Yes its a National Highway above 10000 feet from sea level. Natural calamities are very often in these areas mainly landslide that's why road always will be broken.

Technically it IS a highway, and a national highway aswell. Travelled Manali->Leh August last year and Rohtang pass was the worst experience I've had in my life, but when you've passed it the (still sucky) roads feels like a 5-lane highway.
They'd have to re-pave it after just about every monsoon or skyfall as the roads collaps so very often. Good news though! They are building a tunnel straight through it now! (feels wierd to say "build" a tunnel, but... :)). So when that's complete you have a all-year-round access to the Himalayas (in a way)! Yay!
***** you are right about USA lanes. In India/Nepal/China, the mountains are very high, a lot higher than those of USA so making lanes would be impossible in those conditions!

What do you expect 8 kms above sea level, which is porne to land slides and experiences some of the highest rainfall and glacial melts in the world. And not to mention its a very remote inhabitable place where temperatures plummet to -30 :)
Sorry, but riding on a Highway at such great heights is really creepy to me. just look at the guy trying to get through from that narrow space and above that slippery-sludgy road! A small imbalance and...O.o -_-
