I do follow you and love your videos. Out of topic question. A1C 6.9 Started on MOUNJARO 5mg starting weight 178lb. Lost 4lb for the first month. Up to 7mg for 3months lost 5lb total of 9lb. Then went up to 10mg no weight lost for a month. Eating healthy, fasting, exercising doing all the "rigt things' just a little constipation no major side effect. Started in 9/9/24 and now 1/5/25. Total weight loss 10Ib is very frustrating, I have not yet checked my A1C but BS numbers are great total average of 101. Should I increase my dose? I know you'll say talk to my PC I will but what's your opinion? Please

I came to your channel by chance and am grateful to the algorithm for the connection! As I approach my goal, I am looking to titrate down to a maintenance dose and am ever so grateful for your expertise and generous sharing of resources and insights.
Does GLP1's put any markers on the red blood cells? I just about died from Warm Autoimmune Hemolytic Anemia. I had some sort of markers on RBC's and my T cells starting attacking them faster than I was producing them. I am curious if any markers could cause this condition to return.
I use Mounjaro to manage food noise/cravings and as a byproduct lose weight. Now 7 months along I have lost 45 lbs. but I have had to continue to titrate up my dose to maintain the food noise management. I am now at 10 mg weekly and the side effects are now problematic. I am interested in microdosing but have trouble understanding how 1 mg/week will manage my food noise when 7.5 mg currently does not. Thanks Tony

Interesting! I do believe everyone's maintenance dose (and weight loss dose!) Is different. I'm currently in maintenance on 7.5mg every 7 days. I briefly tried 5mg and felt too hungry. But I haven't tried playing with the days, ie dosing every 10 days or every 5 days. Also, I may want to lose more weight! How did you know when you were at your goal?

Great video. Can you tell us what your titration profile looked like on the way up? What worked well for you?
Absolutely. I started at 1.5mg every 7 days and actually had decent food noise suppression from the start. That has always been my issue so getting rid of that stopped me from binging and I was actually able to control my diet without much else. I eventually moved up to 1.5mg every 5 days after 5 months and then up to 2.5mg every 7 days after another 5 months. I'm currently at my goal weight of 200lbs so I titrated back down to 1.5mg every 7 days.
My concern about staying on this "forever" if needed is that the GLP-1 receptors may eventually downregulate, causing rebound hunger even while on the drug. What are your thoughts on this?
Good question. There isn't any data to really support this theory so the only thing I have to go off of is the fact that alot of my patients (myself included) have been on GLP1s for almost two years and have never noticed the type of situation you are describing. Yes GLP1 receptors do down regulate which is why there is a titration up period and eventually why the appetite suppression stops but even at maintenance I've only noticed normal hunger returning, nothing that I would consider as "rebound hunger". If anything I feel like I would be the type of person who would be prone to that because I have significant history of binge eating and since starting GLP1s I haven't binged once. yes there are days where the appetite suppression is less and I've eaten more, but never to the point of oblivion, which I used to daily in the past.

Thanks for this Video. I decided to reduce my dosage (First Step from 7.5 to 6.5). Every seven days is easy for me - in order to avoid getting confused with my next shot day. Next step will be 5.5 mg… I like using the plotter to better understand what‘s happening and to be able to make a plan.
Perfect! Yea I find that every 7 days is easier as well because its so easy to lose track of shot day otherwise. Let me know how it goes!

What strategies do you recommend for individuals to maintain their weight without the medication if they decide to come off?
Thank you for the support and kind words! If you have to come off I definitely recommend continuing scheduled resistance and cardiovascular training, being conscience of what you eat and making sure you stay on top of your blood work routinely. I truly believe that health starts at the cellular level and builds up and I think your bloodwork is the perfect way to keep an eye on that!
Thank you for taking the time to make another awesome video! All your videos have been so valuable to me on my journey with Tirzepatide. I also respond to a much lower dose than is recommended and am losing 2 pounds per week so far. I'm not sure why but I was unable to get the plotter you linked to work, but I found an app called Shotsy that works the same way. It is a game changer to see how the levels decrease over time and correlate that with hunger and other symptoms. Now I am dosing Tirzepatide every five days. Please keep making your informative videos!!

Been on Semaglutide for 5 weeks with good results one thing I have noticed recently is ensuring I'm eating enough (I track my food daily) are there cases of people not losing weight because they are not eating enough calories while on Semaglutide? Also in regards to exercise - can you provide some general guidance on exercise routines - for some reason I'm worried of losing weight too fast if I exercise while on Semaglutide? Do you also have any thoughts on vitamins/minerals people should take daily while on Semaglutide? Thanks!

Hi doc great video! I was wondering if you were ever able to check out Dr. Jason Fung, and if so, did you form an opinion around his idea of muscle wasting during fasting as a survival imperative? I have another question for you. How would you advise me to find a Dr. to help me in my GLP-1/weight loss journey? I got my cardiologist to write the prescription for Mounjaro, but I don't think he's really up on the nuances of the drug and common treatment regimens. He's a great Doc, but this isn't his wheelhouse. The same is true for my PCP. Any thoughts you might have on locating someone credible, even if it was telehealth, would be awesome!
Hey Scott, I'm sorry I missed your comment earlier. I haven't had a chance to look into Dr. Fungs material but I have a week off starting tomorrow so I'm hoping I can look into it more after the holidays. As far as finding a provider thats well versed in GLP1s I would definitely look into endocrinologists in your area. They were the first to get an influx of patients on GLP1s after it was initially approved for T2DM so they should have alot of experience with it.

Any thought on 15 mg Mounjaro and haven’t lost a pound in 2 months no side affects and a little food noise. Type 2 diabetes. Dr suggested phentermine any thought?? Thank you!
These are all just a tool. It's impossible to lose without weight training and a proper diet.

Ugh I'm sorry to hear about the weight loss stall. Is this your first stall or has it been hard to lose weight on all of the doses? If its your first stall I would say give it some time, maybe even try increasing your calories for a week or two and then cutting back down. If you've had a hard time from the beginning then you may be a poor responder unfortunately. I would still try sticking it out though because the metabolic benefits are definitely helping in the background even if the scale isnt budging. You can definitely try phentermine but its an old school drug, lots of side effects such as poor sleep and jitters.

I did not like it. I think tirzepatide is the best for weight loss. I gained 1 lb on reta during my first 2 weeks. By the end of the month, i was only down 3 lbs. With tirzepatide, i lost 8lbs on week one. The swelling i had on retra concerned me so much i couldnt wait to get off it.
How can I find a provider who is skilled with peptides? I am getting my GLP1 through an online provider, so they are probably not a solution, and my PCP is clueless in this area.
