But one thing we have found is that these fillers often last a lot longer than we think. See the other videos on our channel

      There are many causes of this, including the type of filler used, but one big factor I believe is the introduction of bacteria at the time of injection.

      Look into Sculptra. Find a good injector build collagen over time sounds like that's what you need very natural. It's expensive but worth it.

    Fat transfers are not a good idea. Your body will only take a certain percentage of the overall fat transfer. Then you gain weight and lose weight throughout time. Plus your still at the mercy of the doctors knowledge of placement.....and it’s permanent!!!! A plastic surgeons wife that I knew had all of the fat transfer take around the ocular region of her eyes. It looked terrible and took months and months of injections attempting to dissolve the fat and eventually surgery. Then let’s consider who is going to do the surgery to remove it if things go south or you gain 30 pound through menopause or something??
    Too many variables.
