I'm very confused as to when to use modifier 59 when it pertaining to Needle aspiration, can you explain?

The guidelines are very confusing! See the paragraphs before code 10021. In general a modifier 59 is used when there is an expectation that the codes being reported might be bundled together (meaning not paying for both) by the insurer. the modifier is used for FNA to indicate that these are different lesions and should therefore be coded separately. The guidelines state a modifier 59 is used when: more than 1 FNA biopsy is performed on different lesions, on same day but using different imaging modalities; or when FNA is performed on one lesion and core needle biopsy is performed on a different lesion, same session, using same or different imaging guidance modalities.

The CPT codes are the very first ones listed after the E/M codes - See Surgery, General.

It depends on whether or not all the biopsies are performed using the same modality (without guidance, with ultrasound, with fluoroscopy, with CT, with MRI). If all use the same modality, list one code and then the add-on code x3. For example, 4 biopsies without guidance --10021, +10004x3. If two are without guidance and two are with ultrasound guidance--10021, +10004 for without guidance and 10005, +10006 with ultrasound guidance.

You 😡put animals😢 to sleep for a biopsy, but not humans and I’m gonna have to do it again with no anesthesia which is very painful very scary and very abusive😭
