If you watch my lesion, take note and understand it then it should take you a few seconds to narrow down to the right answer. Watch the minute 1:07 about the principal dx. When the treatment directed at secondary or metastasized site then the secondary site is the principal dx and should be listed first. Now look at your question, it states that the patient is being treated to day for his metastasized or secondary site which is the bone cancer C79.51 so it must be listed first. Looking at A,B,C and D, I see only B has C79.51 listed first so it must be the right answer. I hope it helps.

Hi Leslyn. You got it; the principal dx is Z51.11 (encounter for chemo) however, severe nausea and vomiting are also documented and treated with medications so I would add R11.2 (N&V). When admission is for management of an anemia associated with the malignancy and the treatment is only for anemia, then the code for the MALIGNANCY is sequenced FIRST followed by the code for the anemia. For example, a patient who has the colon cancer causing GI bleeding and that leads to anemia because of frequent bleeding. Now he is admitted to the hospital for blood transfusion to treat his anemia. Following the guidelines, you assign the code for colon cancer FIRST followed by the code for anemia and I also code the GI bleeding if is documented. I hope it helps
