I think the setup is at fault. I imagine this guy was told it would be okay and that everyone else was doing the same thing, so whoever set this up is the one at fault.
I agree, in the military you follow your training, so if they assume the guy below is going to be on top of things, then that's the fault of the system.
His fall was slowed quite a bit by the line. Even though butterfingers let go of the line, it still applied enough friction to save his life. If you attempt this, I recommend you don't go ribs-first.
