Cox is a heavy-drinking, cynical, witty, sharp, and pretty aggressive guy. That makes him an entertaining character to watch. But people don't want to be friends and hang out with a cynical jerk, even if he is funny and witty. Trust me, I know. (P.S. That's also why Dr. House has no friends besides Wilson)
@@mattbodee6326 In season 1, JD showed up with some beer because he thought he'd be alone in his apartment and Cox laughed him away, but there were clearly people in there and one guy shouted that he'd drink the beer JD bought after Cox insulted it. He _had_ friends that he could invite, but I guess they got sick of him.

My brother unfortunately had this same thing happen where he planned this whole big party to share his birthday with another person at work, bought around twenty pizzas and went out to an amusement park and nobody else but me came.

I was super-impressed to voluntarily quit Dr. Cox. Also impressed that Cox asked JD to work for him again.
