I'm a Liverpool fan, but i think Chelsea is the most popular football club in Ghana. Chelsea's success in the 21st century coincided with the boom of premier League in Ghana. The PL viewership became significant in Ghana in the early 2000s when sports viewing centres in Ghana became a prominent business. This gave access to more people to watch PL football cos it was never broadcasted on local tv stations. This was the time Mourinho came to Chelsea and many people here loved his boastful character coupled with his love for African players. Signing Essien was the biggest point. Essien at that time was considered the best Ghanaian player at that time. Young people who were born in Ghana in the 90s and 2000s make up more than half of the PL fans population in Ghana. And most of these young people support Chelsea because of their first acquaintance with PL football coincided with these important events in Chelsea's history.

When I worked in Addis Ababa for a year in 2014, I was the lone Manchester United supporter in the local bar I frequented to watch Premier League and took a lot of stick from the 70 or so Liverpool, Chelsea and Arsenal supporters, but they were always welcoming whenever I showed up. I once showed up 15 minutes late to a packed house, so the owner let me sit on the barrel behind the bar, knowing I would be the only patron who wouldn't be tempted to dip their hand in the till. Messi and Ronaldo are the biggest factors there alongside the dominance the teams had over the past decade, Madrid over the last 8 years or so and Barca during the decade prior. It also coincided with the rise of one of the best National teams in history (Spain) dominating the sport for 4 years, which is a long time for international football. That team was about 70% Barca 25% Madrid 5% other stuff. Barca 2008-2012 was arguably the greatest team ever assembled being one of the only two teams to successfully complete a sixtuple (win every single tournament they participated in a season), while Madrid's 3peat of the UCL naturally speaks for itself.

I may be wrong here but in Malta i think that title would go to Manchester United.
Malta was a British colony for a long while but the cultural and geographic ties to Italy (Malta is just south of Sicily) means alot of Italian clubs have decent fanbases too, with the El classico clubs and the likes of Bayern also having a decently large community of supporters.
However i think it's united who take the crown due to the long ties between the two. To start although Utd have never had a Maltese player, Malta does lay host to the oldest ever Manchester United supporters club (yes even including supporter clubs in the UK), On top of this, Man Utd recently signed a sponsorship deal with Malta's tourism authority (which is why you see visit Malta ads at old trafford), that coupled with the fact that it's not the first time Man Utd legends have had a very warm welcome in Malta makes me think Utd may be the most supported club of all in the country (although several Italian teams like Juve, Inter and Milan and even prem clubs like Liverpool) may also challenge that statment.
Despite that and the fact Maltese clubs almost never make it into European competitions, Many Maltese clubs maintain decent supporter bases in the country, this doesn't mean the most supported club in Malta is Maltese but that local clubs give some competition, It is also a common Phenomenon in Malta that people have a local and foreign club they hold close to their heart.
In conclusion, i think Manchester United is the most supported prem club here in Malta if not possibly the most supported club of all, although i may also be wrong.
(If you read to this point, thank you for taking the time to)
