The smile is fake. But NF is too honest to not over exaggerate the fake smile so that everyone can see the truth. He is also knows that by being honest publicly, he is making it ok for his fans to express themselves similarly as they work out their own internal problems. This has probably been the main driving force behind the deep loyalty of his fan base.
