Good video. My comment would be at 12:56 when you are talking about the adjacent skin transfer your note says "include lesion incision code" and I know you mean that the transfer code includes the lesion code and you don't need to add a separate code for the lesion but it might be misread as "you should include" the lesion incision code. Maybe it's just the way I read it. Luckily you explain it in the narration so anybody who is paying attention should understand what you mean.

I'm SO glad you did that last case study! I would have been lost and gotten hung up looking for the margins or how to figure them out. I am still getting hung on layer vs layered closure? The first question was closed in a 'layer fashion' and it was intermediate. But the next question it was closed in' one layer' so it's a simple repair? It's like the word layer is used for both? What am I missing?

Mr. Nguyen, please clarify the following for 2021 regulations - when choosing the code for the excision, it is based on the area, size + margin +margin, and whether it is benign or malignant. A Simple repair is included therefore not coded. When an intermediate or complex repair is required, what are the parameters to choose the measurement? For example, BSC L Chin 3cm, margin 0.3cm = 3+0.3+0.3=3.6cm (11644), 4 cm layered closure. Do I use the 3 cm, 3.6 cm or 7.6cm for the intermediate repair? In advance, thank you.
Hi Bibi. if the excited lesion is 3.6cm after you add the margins then you need to cover this wound at least 3.6cm or larger. That said, in your CPT book, under intermediate repair for face, ears, eyelids, 12051 only covers 2.5cm; therefore, you have to use 12052 for 2.6-.0cm and this will be larger enough to cover the wound. I hope it helps.
Please watch the "coding steps" slide starting from 20:09. The simple repair included in the lesion excision codes so you do not code them separately. I hope it helps.

Hi Renee. You got it. 27632 is the right code to use for an excision of soft tissue lesion of leg >3cm. Please note that these soft tissue excision codes already include the wound closure unless it is closed by rotational flap or other tissue transfer techniques. I hope it helps.

Hello, I am having some difficulty with determining the size of the lesion for the first coding question. I thought we had to multiply the greatest dimension by 2? I thought we had to multiply 3.0cm x 2? The answer I am getting is 6.5cm, not sure where I am going wrong.
The a x b x c lesion measurement format refers to the dimensions (length, width, and depth) and is not meant to be multiplied. Instead, this format is used to document the size and depth of the lesion accurately for coding and reporting purposes. For coding, you will need to choose the largest number from a, b, and c. One layer = simple repair. 2 layers or more = intermediate repair. I hope it helps.

Hospital does not use modifier 51. Please review the list of modifiers approved for Hospital Outpatient Use in your CPT book. I hope it helps.
