- The Second Opinion is a community-created cosmetic item for the Medic. It gives the Medic a discolored, pale skin graft on the left side of his face. This graft is held in place by a line of visible surgical staples. These staples extend from the Medic's throat, up along the middle of his face, and around the skull to the back of his head, where they disappears under his uniform. The graft also alters the Medic's signature forelock, making it slightly longer and droopy, and turns his left eyeball white.

Just imagine if there was another cosmetic on Whole Medic called "Second Option" where his second self has taken over his whole body, making that Medic from Emesis Blue in the moment where he comes up from the coffin with that cold look and tired face. A true definition of Battle Medic's mind if they had any offense weapons

you could actually give the "Second Opinion" three different styles, and then you dont have to use another cosmetic slot, meaning you could have a full cosmetic set, which i came up with earlier:
The Mad Doctor cosmetic set concept;
Second Opinion, with three styles; style one, normal left half is evil, Ludwig will be the main voicelines while Evil Ludwig will be only every now and then. style two, swapped where right is evil, Evil Ludwig will now be the primary speaker, while our Ludwig will either add on details, notes, or discourage Evil Ludwig from killing his friends. style three, fully evil, all voicelines will be Evil Ludwig, every sentence is cruel or dark, his voice is always that deep and dark tone, and he mutters to himself about harming his fellow teammates, and the enemy team.
the other pieces of the custom cosmetic set i came up with would be the outfit, meaning his labcoat and pants/boots, personally i think the coat would have the same, or similar, styles that i came up with for the Second Opinion, normal being white on our Ludwig side and black on Evil Ludwig side, same but swapped for style two, and then style three would just be a black labcoat, probably stitched up, scratched, and a bit blood splattered for some little detail. when it comes to the pants, i got nothin', but if anyone has any suggestions, feel free to share, im hoping to turn this into either a mod, or get this into TFC. thank you for taking the time to read this if you got to this point, it means a lot, and i always love sharing my custom TF2 ideas...! have a wonderful day, and dont let merde-heads ruin your fun when playing...!
