Yeah, Guts works the way you said it does. The way the attack thing actually works is that at the start of the players turn, the enemies get a set "turn count". The exact one varies depending on what youre fighting, but as a rule, bosses get 3, while enemies get 1 each. Each attack or regular skill the enemy does (no gimmick/break bar skills) consumes one turn - this is also why bosses sometimes get a bunch of attacks if you nuke their adds in your turn, the count just carries over since its set when your turn begins, not theirs. The sole exception to this rule is if there's one servant left and a guts is popped; then the turn ends immediately. Popping a guts also forces a retarget, but doesnt lower the turn count more than usual. This means Kirei will usually eat 3 guts checks each turn. Thats a 51% chance for him to die once his guaranteed guts has popped, or a 64% chance if he still has it. Also I'm definitely grabbing him; he's a half tank with a fairly offensive setup, which means he can draw attention from other enemies, either by being the first tank so that others can get their buffs/gauge up or by allowing hyper offensive servants who lack safety tools in their kit to not have to eat the full attack stat of a strong berserker. (Current Karnamas 90++ on NA has a pretty scary Raikou for example.)
So I found that guts trick where an enemy skips their entire turn works only on your very last unit. For example as a first action enemy aoe nps your frontline but one servant survives, with 1 hp guts, enemy will then attack and kill them if you have anyone else in the backline, but skip their turn if none's there. If a servant with guts dies from 1 hit but also has a taunt enemy will skip 1 action and attack again. When a servant with taunt dies, the enemy turn is usually skipped.
It's funny how some servants are immune to dots, not the spread or how some are not immune to dots that would make sense like Serenity when her entire body is covered in poison
