If it's the military's tradition, then how come in every ceremony, the host addresses the other member of the official party by first name. I do ceremonies with top brass in the air force on a regular basis. Trust me when I tell you it is a typical occurrence. It is not a sign of disrespect. He was likely introduced with his full title in the very beginning, then referred to as Dakota from that point on (except in the medal citation where he would have been referred to by rank and last name)
