Mistakes as a New Grad PT | Day in the Life as a Physical Therapist

  • I am a new Grad-PT and I make frequent mistakes. I'm not going to deny that. But that's all a part of the learning journey! I EMBRACE IT ALL!!
    Here is a dive down my clinical decision-making-process as a new grad physical therapist.
    It is common when shoulder pain is reproduced with movement, especially with rotator cuff pathology. So when the patient complained of shoulder pain with internal rotation of the humerus, I figured the subscapularis was the source tissue, since it is the only internal rotator of the rotator cuff muscle group.
    Then after 10 sessions of trying, I had no luck in helping the patient get better. So then I had to take a step back and decide... do I want to continue down this path, or should I decide to look elsewhere.
    I figured since I have not had any luck or improvement in 10 session, it might be a good idea to move on to a different source tissue. That is when I thought: "what other muscles internally rotate the humerus?" 2 muscles came to mind: Lats and Teres Major.
    And that is how I found out that the Teres Major was the muscle that was causing the patient's pain.

    Great video! This goes right along with what I’m learning in PT school and allows me to see why knowing OINA are CRUCIAL to memorize. This helps put all the content into a real life example. It also helped me practice a clinical based case in my head bc I was recalling in my head the other medially rotating muscles 😂

      ahahaha nice nice!! YESSS I get choked up and sometimes forget myself... but good thing you are able to see the pt multiple times hahaha! Good o internet or quick review of school notes is always a solid choice. Im so glad that these videos give you insight and practical applications on WHY you are learning your info and why its important! This was something I really looked for when I was studying, and now you get to get the full experience!!!

    This is a great self evaluation of your clinical work! What kind of interventions did you do for subscapularis and what interventions did you do to isolate the teres major? Thanks!

      yeah that's a great question! & thank you so much!! For subscapularis I was trying to work towards more of a strain/tendinosis type of treatment, but after a couple weeks there was not any progress. When I started to treat trees major, I first STM the muscle and that helped his progress. So then I started to stretch it specifically and that helped even more. To stretch it I stretched the shoulder into flexion, but tacked down the scapula

    Can u do a video on the different branches of PT like pediatric, ortho etc & different types like a traveling PT, or working with veterans for the government etc

      YESSS!!! I have it in the works right now and I'm working towards getting that out soon. Thinking of making separate videos for them or into one big video.. What do you think?
