    Mr. Nguyen. Thank you for the video. In the homework at the beginning, shouldn't Lumber Spinal Stenosis have 6 characters? I chose M48.061, because the ICD-10 book showed that it is supposed to have a sixth character. Is this correct? Also, explaining the path to the right answer REALLY HELPS! Thank you so much for that.

    Hi Hoang, Thank you for you giving back by helping students pass their CPC, COC, or CCS-P exam. I was a little confused about question 3 the selected answer was a instead of b on minute 14:14. I see where it talks about the excision but after, the question stated the name of the procedure was a biopsy mass of the left breast. Please advise. Thank you kindly.

      The physician uses the term lump/mass biopsy and excision interchangeably and that's why you need to read the details of the operative report to find out which code is the right code to use. If you listen carefully to what I explained in the video, 19101 is for biopsy which means that they only remove a piece of tissue not entire lesion. On the other hand, 19120 is for mass excision which means that the entire lesion or mass is removed as it is clearly described in the op-report " the surgeon dissects down to the mass and removes it entirely" I hope it helps.

    Mr. Nguyen. thank you for all of your videos on the CPC exam. I started watching you after I failed my first attempt. You were recommended so I gave it a shot. I sat for my 2nd exam 13 days after I failed, and I'm happy to say I passed thanks to you. I am now studying for my CCS exam and I noticed that this video has CCS-P. Will these videos still help with just the CCS exam or will you have exam prep videos for that one as well? Thanks again.

The CCS exam also includes CPT coding portion so all my videos will also help you with the CCS exam as well. The only portion that I don't cover is inpatient case study which is more complex and that is why I haven't covered it yet . You should watch those videos that I made 8 years ago about "ROOT OPERATION" and those are for CCS exam prep. I hope you'll keep up the good work in the future. All the best!

      Hi Phung. Let's say the code is your destination and finding the code is the route guiding you to your destination. There could be more than one route (main term) to guide you there (freeway or local road) and your objective is to find the short cut either tab your book or find the best main term that you can take you there fastest and avoid getting lost along the way. That said, you can learn either way, if you don't code everyday, then learn by heart so that next time when you need to look up code for a particular procedure, you know where to find it fast and that's all that matters.       Hi Laura. If you are studying for the CCS, beside watching these videos for the CPT procedure code preparation, please make sure that you also watch those videos that I posted 8 years ago about "ROOT OPERATION" for PCS procedure codes. You need to know this in order to code inpatient cases. I will be thinking about Zoom tutor but I don't know how much they charge for tutoring out there because in order to do that I need to cut back on my work hours(60 hours/week for two hospitals) and I have to know if it's worth it.     Hi! Mr. Hoang, First and foremost thank you so much for posting these videos, they are of great help. Really Appreciate all your efforts. I gave my 1st attempt CPC exam a month ago but unfortunately, I missed the passing score just by a couple of points, that is when I started searching for videos online for guidance and testing. I listen to multiple video's but your's is the best! I want to know if you would recommend a CPC exam practice test book that I should buy online? Thanking you in advance!

      Hi Shabana. I would go with whatever coding materials that you currently have to prepare for this exam and all you need to do now is to focus the strategies that I use then you would pass the exam. Make sure you watch all of my videos and take note then you should be fine. With my backwards approach you can tackle any questions or case studies as long as they are multiple choice question. You can do it. I wish you all the best.

    thanks for all your effort sir, may God uphold you, sir, I have TWO questions that I want to ask, how to report Excision of ( 2cm by 2cm malignant lesson of scalp)? I think we suppose to sum the 2+2 together to get 4cm in total. or is it only 2cm we will work on? second question, can modifiers be added to ADD ON CODES?

      Hi Rasheed. 2cm x 2cm means that the lesion is measured 2cm in width and 2cm in length. If you add 2+2 = 4cm, you will make the lesion bigger than itself. Just follow the coding guideline, lets say you have a lesion measured 5cm Width x 6cm Length x 0.5cm Depth), you pick the largest number which is 6cm and that is the longest incision that you use for CPT. On the modifier question, technically, the add codes already indicate they are additional size, hours or something so there is no need to add mod.59 unless in some cases, you have to use the add on code twice then I will use mod59 just to let know that they are not duplicated codes. I hope it helps.

    I am a bit confused by Question 3. How is 19301 not a good answer? A lump was removed.

      A mastectomy is surgery to remove the whole breast and not just a palpable lump. I hope it helps.

      The difference between lesion excision vs biopsy is that in EXCISION, the entire lesion will be removed by using scalpels whereas in BIOPSY, only a piece of tissue is removed, usually done by using needles. That is why B 19101 (biopsy of breast) is not the correct code to use in this case. I hope it helps.

    For Question 3 - I thought it was 19101 because the procedure is described as "biopsy". In this case, how do we determine if it is biopsy or excision? Or do we assume that if the mass was excised, it is automatically an excision?

      Hi Sharon. The difference between biopsy and excision is that biopsy is a removal of a piece of tissue versus excision is a removal of an entire mass. For example, they do a needle biopsy to assess a breast mass or lump so only a small tissue sample will be taken and if the pathology confirms it is a malignant cancer then they will do an EXCISON to remove an entire mass.     For question #3 of the homework why would we not code E11.69 as well? And the sequencing for that would be determined by what was treated, correct?

      You should not report E11.618 which specially states that it is a type 2 diabetes with other diabetic arthropathy in addition to E11.69 for type 2 diabetes with other specified complication.

    In the homework assignment, question 9, in the 2021 ICD-10-CM manual the code M48.06 has a red Stop sign in front of it, so I added an additional character which made my answer M48.061. Is that incorrect?

      M48.06 is not a complete code in ICD-10-CM and that is why you see a red stop sign in front of it. It needs a sixth character so you have to look down and see there are two codes : M48.061 and M48.62 for you to choose. Similar, when you look up code for fracture, you will see the code(s) need a seven character for either initial or subsequent treatment. I hope it helps.
