I am 8 weeks on trizepitide and slowly worked my way up to the starting dose. It's working, I'm losing slowly but very mild side effects. I only have 25 lbs to lose and I am not a diabetic, so this works for me. Thank you for explaining the half life, now I'm not afraid to take it every 5 days when the hunger and food noise returns.😊

This is so important to understand. I use IVIM tirzepatide and I microdose. Here is my schedule for the last five weeks: Week one .3mg Week two .5mg Week three .5mg Week four .8mg Week five 1.0mg. I told my provider at IVIM and she freaked out and said I need to get up to 2.5mg ASAP lol. I plan to stay at 1.0mg to ride out the FDA tirzepatide issues and see what happens. I have felt fantastic at the low doses. No side effects and my brain feels so much better. No brain fog, no ADHD symptoms, no feelings of inflammation in my body. I haven't lost weight but I don't care because it takes a while for it to work.

I am 47 years old female in peri menopause, started pre diabetic with A1C of 5.8 in 2/24 and hypertensive. 5’1” 150-155 lb for the last decade. I’ve always struggled with my weight. Started Tirzepatide at 2.5mg in May, titrated up by 0.5mg each month and now on 4mg/week. I’ve lost 27 lb, my body fat % went from 31% to 21.4% via DEXA scan. My A1C is now 5.1, and sbp now low 120’s from mid 140’s. I’ve been an ICU RN for 25 yrs and see everyday the chronic disease and demise that hyperglycemia and obesity cause in humans. This medication has given me my life back. Now as I get close to maintenance, I will begin experimenting with extending my dose and or decreasing weekly dosage. My provider had me stack Metformin (500mg 2x day) with tirzepatide. Should I be concerned about this combination with regard to hypoglycemia? I am pre-diabetic (5.7 a1c). 61yo. 68 inches tall. 319 is current weight, down from 358 (June 24th start). Through trial and error and the glp-1 plotter, over a number of weeks, I am dosing 3mg every 4 days and plan on staying low and slow. No side effects. No food noise. Good satiety, and hunger is appropriate for how much and how often I eat. It's a great day steady state for me. Love your health philosophy. Thanks! 🙂.

Thanks for the question! There is no scientific evidence or data to show that cycling GLP1s improves its efficacy. If you are using it for blood sugar control and anti-inflammatory benefits then I would definitely not cycle off. It is more beneficial in the long run to keep your blood sugar controlled and the inflammation at bay. Good luck!

I use Mounjaro to manage food noise/cravings and as a byproduct lose weight. Now 7 months along I have lost 45 lbs. but I have had to continue to titrate up my dose to maintain the food noise management. I am now at 10 mg weekly and the side effects are now problematic. I am interested in microdosing but have trouble understanding how 1 mg/week will manage my food noise when 7.5 mg currently does not. Thanks Tony

Practically is there any reason to titrate up if food noise and appetite suppression are fine even if weight loss is low or stalled? Maybe it's more effective to focus on ramping up fasting, diet, sleep, exercise, movement instead of titrating up as long a food noise is low? Thanks
Great question. As long as your appetite and food noise is well controlled I would not increase your dose. The only other reason I would increase a dose is if your blood sugar is not well managed on the current dose. There are many reasons you may experience a weightloss stall so I would try increasing activity and making sure you are tracking your calories properly. Also weightloss is not linear and stalls are unfortunately inevitable. I dont consider it a stall until you haven't lost ANY weight in over 2 months with proper calorie tracking. I ordered compound semaglutide. The provider is sending it in 5 mg vials. If I microdose it will take quite a while to go through one vial. The provider says that once opened, the medication expires in 28 days, even if kept in the fridge--and the vial should be thrown out after 28 days. I am having a hard time believing this and I want to continue using the same vial for cost saving purposes. Do you have thoughts on this?
The lifetime of the diluted peptide is Limited by the contamination by bacteria and fungi, which will eat the peptide much slower at low temperature. Each time you are using the vial, you will add contamination! Use gloves, alcohol and a Face mask to protect the vial and keep it sterile
I'm on Mounjaro 7.5 mg and I'm still ravenously hungry. I don't think micro dosing is for everyone. My weightloss has been pretty slow going lately and Im a few weeks shy of 5 months on Mounjaro. It worked great in the beginning but now not so much. My APRN increased my dose due to decreased hunger suppression each time. In total I've lost about 40lbs. Most of that on 2.5mg and 5mg. And yes I eat a good amount of protein each day for the most part.
whats your thought process behind your decision to prefer microdosing, only when goal weight and glycemic control is met? Via the official protocols you get one pulse a week, albeit higher, but levels are a lot less stable. Couldn't you lose weight on a more stable microdosing regime?

Hey! Sorry I had to take my email down because I was being flooded with so many personal medical questions that I can't legally answer unless we have a patient physician relationship. I would start by searching for providers in your area or asking your PCP or an endocrinologist. The exit strategy really depends on your goal. If it is weightloss, yes people are able to come off of the medication and keep the weight off. If it is for inflammation, CKD, or cardiovascular benefits, unfortunately its a lifelong medication.

Thank you for the question. Its difficult to tell exactly what could be going on. I would follow up with your primary care physician and get a set of labs done, especially to check your thyroid function since you are on synthroid and GLP1s can effect thyroid hormone levels.
whats your opinion for someone that is type 1 to get on a glp? ive read in some trials that could help reduce the amount of insulins someone would have take.
I think a type 1 diabetic can definitely benefit from GLPs. The trials only included type 2 diabetics so that is what they are currently FDA approve for but you are right, they can improve insulin sensitivity which leads to needing less exogenous insulin. It will also minimize the highs and lows that T1 diabetics often see when it comes to blood glucose throughout the day. Also alot of type 1 diabetics also struggle from other autoimmune issues so GLP1s can help reduce inflammation and regulate the immune system as well!

I have 1 ml syringes and I’m so confused on what 2.5 mg is on these syringes. If you can explain or even make a video that would extremely helpful!!! No one has done that I can find.
It depends on the amount of peptide and the amount of liquid ( usually bacteriostatical water) it is diluted in. Knowing this, You can use a free peptide calculator app to calculate how much of the solution to draw into which size of syringe, to administer a certain amount of peptide . Have you heard of akkermansia.. I was listening to another doctor that works with people in GLP-1 and he was discussing the benefit; I believe it is a prebiotic and probiotic that helps your body produce GLP-1 he also discussed being successful in using metformin in helping his patient maintain there weight loss .. he has had success in them off GLP-1 after 4 mths and use metformin for a period of time to let their resistance subside and then back on GLP-1 and increasing from starter dosages again (cost effective also) and some patient using both medication together . Wondering what your medical opinion on this method is?
I have a question. once you puncture the vial of compound Tirzepitide, how long can you use it? I’m microdosing and my vial will last a few months but I’ve heard it needs to be tossed after 28 days??
I looked that up on Google and found that it can last 21 days out of fridge and lasts 1 year in fridge. Google refrigeration of tirzepatide or whatever you are taking

I have the same question, conflicting info out there. The general advice is that the freeze dried powder before you reconstitute can last a couple years (some say indefinitely) if kept in the freezer. Hydrated ie liquid vials breakdown within days at room temp. And info is most conflicting whether they loose effectiveness at a week or month or a few months in the fridge. Some say you can decant into smaller amounts and freeze the liquid into monthly batches. Drug and food companies often use expiration dates to drive sales when the product actually lasts much longer so what is the true lifespan of this peptide is the question.
I am currently on 10mg - month 8, down 66 lbs. Can I go back to a lower dose and still have any benefits? I think I should lose another 50 lbs. I was not pre-diabetic nor had any other chronic issues except for lymphedema when beginning. I still have swelling in my ankles but it is so much better.
What compounding pharamcy is legit and safe for these medications? I have a PCP who will work with me, but I have no idea where to purchase medication that is safe.

Everyone is different. My anxiety went down tremendously. Usually with things happening at work, I'd be overwhelmed and emotional. Now, I'm overwhelmed and my attitude is just to do my best to get it done and go for a walk to destress.

The lifetime of the diluted peptide is Limited by the contamination by bacteria and fungi, which will eat the peptide much slower at low temperature. Each time you are using the vial, you will add contamination! Use gloves, alcohol and a Face mask to protect the vial and keep it sterile

I'm on Mounjaro 7.5 mg and I'm still ravenously hungry. I don't think micro dosing is for everyone. My weightloss has been pretty slow going lately and Im a few weeks shy of 5 months on Mounjaro. It worked great in the beginning but now not so much. My APRN increased my dose due to decreased hunger suppression each time. In total I've lost about 40lbs. Most of that on 2.5mg and 5mg. And yes I eat a good amount of protein each day for the most part.

whats your thought process behind your decision to prefer microdosing, only when goal weight and glycemic control is met? Via the official protocols you get one pulse a week, albeit higher, but levels are a lot less stable. Couldn't you lose weight on a more stable microdosing regime?

Hey! Sorry I had to take my email down because I was being flooded with so many personal medical questions that I can't legally answer unless we have a patient physician relationship. I would start by searching for providers in your area or asking your PCP or an endocrinologist. The exit strategy really depends on your goal. If it is weightloss, yes people are able to come off of the medication and keep the weight off. If it is for inflammation, CKD, or cardiovascular benefits, unfortunately its a lifelong medication.

Thank you for the question. Its difficult to tell exactly what could be going on. I would follow up with your primary care physician and get a set of labs done, especially to check your thyroid function since you are on synthroid and GLP1s can effect thyroid hormone levels.
whats your opinion for someone that is type 1 to get on a glp? ive read in some trials that could help reduce the amount of insulins someone would have take.
I think a type 1 diabetic can definitely benefit from GLPs. The trials only included type 2 diabetics so that is what they are currently FDA approve for but you are right, they can improve insulin sensitivity which leads to needing less exogenous insulin. It will also minimize the highs and lows that T1 diabetics often see when it comes to blood glucose throughout the day. Also alot of type 1 diabetics also struggle from other autoimmune issues so GLP1s can help reduce inflammation and regulate the immune system as well!

Very few providers support microdosing unfortunately. Your best bet may be trying the online compounding companies.
I have 1 ml syringes and I’m so confused on what 2.5 mg is on these syringes. If you can explain or even make a video that would extremely helpful!!! No one has done that I can find.
It depends on the amount of peptide and the amount of liquid ( usually bacteriostatical water) it is diluted in. Knowing this, You can use a free peptide calculator app to calculate how much of the solution to draw into which size of syringe, to administer a certain amount of peptide . I was listening to another doctor that works with people in GLP-1 and he was discussing the benefit; I believe it is a prebiotic and probiotic that helps your body produce GLP-1 he also discussed being successful in using metformin in helping his patient maintain there weight loss .. he has had success in them off GLP-1 after 4 mths and use metformin for a period of time to let their resistance subside and then back on GLP-1 and increasing from starter dosages again (cost effective also) and some patient using both medication together . Wondering what your medical opinion on this method is?
I have a question. once you puncture the vial of compound Tirzepitide, how long can you use it? I’m microdosing and my vial will last a few months but I’ve heard it needs to be tossed after 28 days??
I looked that up on Google and found that it can last 21 days out of fridge and lasts 1 year in fridge. Google refrigeration of tirzepatide or whatever you are taking

Hi, how do you measure the microdose?
I have the same question, conflicting info out there. The general advice is that the freeze dried powder before you reconstitute can last a couple years (some say indefinitely) if kept in the freezer. Hydrated ie liquid vials breakdown within days at room temp. And info is most conflicting whether they loose effectiveness at a week or month or a few months in the fridge. Some say you can decant into smaller amounts and freeze the liquid into monthly batches. Drug and food companies often use expiration dates to drive sales when the product actually lasts much longer so what is the true lifespan of this peptide is the question.

I am currently on 10mg - month 8, down 66 lbs. Can I go back to a lower dose and still have any benefits? I think I should lose another 50 lbs. I was not pre-diabetic nor had any other chronic issues except for lymphedema when beginning. I still have swelling in my ankles but it is so much better.

Everyone is different. My anxiety went down tremendously. Usually with things happening at work, I'd be overwhelmed and emotional. Now, I'm overwhelmed and my attitude is just to do my best to get it done and go for a walk to destress.
I have had anxiety for over 20yrs. I have not had one day of anxiety on this medication.
