    I had 680 ng/dl of total testosterone at 38 and felt absolutly hypogonadal. Severe depression, low sex drive (I was able to masturbate or have sex once or twice a month), fatigue despite of forcing myself to work out and keeping weight and absolutly brainfog. My SHBG was at 80 nmol/l but noone seemed to care. I lived like that a couple of years and almost lost my job. This was hell on earth.

      Your SHBG is too high and can certainly reduce your free testosterone and cause issues. Your own normal range may be higher on the general population curve, so the TT may be low for you even without the SHBG complication. Your estradiol may also be higher than it should be and should be checked along with any testosterone work-up. You might try starting with 10 mg of boron daily to see if you can get your SHBG down and see if the symptoms improve. If so, that's a cheep solution. If not, then look at other reasons for the symptoms. If you get the test/SHBG/estradiol levels normalized for you and still have problems, then move on to other issues. Depression can also be an independent contributor so you may want to get some help there as well, especially if the hormones are normalized and there is no improvement with the depression.

      @@ianbrown5955 The more I read, the more I avoid processed foods. I have no idea if it affects SHBG, but if nothing else is working, read the ingredients on what you buy at the grocery store. If you cannot recognize the words as fruits, vegetables, meats, dairy, or spices, then it is likely a processing agent. Eating certified organic will likely cut back on the herbicides, pesticides, hormones, and fertilizers you eat. It may not correct your SHBG, but I can't believe that it won't be good for your health in a myriad of ways.

    My level is around 850-1000 ng/dl and calculated free around 24-25 ng/dl. (With weekly injections). I just turned 43 years old.

      @Helios Telemedicine I feel great. It's a little harder to lose weight, though. I have to be really strict. My last blood work the test was actually 1179 ng/dl. My estradiol level was 19.5 pg/ml.

      @@bobbydelamar606 I wouldn't try to drive your testosterone levels higher. There are a lot of reasons why it could be harder to lose weight. Everything from other hormonal issues to changes in exercise levels to changes in non-exercise activity thermogenesis (the energy you burn doing every thing you do that is not intentional exercise). Small changes in diet, getting a little less sleep, increased stress, and the amount of processed food in the diet will certainly contribute. You might want to talk to your doctor about it to see if y'all can figure our a better regimen to lose weight.

      @@bobbydelamar606 As the testosterone level rises, it CAN but doesn't always stimulate the bone marrow to make more red blood cells. If the concentration of RBC's gets too high, then you would be asked to donate blood. It is absolutely mind-boggling to me that, if you tell them that you have to donate because of testosterone therapy, some blood centers will just toss this blood away. This is called therapeutic phlebotomy and the blood is wasted. It's really stupid. After all, if I was in an accident and needed blood to survive, I wouldn't care at all if there was a smidge of testosterone in it. Because of this, some testosterone clinics will offer therapeutic phlebotomy to their patients if only for patient convenience. Oh, and the reason for donating blood is that, as the cell count get higher, the blood gets more viscous and flows through smaller blood vessels slower increasing the risk for blood clots and strokes. It may also be a good thing for the liver as that is where iron is stored, and having too much iron in the liver can injure it.

    For those of you in Canada, where going on TRT is very difficult to do with a doctor. Start yourself at 80 mg per week for 3 months. Eat a well balanced diet, and work out regularly. As your testosterone levels out, and your own production ceases, determine how you feel. Are you sleeping better? is your energy better? has your libido started to become normal? I raised mine to 100, then 125 mg. This is where I feel as good as I did at 30 years old (I am now 60, and started myself on TRT this year). I've always been into natural health and fitness, so I am in good tune with my body, and know how I should feel. I do a subcutaneous injection every day, equaling 125 mg per week. Doing it every day is much healthier than once or twice a week. Every day will lower side effects. I have been doing this for 5 months. I have noticed only a slight increase in muscle size, but I'm not taking TRT for muscle size. I had already reached my potential after 40 years of lifting. So at 125 mg per week, I am probably getting a bit more than I need, but I feel great again.

      I'm in Canada and it was very simple, I got blood work done, showed low T levels, they gave me 200mg of test e a week, I got blood work done 90 days after, dialed in my dose, after another 90 days got blood work again things were great, now get blood work done twice a year. It was very easy, plus testosterone is cheap if you have medical insurance, and the fraction of the price than under grown labs. In Canada you can order testosterone off websites and get it delivered to your door. Might not be legal but you can and if you are not buying by bulk you'll prob not be getting into trouble

      @@jamescoffin8286 I'm happy for you that your doctor was on your side. Many doctors in Canada are not onside with testosterone treatments. It costs me about $20 per month to my door from an underground lab; and that includes the insulin needles. But I also do HCG to keep everything working; that is an extra cost if you opt to do that.

      @@Tarik.krblt6 If you have the level of a healthy 30 year old man, then you don't need testosterone. Testosterone levels don't start to go down until after 30 years old unless you're lifestyle changes, or you don't eat healthy and exercise.

      Probably not, as you are still at the tail end of puberty. It is likely only to be a problem if you are having symptoms, like anger issues, anxiety, and acne.

    I just got a call from my doctor who said she is checking with my insurance for injections she is going to call into the pharmacy. She said my Testosterone level was 371. Not sure what's all about to happen or if I should even take the injections. I'm 52 and fairly active. I eat plant based with salmon three to four times a week. She tested me because I told her I had been more tired lately. Really confused on what to do.

      I would suggest that you go to my website at www.heliostelemedicine.com and take the free low-testosterone questionnaire under the Am I a Candidate button. If your only symptom is a little fatigue, then there are so many other things that can be the cause. Yes, I believe that a total testosterone of 371 ng/dl is low in a 52 year old man, but if you don't have more symptoms than just some fatigue, it may not be the real issue. If the survey suggests low-T then you should get a complete evaluation. You may want to watch some of my other videos, such as the ones on how low testosterone is diagnosed and how it is treated. They may be somewhat dry, but they are packed with the information you need to be an informed team member in your own healthcare.

At this point, I think you consider seeing what your LH and FSH are, but you should also consider that there are other causes of depression, from situational at work or home to environmental to mental and you may want to consult a therapist for the depression. Depression, by itself can cause mental and physical fatigue.

      No, I can't. Not even a clue. I can say that prior to the industrial revolution, testosterone levels did go down with age, but were likely higher than they are now, even in younger ages. Since then, we have been living in an increasingly complicated chemical soup. There are biologically active chemicals in our air, water, ground, food, everything and these have an increasingly detrimental effect on testosterone levels. Our stress levels have increased and our exercise and non-exercise activity has decreased, also bad for hormone levels. Regarding genetics, there are a lot of different genes and epigenetic factors that affect testosterone levels and they are unique to the individual, too many to make a blanket statement about % effect.

    Hey Doc, I'm 56 years old and just got my levels checked. My total is 636 mg/dL, free is 83.2, and bioavailable is 171.1. I train for competition marksmanship, do weight training, and seem to have a healthy sexual apatite. I keep hearing from guys over 40 saying that TRT changed their lives. They say they want their level around 850. Should I even try it? My doc tried to talk me out of even testing my testosterone level, so I'm probably going to switch. Thoughts?

      It doesn’t look like you have any symptoms of low testosterone. Without symptoms I wouldn’t treat you either. Everyone has their own personal normal levels and it looks like you are in your normal range. Keep your doctor and ignore the crowd.

      Not a bad thing to start with. It could get your TT up into the 700 range and doesn't cause potential low sperm count or high blood count. If it doesn't work (and give it time to work as the dose can be tapered up if needed) then you can go on to testosterone replacement therapy.

      @@Hoganblade It may take more than a month to reach full effect. Even then, we do not start with the maximum dose, so if there is some effect, but you still have symptoms, you could increase the dose. If she wants to stop for a couple of months to see if your testosterone levels are kickstarted and stay up, I would wait to try that until you get to a comfortable testosterone level. I know that sounds like a long time, but you have years to go in this life so a little patience now can really pay off.

      There is no single test order that evaluates all of the hormones. For testosterone evaluation, in the US, I order total testosterone, free testosterone, SHBG, and estradiol in the first order. If these suggest low testosterone, then I order them again to confirm along with prostate specific antigen (PSA) to screen for prostate issues, luteinizing hormone, follicle stimulating hormone and prolactin (LH, FSH, and PL) to look at the pituitary and hypothalamus, blood count to check for cell levels, and liver function tests. Once those results are back I can discuss treatment.

    Got my blood done in the morning and it was 128. I’m 27 years old with extreme fatigue and no sex drive and there trying to say my testosterone was just in the lower average range..I don’t even know what to do at this point

Any physician can prescribe TRT, but you should find one with interest and expertise in it or they may be relying on what they learned years ago. Endocrinologists and urologists are the usual go-to guys as they are the ones who specialize in hormones and gonads. Low-T clinics are also good places to look as well. Some of them want you to come back weekly for injections, some don't. There are also on-line clinics like honehealth.com which are licensed in OH. I hope you find what works for you.

    Total Testosterone: 725 ng/dL
    Free Testosterone (Direct): 5.6 pg/mL
    My primary care doctor said everything is fine, but according to the lab my free testosterone should be in the range of 8.7-25.1. What should I do. Please help.

      Your total testosterone is in the normal population range, but your free testosterone is not. If he didn’t check your SHBG and estradiol, then you should have them tested.     When you say highest at the morning. Is that because you just woke up ? What if someone slept late and woke afternoon. Should he measure it after waking up

      It's kind of hard to find a great answer for you. My personal thoughts are that if consistently sleep late, that is your usual schedule, and your circadian rhythm is shifted so testing within 2-3 hours of waking should be OK. I did find this guideline from British Columbia www2.gov.bc.ca/gov/content/health/practitioner-professional-resources/bc-guidelines/testosterone-testing#men-testing that supports that idea. I doubt that most insurance companies would accept this concept though as they can be very rigid to their protocols. Good luck.

    What are the causes of high testosterone in 19 year olds, I got tested at 37.1nmol/L or 1070ng/dl and the only explaination online is steroids (which I don’t take) or cancer, I do not think it’s either, what are the other explanations doc?

      It may be perfectly normal for you to have testosterone levels this high. Unless you are having anger issues, I wouldn't be too concerned.

    Most recent blood test was 317 ng/dl…
    My doctor said that this is in the normal range..
    I asked for a 27 year old or a 70+??
    Still no treatment.. booked another appointment for next week..
    Symptoms are the usual
    Little to no morning wood, most of the time depressed, packing fat on awkward places, low energy

      @@Viikk785 That's low for a 27 year old, at least in MY book it is, especially if you have the signs and symptoms. At 27, you may want to give Clomid or enclomiphene a try as they don't suppress anything, rather they stimulate your body to make your own testosterone and sperm.       @@Viikk785 The guidelines suggest that we could take you off of testosterone at about 6 months to see if we have reset your hypothalamus to expect a higher level to be normal, and I have seen it work in the rare patient. On the other hand, testosterone replacement therapy suppresses the normal control axis, and treatment is life-long. That's another reason to try enclomiphene or Clomid first. The whole axis is stimulated and if resetting the normal level that the hypothalamus expects is possible or likely, then going the stimulation route seems more likely to succeed.

    Hey doc I just had my T levels checked and they where 335 ng/dl. I live a very active live style. I play sports with my kids daily, former military, and I work on my feet. I’ve had problems with shedding weight, Sleep issues, Ed. Thoughts?

      If you had your blood drawn after 10:00 AM, then your testosterone is even lower. The ED suggests fairly strongly that you would feel better with a higher testosterone level.

      I would say that your total testosterone is low for you, especially if you are having symptoms. It is variable if you could convince your insurance company to pay for therapy, though, since most like to use a blanket 300-1000 ng/dl normal range.

      Your testosterone is low for your age. You should find someone to get treatment. If you are worried about fertility, you might consider clomiphene or enclomiphene as they stimulate the whole hypothalamus-pituitary-testes axis so the sperm count should be preserved or even increased. It may take 3-6 months to reach it's peak effect, but improving the chances of having kids may be worth it (and remember that we are talking about a lifetime thing here so this isn't really a looooong time. If that doesn't work, you can still go on to testosterone replacement therapy.

    I’ve had about 4 blood tests and ranges from 300-500
    Strange , I’m 43 and the last year or so I’ve had all the symptoms .
    Doc said no way you’re normal even though all my other bloods were good and no explaining the crushing fatigue and bad mood .
    Private clinic said they would treat me , i appreciate business is business , I was just curious if your opinion?

      Your total testosterone is only one of the metrics to consider. What are your free Testosterone, bioavailable testosterone, SHBG, and estradiol? Even if they are all in acceptable ranges on a population basis, your own needs may be higher. Have you looked at your adrenal hormones as adrenal fatigue can have a lot of the same symptoms? Thyroid hormone issues can also show up as fatigue and mood changes. A thorough physical exam with bloodwork to look at other organs may also help.

      @@mattlofty5884 I think that the free T is low for your age especially if you are feeling the symptoms. The SHBG is fine. The estradiol is high if measured in pg/ml. I like to keep the E2 between 20-30 in my patients. If the Prolactin is measured in IU (International Units) or ug/L then it is normal. If it is measured in pg/ml then it is too high, by a lot.

    Hi Doc, I'm suffering from severe insomnia also anxiety. I think I've overtrained especially when I finished nights at work I would sleep for 1.5 hrs then train. Had my testosteone levels checked 3.30pm 6.6 and another day 8 am 10.6. Still no sleep unless I take 2 sleeping pills. Will trt help me with sleep? 44 year old male.

      Sorry for the delay. There are a number of issues that come to mind. The testosterone levels are normal according for the populations, and if insomnia is your only symptom, then it likely is. Sleeping on the train after work likely contributes to the problem as does exercising late in the day. It may be better to wake up earlier and train in the morning. Try not sleeping on the train. You may also have a habituation/addiction to the sleeping pills and weening yourself off of them may help. There could be other issues as well, like stress, how much caffeine you take in and when, or other medical issues.       Sorry about the delay in the reply. Testosterone normally drops through the day, which is why we check it in the mornings. There are a lot of reasons for insomnia including low testosterone, including training before sleeping. Try training before work instead of after. Other common reasons include stress, other endocrine or metabolic issues, ingesting too much caffeine, sugar, or alcohol. timing of meals and sleep, and sleep apnea. You might want to get a complete physical with bloodwork to see if anything else is going on.

    I wasn’t able to sleep for 2 days straight and then I went to the ER cause I got bite by my dog and I also went to get my test levels checked and it’s 500 and I’m 19 is that bad??

      It is officially in the normal range. If you do not have symptoms suggestive of low testosterone, then it is likely normal for you. If you do have symptoms, then they should check the FSH, LH, and PL along with SHBG, Free Testosterone, and Estradiol levels. It may be that your body wants to be at a higher testosterone level.

      Your level is at just about the middle of the normal range for men your age. That said, if your SHBG is high, your free testosterone level may be low. If your estradiol is high, you could get symptoms from it.

      In my book, the total testosterone is low, but the free testosterone is normal. Your SHBG is probably low (not a problem).

      Your TT level looks ok. What are your symptoms? While the estradiol level is officially normal, I like to keep my patients in the 20-30 range.

      @@HeliosTelemedicine those labs were from February 2023. I just had lab work done again on Monday and got my results. Testosterone was 440 & estradiol was 21.6. Symptoms include low sex drive, ed, reduced muscle mass, irritability, chronic joint aches and pains, depression, fatigue, obesity by military standards (6’2 210) poor memory, gyno surgery in 2022 (never done anything other than taking protein) loss of armpit and pubic hair.

      @@HeliosTelemedicine I actually just got a script today for testosterone cypionate 200 (.40 2x weekly), HCG (50 units twice weekly SQ) and anostrozle (.5 weekly used as needed) and am paying $300 every 10 weeks. I recently moved from MO to UT. The VA/Tricare wouldn’t cover it so I pay out of pocket.

      Not really. You can only really boost your resting T levels by 20% from lifting weights and getting exercise and eating a healthy diet. So you can raise it but not that high

      Hi Khaled, There may be many reasons that you are not getting the results in the gym that you want. Your total testosterone is still in the normal range for the population, and may be in your own personal normal range. That said, if your Sex Hormone Binding Globulin is high, your free testosterone levels may be low. If you are really good at converting testosterone to estradiol, you may have a high E2 level and a bad E2:Test ratio, either of which can interfere. Do you know your free T, estradiol, SHBG, LH, FSH, and prolactin levels? You may be normal for you and a whole host of other issues could be the issue, from other medical issues to problems with technique at the gym. One thing that I would NOT do is start taking anabolic steroids that you may have access to at the gym. That's a good way to get into short- and long-term medical issues.

      With symptoms, I'd say that you had low levels. At 27, I'd suggest looking into enclomiphene to stimulate the system to make more testosterone before resorting to testosterone replacement therapy which suppresses everything, including sperm production.

    Hey Doc. I’m 31, i weight 234lbs. Total testosterone is 268ng/dl , free, calculated pg/mL is 72.5, and % free is 2.7. Bioavailable is 170. My sex hormone binding Globulin is 16.9. I have brain fog, low sex drive and can’t seem to lose weight. Are those levels ok? They show all with in the normal ranges on my clinic website but I feel like the total testosterone is too low.

      The total T is low. The Free T is in the normal range, but may be low for your metabolic needs. Your low-normal SHBG is likely the only reason your fT is as high as it is. It might be worth trying a 3-4 month course of clomiphene or enclomiphene to see if you can get your testosterone levels up.

    Hi Doctor, I caught a nasty bout of food poisoning and lost a lot of weight and I thought crashed my hormones as I had ED for 2 months along with being very skinny. My Total T came back 35.7nmol (1000ng) but I didn't feel good at all. Is there any reason my T would be quite high when ill? And do you think I should get it checked again encase it's now higher when I'm back to full strength. I was expecting a really low number.

      Everyone's normal testosterone levels are unique to them. That may be your personal normal level, especially if you aren't having anxiety, mood, or anger issues. I would expect the level to be a little higher if you were dehydrated, but not that much.

      If that is your total testosterone, then it is very low for an adult male. Yes, fatigue is a very common symptom of low testosterone. You may need to have your LH, FSH, and prolactin levels checked to rule out an issue with your pituitary gland.

      I'm not an expert here, but I would think that it would have resolved for you by now and I doubt that any medical management would help. Have you had your total testosterone, free testosterone, bioavailable testosterone, SHBG, and estradiol checked. That may guide your physician in how to treat you. If nothing medical works, a surgeon can remove the breast tissue.

      In my opinion, you are hypogonadal and the symptoms could certainly be due to that. Insurance companies my look at the number and disagree. Treatment would depend on testing to see if it is due to the testes, hypothalamus, pituitary, or something else. It would also depend on your desire to have children in the future.

    doc, do you think modern xenoestrogens like those produced by micro-plastics effect these levels considerably? im 24 and i measured at 670 ng/dl but i wanted to go higher and the doctor almost seemed offended by that. i want to negate the effects of the modern world has on me

      We live in a chemical soup and you will never negate the effects of the modern world on you, but you can try to reduce them. Live somewhere away from refineries, factories that use or make volatile chemicals, and away from major sources of particulate air pollution. Eat organic and look at ingredient lists to avoid processed foods. Find more natural products for household use. Avoid foods and drinks in plastic containers. But you already know all that if you are asking about xenoestrogens. All that said, your total testosterone could still be low for you if you are having symptoms. If not, then it’s probably normal for you. Don’t just look to treat a number.

      Likely yes, but testing should be done in the morning before 10:00. Codeine shouldn’t be used regularly. You habituate to the pain relieving effects and you can get addicted.

    I’m on Nebido(Undecaonate)injecting myself every 10’th week with a trough/lowest level of 657 ng/dL.
    Very content with that, though must admit I feel even better the first 5-6 weeks after having injected myself as my levels are higher then.
    But hey, one can’t have it all. Btw I’m on TRT due to having had testicular cancer, which I’m actually happy about due to getting TRT and will have a good testosterone level till the day I drop dead. I’m also on HCG 1000iU every other day to keep my last nut alive.
    Being 40 y old now with a trough level of 657 ng/dL I aint complaining.
    Edit: Just calculated my free T as well and it was 13,7 ng/dL which is pretty much optimal especially considering that’s my trough/lowest level.

    Hi Doctor, 25 year old black male, 5’10”, 240 lbs, 20% body fat (pretty muscular. Total T at 350 ng/dl, Free T at 62.1 pg/ml, SHBG at 40 nmol/L, TSH at 3.13 uIU/ml, FT3 at 4.05 pg/ml. I’ve had Gynecomastia and low energy since high school, low body hair, facial hair came in during college. Exercise 5 times/week 1+ hour, 12+ miles running, very strong weightlifter. Very fatigued even on rest days, low energy, motivation, libido. Possibly low sperm count. Thoughts?

      Your Total T is "low normal" as is your Free T. It could be a cause of low body hair and the fatigue. Remember that the "normal" ranges have to include 99% of the population studied and that population includes everyone from about 25-70+ years of age. Your own normal range is much narrower and may be higher than what you are seeing now. Regarding the gynecomastia, check your estradiol levels. If they are high or high normal, you may want to get it down with a supplement called DIM or a prescription medication called anastrozole. You can raise your free T if you get your SHBG down as well. 10 mg Boron daily can help with that. If you live in TX, NY, or MO (and soon to be FL) I can help you, but I'm not licensed elsewhere. Gynecomastia is not something to ignore. Male breast cancer does occur and is more frequent in men with gynecomastia. It can be treated or removed.

That said, if you are having symptoms typical of low-T, then it may be low for your personal normal range. Before going down that road, I would suggest that you get a complete physical with bloodwork to make sure that you do not have a different reason to feel whatever it is that you feel.

      @@HeliosTelemedicine In FL I went to a wellness clinic called Juvenate. My GP would not give me TRT. The wellness clinics don't charge insurance so you have to pay cash but it's not all that bad. I have been paying $160 month for both TRT and B12 injections. TRT is 2 times a week and B12 is once a week. It was life changing I'm telling you. I feel so much better and back to normal. I've been on TRT since Nov last year.

      @@dirtwhisperer658 I am very happy for you. If you are taking the B12 injections, I would recommend that you start taking a Super B Vitamin Complex tab daily. It MAY extend the feeling of increased energy you get from the injection by a day or so. It also seems to have something in it the mosquitoes hate. You may find that you get many fewer bites.

    Hey Doctor, I just got my testosterone tested and I'm 600 ng/dl. I'm 27 years old, I have serious weight gain, no sex drive what's so ever, I eat veryhealthy, I work out constantly and live a healthy lifestyle but as much as I work out I don't see results and just unhealthy weight coming on. I'm depressed, no energy, insomnia and as hard as that it is I still push myself. Would I qualify for TRT ?

At 27, you may want to try enclomiphene or clomiphene first as they do not suppress the system and won't cause a lower sperm count. If your normal range is higher than your numbers and you have symptoms consistent with low testosterone levels, then you may feel better with treatment. It is unlikely that insurance will cover it. Your therapeutic options should also include a look at your fertility and desire for kids in the future.

    Hello doc, i was 16 years old when i checked my testosterone levels and the results were that i have 1,6 nmol/l which is probably very low, considering that i always get treated like trash and i’m always regected by people since i was little. I even here people sometimes saying homophobic stuff to me and i really feel down and always crying. I always experience feminine feeling towards men and i hate the fact that i do. Is low testosterone the main problem that i’m facing in this situation?

      It this is a Total Testosterone level, then it is low and you should get further evaluation. If it is a Free Testosterone level, then it is normal. At 16, if that was a total testosterone level, then part of the evaluation should be genetic testing.

      Your Total testosterone is at the lower end of normal on the population-based scale, but you free testosterone is closer to the middle of that scale (which is the more important of the two numbers.) Your body may like to be at higher levels, but I would look for other causes of fatigue.

      You are likely in the lower 5-10% of guys your age. If you are having symptoms of low testosterone, or if you have delayed puberty, then this may be an issue. Ask to be referred to an endocrinologist.

      If you don't have symptoms, then you don't need to do anything. You can try taking boron to lower your SHBG, most start with 10-12 mg daily. If you have significant low-T symptoms, you can look for help from your physician, but insurance companies won't pay for therapy and most low-T clinics won't treat a total testosterone in the 500's.       I’m not surprised. The guidelines say that the target for total testosterone level on TRT is 600-800 ng/dL. You didn’t mention your free T level. That is really what is important as that is what determines your symptoms. You should also know your estradiol and SHBG levels.

    Hi doctor, I'm 54 and have a question about trt, my primary doctor tested me and first test was low and went for second and she said it was normal at 260, I went to a urologist and he tested me and said I was low but before anything he referred me to an encronolgist to see if something was making me have low t and got tested and it was 324 and was told it's normal, but have all symptoms of low T any advice????

      You have labs that straddle the lower limit of "normal", but you likely need a higher total testosterone level if you have the symptoms. Did they check your free testosterone? SHBG? Estradiol? At your levels, I don't see why a short-term trial on testosterone replacement or clomiphene couldn't be done. If it relieves the symptoms to get your TT into the 600-800 range, great. If not, then they shouldn't stop diagnosing the symptoms.

      From my research the “normal” range is a joke. It seems to me, anything under 500 should be considered and anything under 400 should almost definitely be low enough to be on TRT. Doctors seem very against TRT. Almost the same level of fear of giving legitimate opioids to people in severe pain.

      I just turned 60 this month and I tried for years to get my GP's to put me on TRT. None of them would do it even at 400. My last doctor actually chewed me out for a good 10 minutes telling me how bad it would be for me. He listed every single falsehood that you hear about testosterone therapy. I went to a wellness clinic where you pay cash because they don't charge your insurance. They gave me a shot that day. I've been on TRT since Nov 2022 and I haven't felt this good in years. It is life changing. I haven't experienced a single negative with it so far.

      Sorry I missed you until now. It looks like it is in the normal range, but if your doctor cannot find any other cause of low testosterone suggestive symptoms, you can do a trial with clomiphene to see if you can nudge the testosterone up and relieve them. Check your estradiol and SHBG as well as they can cause symptoms and can be treated.

      Check your diet first. Make sure you're getting enough carbs and fats. A number of low test guys have increased their test just by having a balanced diet, i.e., not doing stupid zero carb diets. Also, if you've done steroids, TRT might be the answer if nothing else works.

      Find out what your SHBG level is. It’s probably well over 40 or 50. Eat healthy. Exercise. Eat cruciferous vegetables. Take Boron supplements. These can each help with SHBG.

    I am 52 years old, I was on Testosterone for about 1 1/2 years under my physicians care, when first diagnosed my level was around 400, with testosterone it went to roughly 850 and I felt great. I have moved recently, and have a new Dr that has taken off testosterone. My level today was 420 and I don’t feel as good as I did, my new Dr say 420 is normal level for me and that 800 is way too high a level and to dangerous at my age. But I felt a lot better when my level was at 850. You’re advice??

      I have patients who are older than you on testosterone therapy with levels at and sometimes higher than 850. If you felt normal and had no side effects such as anxiety, anger, increased red blood cell count, or uncontrolled elevations in your estrogen levels, then I think you should be back on your testosterone replacement therapy. While 400 ng/dl is in the population-based normal range, your normal range is narrower and may be higher than your level is off of testosterone. You may also ask to have your free testosterone, bioavailable testosterone, SHBG, and estradiol checked as they may be out of range.

      @@brianweatherman1207 850 is usually a great level. Like I've said in other videos, therapy is a team approach and the actual tested number is only one factor to consider. If you felt normal at 850 ng/dl at one week after your injection, then that's great, but a few people need to be closer to 1000. For some, a lower number like 500-600 is great and they would feel hypergonadal symptoms at 700. Getting to the right dose for requires adjusting the dose to find the one that is right for YOU. There is no cookbook recipe or one-size-fits-all number. Just remember that, the higher the testosterone level, the more likely you will have to donate blood to get your blood count down as testosterone will stimulate the bone marrow to make red blood cells.

      There are some guidelines that suggest treating for 6 months then stopping treatment to see if the hypothalamus has reset it's expected testosterone levels so that therapy can be stopped. I have nothing against doing this, but doubt that that most patients are going to "cured" this way. If we can address an outside cause of the low testosterone levels (alcohol, meds, poor lifestyle, nutritional deficiency, etc.), then we can also try stopping treatment to see if the symptoms stay away. If the cause is innate but related to something that is not a part of the thalamus-pituitary-testes axis, then correcting that issue may fix the testosterone problem without permanent testosterone therapy. If, however the cause of the low testosterone levels and symptoms is due to problems in that axis, then I believe that most will need therapy on an ongoing basis.

      Every lab has its own normal range and they may be differently based on how they are running the test, what machines they are using, and which professional association they are using. They may also report their results as ng/dl as in your first range or nail/ml. There is also total testosterone, free testosterone, and bioavailabile testosterone, and each has its own normal ranges.       That’s a good level. You aren’t telling me about symptoms, but I suggest that you look for other causes before blaming your testosterone level.

    Hi, I'm a 32 year old male with Total T-levels measuring at 452 (taken at 11am) and free testosterone of 10.1. My SHBG levels are at 25.9 and Albumin is at 47. According to online sources (which is why I comment here as I know they're not always reliable) my total T-level is mid-normal range but my bioavailable T-level would be 292 which would be very high? I'm not sure what to make of the test.

      Your total testosterone is at the low end of what I would call normal for your age. Average is about 615. If you are having symptoms of low T, then you probably are. That said, your TT is in the normal range for the population and insurance companies are not likely to cover your therapy. If you have no symptoms, then the levels are just numbers.

      @@HeliosTelemedicine thank you for your swift response, I have no symptoms but do worry about maintaining (and increasing) muscle mass. I also don't want to have low numbers or hypogonadism as I get older, I feel as though the numbers are low, I just didn't understand them properly.

      @@Forevermist at your age, I usually recommend Clomiphene or enclomiphene instead of testosterone if the rest of your work-up suggests that they could work. Regarding later in life, don’t worry about it until you have symptoms.

      Yes, especially if you have symptoms (and you likely do at that level.) Make sure that you discuss fertility with the doctor if you want kids in the future. Please do that off of a public forum, though as you have no privacy here. You will need more testing to diagnose the problem to get the best treatment.

    Hi doc.. im 18 and my testosterone is 576, i think it should be higher because 18 is basically the peak of testosterone. Do you think it is normal?

      According to a number of studies, your total testosterone is right around the 50% range for men 17-18 and 19-39 years old. I would have thought that it would have been higher, but apparently not. If you are having symptoms suggesting low-testosterone, then you should look further. Look at the free and bioavailable testosterones, estradiol, and HSBG. It could also be that you are one of those who need to be in the 800-1000 ng/dl range to be normal (and still be in the normal range according to the population studies.)

      @@hamzasulman9726 It is certainly possible to increase your testosterone level, but if you are not having symptoms and signs of low testosterone, then you probably don't need to. Everyone has their own unique normal range and you may be at your body's ideal level. If you are asymptomatic, then you shouldn't push it up as you may get to levels that are too high for YOU and have resultant short-term and long-term consequences. There is a simple questionnaire on my website that you can take. www.heliostelemedicine.com Also remember that the free testosterone is the active stuff and if your SHBG is low, then your free-T may be higher than you would predict based on your Total-T alone. Estradiol levels are also important to symptomatology. That's why I always check both testosterones, SHBG, and estradiol.

      I had replied from my personal account last year. If that is your bioavailable testosterone, then it is low by the numbers, but you should only get therapy if you have symptoms.

    Hello doc i just did some test because of my insane cystic acne that i have had for a long time
    Im 23 and my testosterone is too high at 53.48 nmol/l or about 1550 ng/dL if the unit converter worked
    My DHT is 1304 pg/mL and my androstendion is 6.3 ng/mL
    Estradiol is 41 pg/mL
    Is it normal for all of these to be high as i never took any steroids or anything like that.

      Your levels are too high. You should ask to see an endocrinologist. Normally most androstendione is made in the adrenal glands, which can also produce testosterone. Congenital hyperplasia (overgrowth) and some adrenal tumors can cause elevations in these hormones. They might want to look for a testicular tumor if the adrenal work-up is negative. There are short and long-term consequences that you should try to avoid, such as scalp hair-loss and enlargement of the prostate gland.
