I've gone to my derm 3 times to try to fix my nasolabial folds. I have 4 ml total in my cheeks (foreward and lateral) and have had minimal improvement. But in this case, I can't tell if it's poor placement, or if fillers arent the solution at all.

This video was so informative. I’ve been seeing a dermatologist for a few years who has been using fillers on me for lifting and wrinkles and I’ve had virtually no improvement. I should have saved my money for a facelift.
I had a face lift with liposuction on my neck 7 years ago.....it was the best thing I’ve ever done. I’m 63 and have no jowls or sagging. I would absolutely do it again!
I had filler for wrinkles below my mouth. Total waste of money. Microneedling has been more helpful for superficial wrinkles. Great info here. Lot of doctors are using filler as a facelift procedure and people are walking around with balloon faces.

So true. I see people commenting all the time that if they had money they'd look like Jlo. The reality is that it's good work and genetics. Surgery and injectables can only do so much before you start to look crazy. A lot of it is actually lifestyle and genetics.
Nice, honest, informative video. Fillers can be a good thing and a bad thing. I think it's best for a woman to accept that the face will NOT be perfectly symmetrical, that's just not normal. I have nothing against fillers, I have a bit of filler in my face, but as in all matters; moderation is the key. A face filled with a lot of filler is very noticeable.

I'm learning so much from you. You should do Skype consults on what would be best (Botox vs. Fillers). Are you a dermatologist, plastic surgeon, aesthetician? Guilty of possibly wanting fillers with the wrong expectations...how long is Botox supposed to last? I've only had it once about 5 months ago, but am going tomorrow for a touch up.
What I’ve noticed as for me as a almost 50 year old. My diet , water ‘ micro current device , using non chemical skin care and exercise has improve my skin and preserve it a bit longer. I did loose loads of weight so I did notice my face did sag a bit but if I didn’t take of my skin way back when I was 17 I think I would of look like a raisin :). What do now for that is my micro current and cupping. In my opinion is what you put inside your body. Eat healthy and don’t forget your sun screen :). I have medium- thin lips but a very heavy Cupid’s Bow. I’d like to see a treatment of that. I love my slight eye bags, and really feel quite happy with my beautiful 45 year old face, with just my glabellar area relaxed, however my pointy top lip has always been quite irritating. I’m in the uk and had dermal fillers for marionette lines. I had 2 ml and then another 1ml, he final 1ml was injected above my top lip. This was done by my dentist and cost almost £500. That was done 15 months ago before I had properly educated myself. Presumably It must have made some difference ? 2 months ago I had cheek fillers. The practitioner wanted to use 2 ml, 1 ml on each side, but I asked for just .5ml in each cheek and was very happy with this because I could see a difference but no one commented! Profhilo is quite popular but I’ve not seen any independent reviews on this.

E H I had 2 treatments of Fraxel many years ago . It did nothing for my skin , let alone my wrinkles . It was painful with considerable down time and not cheap . Fraxel probably has improved by now . I’m not keen to try Fraxel again although some I’ve seen get good results for pigmentation . Chemical peels are less expensive than laser but of course always down time to these treatments and of course a risk . My friend used an excellent Sydney Australia specialist . Reliable Recommendation is a major consideration . Cheers 😊
Older Women Rock . Fashion for Over 50 what about plasma fibroblast? Hearing good things. Friend of a friend had it done and she said it was amazing on tightening upper lower lids. She said is was red for quite a while though.

My observation has been: as you age, not only you lose volume but gravity also stretches the skin on your face .. when women put fillers on stretched skin (especially on older women), you have this balloon filled fat face affect (looks nothing like what you use to look like) and it looks strange and unnatural.. like this video suggest laser or face lift (skin tightening makes more sense and early retin- A.
There is a filler in Europe and the Americas called Profhilo which is a structurally different HA compound and injected all over the face for all over volume. It has great reviews, like everything we are so slow to get it in Oz.
Fillers aren't great for the upper lip. But if you have only mild lines or the beginnings of lines it can be a hard decision whether to have laser or just wait until it gets worse

This is really helpful. Im getting cheek and forehead (hopefully) filler in 2 wks and i just want it for volume loss, however there was a small part of me that was hoping it might slightly lift my slight jowls. Im 38 now and always had full cheeks, but over time and some weight gain and loss my upper cheeks have lost volume and i have some slight sagging with a jowly look. I hate it because my jawline bone structure is very v shaped and narrow, but it looks more rectangular due to my fat lower cheeks. Its like my cheek fat moved lower on my face sorta. Im currently 5'6 140lbs and when i was down to 120lbs (too thin) I didnt have the fat in my lower cheeks. But that was 10 yrs ago so now that i know my facial skin is more lax im afraid losing the fat in my lower face will make my skin there even looser. Im prob making myself sound worse than it is, but it's something that really bothers me. Im very self critical, but at the very least at a lot of people (patients) tell me I look a lot younger than I am. So i guess i have that. As far as my cheeks go i have high full cheek bones, but the apple area is what im concerned about. When i smile that area looks good and full, but when im not smiling it looks a little too flat and slightly aging to me. And because of that I'm starting to see the nose to mouth lines. As far as my forehead I've notcied fat loss as there as well. Especially toward the lateral aspects and a slightly above my eyebrows. (Ive notoced it in my mom so i think its genetic.) Again i think it looks really aging so im hoping they're comfortable injecting filler there. Wish i could come see you! Would love to have your expertise. Love your channel btw!!
Idk doc fillers improved my smile lines drastically. I went from having deep smile lines to hardly noticeable fine line on one side, and the other side completely smoothed out. Maybe it could be because my lines where caused by volume loss ?
Or your skin could’ve been dehydrated. Hyaluronic acid attracts moisture. I notice I have more visible lines in my face when my skin is dry like during the winter or after heavy drinking
i lean oh no my skin is always hydrated, I make sure of that. Lots of hyaluronic acid serums, squalene oil , moisturizers with HA and ceramides. This was a super deep wrinkle, not a fine line from dryness. I also notice my skin looks worse when it’s not hydrated but this wasn’t it
Juvederm was always advertising the “soft lift” as a non surgical option for a face lift using 6-10 vials of filler. Something told me just no. I had 4 at one point and even then I wasn’t convinced another 6 would give me face lift like results. I also found injecting directly into the wrinkle went out of favor for this “soft lift” but I found it useless. I finally insisted on having the filler injected directly into my smile lines and it worked perfectly and lasts for 9 months. I remember thinking just how many vials of filler are needed in the cheek area to smooth out fine lines. I’d look like an alien. 👽
Thanks for your channel. If I have fillers and what I see at that moment after the procedure, that is not the real result correct? They will absorb water and will be puffier? So less is more right?
What do you think about threadlifts? If you do a facelift with threads because you feel that you have some nasolabial folds etc but not serious enough to have surgery and you are too young to have it.

lana Save ur money it doesn’t work. I had no acne but I was interested to try micro-needling with PRP and all it did was cause me to have an allergic reaction called dermatitis on my face. Little teeny tiny flesh colored bumps. After 6 months it’s finally completely gone, and my face is back to normal. I also had PRP injected to my tear troughs, it’s supposed to help regenerate your own collagen. And it seemed to look nice for the first month or so. But not worth the $1000. So needless to say none of these things worked for me. But I didn’t have much to fix in the first place. I think micro-needling is good for ppl with acne, my friend had it done and her face seemed to clear. But what doesn’t work for one, may work for another. So if you really want to give it a go!!

Hi. I had lots of filler over a 10 year period in my top lip (mainly) which I stopped more than 4 years ago and it's absolutely still there and, like you've pointed out in your video, (great, honest and informative video) the fillers have travelled upward, and thanks to this video, I now want it dissolved. I can feel what resembles a long tube of thick rubber along my lip line when I feel my lip. I'm worried that I may end up looking worse than I do once it's gone. Will it take more than one attempt? Do you have any advice? Once I have this old filler dissolved, can I get a new small amount injected on that same day or how long will I need to wait if I feel the need to do that?
I had old filler removed and ended up with loads of lines really deep it was awful. I went and had Dermapen and it helped. I then stupidly went and had some more filler and ended up with a ledge in the ‘moustache’ area. It was dissolved and It didn’t help the ledge look unfortunately. I’m scared to dissolve it as doctors say there’s so filler there. Need an MRI scan! My goal is to just go for endymed Intensif and have no more filler as it def migrates.

Thank you for your great video I have one question for you I have been practicing Botox for 10 years I have applied to myself 10 years before for the first time but for years before after applying 30 units Of Botox Allergan and exposing to son one day after I have got an Alergy rash on the skin on the exposured parts I took medicine steroids and antihistamines for ten days And that has been better. After that I applied it for several times without sun exposure and everything was ok. But last time in April last year I put it again 40 units of disport and went out in garden to work. One day after that my face swelled and I got red rush on the exposed skin. Again I’ve got steroids intravenously and antihistaminics for two weeks. I have a jowls, strong marionette lines and drooping cheeks but I'm only 29 and feels a bit wrong to have a facelift this young. Would filler be an option to get a small lift or is filler equally useless in lifting regardless of age or severity of droop?
There is one area where fillers "lift" : the upper eyelids! Dr. Gavin, do you believe it is safer to fill this area with hyaluronic acid based fillers or micro fat grafting? My only chagrin with fat grafting to the upper lids is that I've seen videos of doctors vigorously injecting with the same back and forth force as if they are doing an aggressive liposuction procedure- only they are doing this around the delicate eye area! Yikes. Shouldn't they be administering the fat grafting in a safe and SLOW similar in manner as you described is appropriate for fillers? They are just yanking a big needle furiously back and forth into the eyebrows, upper lids, cheeks and temples. Why do they do that so fast? What is the rush? Surely, an artery will get punctured and punctured hard if they happen to hit it. Why don't they inject fat grafts to the face slow and gentle like they do with hyaluronic filler? Is there a reason for the vigorous swishing?? This makes me fear that fat grafting is a more dangerous procedure since arterial occlusion seems much more likely.
Doc I had skin tightening in a few sessions with laser from a laser clinic which was heat based I was told could help with wrinkles and tightening . I had no idea it would destroy my face fat and was not made aware of that risk. I am now left with very saggy marionnetes and neck on neck and face has really affected my confidence. Is a face lift my only option? I am 43
I’ve had various fillers, botox and two erbiums. I think age is important for these issues.. the filler has helped with some mild sagging/creases, but I’m in my 30s.. I know at some point I will have to put the filler on hold and get a lift and I definitely would not look how I do now without the erbiums I had done
tuffstuff Who said “so much” ? I did not say how much filler I get. I have never had more than two syringes of filler injected in my face which is enough to correct the mild (as I stated, mild issues) I have, and sagging is determined by genetics. Some people get more at an earlier age than others, and if they have experienced weight loss in the face. You don’t know what you’re talking about.
Get both sides (the pros and cons and potential problems) from laser reviews on Realself.com before doing lasers or any high energy device on your face. Just a word of caution.
Yes but if it is just that area then fillers are much easier. You can't reverse fat like you can with filler, so generally it is always under treated even more so
I've never had fillers (though I considered it) but I somewhat disagree. Fillers are great at doing some lifting. For example, they can lift the tip of your nose. Also, if injected in the upper cheek above the nasolabial fold they beautify the eyes by lifting the horizontal base line of the eyes giving a more "stable" looking eyes and a nicer reflection of light. Another possible lifting capacity is enhancing the jawlines which induces a lift like effect on the neck.
I am generally not a fan of threadlifts. I did them for about a year and ended up with a few complications (including puckering of the skin). Worst of all , they don't seem to work or last.

Would pan facial volume loss be the result one might see from significant weight loss as well? I’ve lost 70 pounds two years ago at the age of 44 and my face looks very different to me. I am seeing a cosmetic surgeon next month to talk about fillers or whatever he recommends. What do you usually recommend to patients who lost volume from weight loss?
I had a dermatologist who lost 150lbs and her face looked so different afterwards. Though to be fair she had a lot of fat in her face before the weight loss. She ended up injecting a LOT of filler to her face to help compensate and it did look a lot better.

Great talk, but the BEST results can only come from GREAT injection technique. Unfortunately, as you suggested, lots of injectors inject in places that are completely INAPPROPRIATE. I think you can get a very nice life from fillers. If you have tons of lines, then FACELIFT is your only option, but you will not look the same. You will look improved, but not the same.

Fillers should be done at bone level in order to avoid puffiness, migration and the Tyndall effect (bluish didcoloration).
I wish there were more honest doctors like you, as some are just interested in how much money they can

My observation has been: as you age, not only you lose volume but gravity also stretches the skin on your face .. when women put fillers on stretched skin (especially on older women), you have this balloon filled fat face affect (looks nothing like what you use to look like) and it looks strange and unnatural.. like this video suggest laser or face lift (skin tightening makes more sense and early retin- A.
There is a filler in Europe and the Americas called Profhilo which is a structurally different HA compound and injected all over the face for all over volume. It has great reviews, like everything we are so slow to get it in Oz.

Fillers aren't great for the upper lip. But if you have only mild lines or the beginnings of lines it can be a hard decision whether to have laser or just wait until it gets worse

This is really helpful. Im getting cheek and forehead (hopefully) filler in 2 wks and i just want it for volume loss, however there was a small part of me that was hoping it might slightly lift my slight jowls. Im 38 now and always had full cheeks, but over time and some weight gain and loss my upper cheeks have lost volume and i have some slight sagging with a jowly look. I hate it because my jawline bone structure is very v shaped and narrow, but it looks more rectangular due to my fat lower cheeks. Its like my cheek fat moved lower on my face sorta. Im currently 5'6 140lbs and when i was down to 120lbs (too thin) I didnt have the fat in my lower cheeks. But that was 10 yrs ago so now that i know my facial skin is more lax im afraid losing the fat in my lower face will make my skin there even looser. Im prob making myself sound worse than it is, but it's something that really bothers me. Im very self critical, but at the very least at a lot of people (patients) tell me I look a lot younger than I am. So i guess i have that. As far as my cheeks go i have high full cheek bones, but the apple area is what im concerned about. When i smile that area looks good and full, but when im not smiling it looks a little too flat and slightly aging to me. And because of that I'm starting to see the nose to mouth lines. As far as my forehead I've notcied fat loss as there as well. Especially toward the lateral aspects and a slightly above my eyebrows. (Ive notoced it in my mom so i think its genetic.) Again i think it looks really aging so im hoping they're comfortable injecting filler there. Wish i could come see you! Would love to have your expertise. Love your channel btw!!
Idk doc fillers improved my smile lines drastically. I went from having deep smile lines to hardly noticeable fine line on one side, and the other side completely smoothed out. Maybe it could be because my lines where caused by volume loss ?
Or your skin could’ve been dehydrated. Hyaluronic acid attracts moisture. I notice I have more visible lines in my face when my skin is dry like during the winter or after heavy drinking
i lean oh no my skin is always hydrated, I make sure of that. Lots of hyaluronic acid serums, squalene oil , moisturizers with HA and ceramides. This was a super deep wrinkle, not a fine line from dryness. I also notice my skin looks worse when it’s not hydrated but this wasn’t it

Juvederm was always advertising the “soft lift” as a non surgical option for a face lift using 6-10 vials of filler. Something told me just no. I had 4 at one point and even then I wasn’t convinced another 6 would give me face lift like results.
I also found injecting directly into the wrinkle went out of favor for this “soft lift” but I found it useless. I finally insisted on having the filler injected directly into my smile lines and it worked perfectly and lasts for 9 months. I remember thinking just how many vials of filler are needed in the cheek area to smooth out fine lines. I’d look like an alien. 👽

lana Save ur money it doesn’t work. I had no acne but I was interested to try micro-needling with PRP and all it did was cause me to have an allergic reaction called dermatitis on my face. Little teeny tiny flesh colored bumps. After 6 months it’s finally completely gone, and my face is back to normal. I also had PRP injected to my tear troughs, it’s supposed to help regenerate your own collagen. And it seemed to look nice for the first month or so. But not worth the $1000. So needless to say none of these things worked for me. But I didn’t have much to fix in the first place. I think micro-needling is good for ppl with acne, my friend had it done and her face seemed to clear. But what doesn’t work for one, may work for another. So if you really want to give it a go!!
I have a serious problem. Doctor, how do you remove failed filler like granuloma beneath my skin? I have consulted numerous doctors, many of them advise incisions are required, but risk of scarring is prominent. Please help! What methods do you use? Perhaps a video to explain?

I had lots of filler over a 10 year period in my top lip (mainly) which I stopped more than 4 years ago and it's absolutely still there and, like you've pointed out in your video, (great, honest and informative video) the fillers have travelled upward, and thanks to this video, I now want it dissolved.
I can feel what resembles a long tube of thick rubber along my lip line when I feel my lip. I'm worried that I may end up looking worse than I do once it's gone. Will it take more than one attempt?
Do you have any advice? Once I have this old filler dissolved, can I get a new small amount injected on that same day or how long will I need to wait if I feel the need to do that?
I had old filler removed and ended up with loads of lines really deep it was awful.
I went and had Dermapen and it helped. I then stupidly went and had some more filler and ended up with a ledge in the ‘moustache’ area. It was dissolved and It didn’t help the ledge look unfortunately. I’m scared to dissolve it as doctors say there’s so filler there. Need an MRI scan! My goal is to just go for endymed Intensif and have no more filler as it def migrates.

Thank you for your great video I have one question for you I have been practicing Botox for 10 years I have applied to myself 10 years before for the first time but for years before after applying 30 units Of Botox Allergan and exposing to son one day after I have got an Alergy rash on the skin on the exposured parts I took medicine steroids and antihistamines for ten days And that has been better. After that I applied it for several times without sun exposure and everything was ok.
But last time in April last year I put it again 40 units of disport and went out in garden to work. One day after that my face swelled and I got red rush on the exposed skin. Again I’ve got steroids intravenously and antihistaminics for two weeks. Now I am in serious doubts should I put a Botox again thank you for your answer in advance

I have a jowls, strong marionette lines and drooping cheeks but I'm only 29 and feels a bit wrong to have a facelift this young. Would filler be an option to get a small lift or is filler equally useless in lifting regardless of age or severity of droop?
There is one area where fillers "lift" : the upper eyelids! Dr. Gavin, do you believe it is safer to fill this area with hyaluronic acid based fillers or micro fat grafting? My only chagrin with fat grafting to the upper lids is that I've seen videos of doctors vigorously injecting with the same back and forth force as if they are doing an aggressive liposuction procedure- only they are doing this around the delicate eye area! Yikes. Shouldn't they be administering the fat grafting in a safe and SLOW similar in manner as you described is appropriate for fillers? They are just yanking a big needle furiously back and forth into the eyebrows, upper lids, cheeks and temples. Why do they do that so fast? What is the rush? Surely, an artery will get punctured and punctured hard if they happen to hit it. Why don't they inject fat grafts to the face slow and gentle like they do with hyaluronic filler? Is there a reason for the vigorous swishing?? This makes me fear that fat grafting is a more dangerous procedure since arterial occlusion seems much more likely.
Doc I had skin tightening in a few sessions with laser from a laser clinic which was heat based I was told could help with wrinkles and tightening . I had no idea it would destroy my face fat and was not made aware of that risk. I am now left with very saggy marionnetes and neck on neck and face has really affected my confidence. Is a face lift my only option? I am 43
I’ve had various fillers, botox and two erbiums. I think age is important for these issues.. the filler has helped with some mild sagging/creases, but I’m in my 30s.. I know at some point I will have to put the filler on hold and get a lift and I definitely would not look how I do now without the erbiums I had done
tuffstuff Who said “so much” ? I did not say how much filler I get. I have never had more than two syringes of filler injected in my face which is enough to correct the mild (as I stated, mild issues) I have, and sagging is determined by genetics. Some people get more at an earlier age than others, and if they have experienced weight loss in the face. You don’t know what you’re talking about.

Yes but if it is just that area then fillers are much easier. You can't reverse fat like you can with filler, so generally it is always under treated even more so
Could fillers fill out a cleft chin completely ?
I've never had fillers (though I considered it) but I somewhat disagree. Fillers are great at doing some lifting. For example, they can lift the tip of your nose. Also, if injected in the upper cheek above the nasolabial fold they beautify the eyes by lifting the horizontal base line of the eyes giving a more "stable" looking eyes and a nicer reflection of light. Another possible lifting capacity is enhancing the jawlines which induces a lift like effect on the neck.

I am generally not a fan of threadlifts. I did them for about a year and ended up with a few complications (including puckering of the skin). Worst of all , they don't seem to work or last.

Yes could be but not always from weight loss. Genetics can contribute too. Best answers are fat transfer or a collagen stimulator like sculptra

I had a dermatologist who lost 150lbs and her face looked so different afterwards. Though to be fair she had a lot of fat in her face before the weight loss. She ended up injecting a LOT of filler to her face to help compensate and it did look a lot better.

Great talk, but the BEST results can only come from GREAT injection technique. Unfortunately, as you suggested, lots of injectors inject in places that are completely INAPPROPRIATE. I think you can get a very nice life from fillers. If you have tons of lines, then FACELIFT is your only option, but you will not look the same. You will look improved, but not the same.
