    In this video, we’re going to talk about bile acid diarrhea.
    Bile can cause diarrhea as a side effect. Why? Because bile inhibits H2O reabsorption.
    Since too much bile can cause diarrhea, you don’t want to take purified bile salts if you already have diarrhea-it can make matters worse.
    The gallbladder stores bile, so your body has plenty when it needs it. Many times when you have the gallbladder removed, there’s no way to regulate the release of bile. This can cause too much bile in the small intestine, which may lead to diarrhea.
    97% of your bile is recycled for future use. If you have damage to your intestines, you may not be able to reabsorb and reuse bile. This can cause you to have more bile in your intestines than you need. If you have this situation, it could be a sign of IBS or SIBO.
    You can have the bile levels in your stool tested to check for high levels of bile.
    Consuming fiber may help with symptoms. Fiber can bind with bile and potentially help regulate your stool. You want to cut back on your fat consumption. Fats stimulate the production and release of bile.
    A low FODMAP diet may also help with a bile acid diarrhea situation.

    I had my gallbladder removed the end of 2022. I was doing fine a month after it was removed, but then I started have Bloading, diarrhea, gas excessively months later. I found that oily foods made the symptoms worse. I would wake up in the middle of the night with diarrhea ongoing. I was so afraid of going out in public places with the fear of having to pull over to a gas station because the diarrhea was uncontrollable. I started drinking 1 teaspoon of ACV in the morning diluted in 16oz of water before I eat. I repeat this at night after my last meal. And I still avoid lots of oily foods. I no longer have bloading, gas, or diarrhea. I feel as good as ever. I give Glory to GOD issue Resolved.

      Nice also eating bitter herbs with fats like dandelion or chicory anything with bitters and use acv or lemon juice with the butter greens. This is why the world over the olden people ate a salad befire their meal to prepare the stomach ..it also helps to eat raw beifre cooked as it prevents the gut reacting in an inflammatory immune response to cooked food. So a bitter green leafy raw salad with lemon or acv to help the oxalic acids in the leafy greens have more bioavailabke and less stone creating oxalic acids and minerals... and then see if you can tolerate some fats. You do need the fats so don't not eat them but if they dint get dissolved well instead if you getting them the bacteria in your gut will get them and bloat you out. You can also eat things like they used to in the old days always eat butter herbs with different meats and bitter berries like red currants ... the great grandmother's of all the world have similar advice like this and now science is catching up with them wisdom of the old wives talles.

    I had this issue for nearly a decade. I saw several doctors and they kept talking about "diet". I kept explaining that it doesn't matter what I eat, I have immediate need to go. One even tried to give me antidepressants 😠, like it was in my mind. It was almost debilitating because I literally went all day without eating until I got home. Finally one doctor understood because the same happened to his wife after gallbladder removal. He prescribed 4 grams of cholestipol a day (no, I didn't have high cholesterol). Changed my life completely! I can now eat with friends and no longer have to pull over at gas stations.

      If it helps, some antidepressants are recommended for diarrhea to help slow gut motility. So they might have suggested it because of that, not because they didn't believe you :)

      @Debra I feel you 100%, it's not your fault, its fam so its hard to drop em so pls for your own well being talk to a professional about self care/health & learn to properly handle such beings. Excercise is a big stress reliever with proper nutrition & sleep.

    I’ve been through the up and down of bile acid diarrhea. I’ve found that the more I acidify my stomach ( with betaine hydrochloride), the more the diarrhea goes away. Hope that helped someone!

    This explains chronic diarrhea after having a bowel resection because of nine feet of dead small bowel caused by Crohn's disease. In the years since two resections and two gastroenterologists, no one has explained this like you.

      Me too. I basically had to figure it out on my own. I feel like a lot of GI doctors need to get refreshers courses on common GI issues.
      I had a urologist tell me after I got a kidney stone, that having issues with constant diarrhea could cause kidney stones. My GI Dr had no idea about this.

      I’ve had two small bowel resections and always have problems with loose uncontrollable bowel movements! It has ruined my life ! Any improvement or advice ? I try to eat apples and celery . It kind of helps

      There is a Medication called : Colesevelam that should bind to the bile acid reducing its quantity in the stool.. just try and read on it

    Between having gastric bypass and gallbladder removal I had chronic diarrhea. It was debilitating. My Dr prescribed colestipol. Game changer!

    What a gem of a channel! Thank you Dr. Berg for all you do. One of us in the family is on a Keto diet and the other had their gallbladder removed and suffers from this, and so we're challenged with how to eat together (because of the fat issue). How can I incorporate the fat in my diet so I can remain on Keto without exacerbating the issue for the other person?

    My doctor prescribed Prevalite powder for the bile acid diarrhea since it’s a binder. And so far it’s helped with that end. I usually mix it with a smoothie twice a day and you can’t even taste the texture of the powder which is nice I mix prune juice in that as well. That helps with constipation this can be a side affect from Prevalite ❤ prayers for all of those who are dealing with this issue because I know it’s a rough road to walk down. But you’re not alone. Oh, and make sure you brush your teeth really well after using prevalent because it can cause tooth decay from what I read online.

    Very interesting. I didn’t know that there were times when you should not take bile salts. Thank you for information on fodmap vegetables. My brother has Crohn’s disease and he is using FODMAP (Fermentable Oligosaccharides Disaccharides Monosaccharides And Polyols).

      I have this condition and take bile salts daily to help my digestion, it doesn't make it worse for me, maybe because my bile binder deals with it in the same way as my own bile.

    I believe i had bile acid malabsorption, and mistook it for IBS. I was taking the Gallbladder Formula and I think was adding too many bile salts which made things worse. I had a large mass in my large intestine that ruptured and had to have emergency life saving surgery, where I woke up with a colostomy bag, that I had to wear for a year and a half. They removed 5ft of my small intestine and 2ft of my large intestine because they fused together. I wish America would have the SeHCAT scan like the UK does

    Wish I'd seen this before my Mum died. She had her gall bladder removed & the rest of her life had tummy issues which ruined her life. Thanks for helping others though. I'm beginning to think that I can't believe anything we've been told anymore by the NHS etc. Your video on acid reflux was very illuminating too. Been given the exact opposite treatment to what was required. Have started taking apple cider vinegar...& symptoms have completely gone! Throw the antacids away folks!!

      @@athomewithmelissa Hi Melissa. I'm new to this but have been taking 30ml in a mug of water. I drink it through a straw to prevent it touching my teeth & possibly damaging the enamel. I've been taking it daily. I've alongside this been taking a smoothie made of organic kale strawberries & almond milk...but think I possibly changed my diet too rapidly trying to get loads of the 'good stuff' in me then felt too full, too close to bedtime resulting in a return to the symptoms for one day since I wrote my comment! Maybe it's a combo of things that work so I'm now not eating too late at night or big meals & not drinking pints of green juice!! Hope that helps.

      @@amyhernandez5431 Thanks for your kind words. I was surprised with all Mum's tummy issues she definitely didn't have cancer at least. She used to yo yo between constipation & diarrhoea. She became intolerant to so many foods, vegetables, bread tomatoes, grains, fruits with bits such as raspberries etc. In the end she was hungry but at a loss as to know what she could actually eat. Sometimes simply bending over to put her shoes on for example seemed to 'set her tummy off' she daren't leave the hse for long in case she needed the loo...it was very sad to see & no Dr ever helped her...they just gave more & more tablets for all the symptoms...which had side effects causing more symptoms!! My advise, steer clear of all pharmaceuticals if possible ..Good luck. I'm sure if you eat a healthy balanced diet you will be free of colon cancer...sometimes I think worrying & stress is worse than smoking 50 cigarettes a day & continually eating junk food added together!!

    I had my healthy gall bladder removed when the diagnosis should have been coeliac disease. I still get bowel problems, a recent gastrostophy revealed regurgitation of bile into the stomach. I was prescribed PPIs. They seemed to make the bowel problems worse. I reasoned that taking 10mls of oil may give the bile something to work on, and yes it did alleviate my symptoms. So 10mls of cod liver oil first thing in the morning alleviates the indigestion feeling on an empty stomach. I eat a healthy diet but also take vitamin B complex as my diet does not include much. I was never advised that vitamin and trace element absorption is compromised by both villi damage and bile availability. I am a farmer who had read up on animal nutrition and disease profiles affecting failure to thrive. I often think doctors and vets should get together more often.

    This might be the video I needed at this exact moment in time. I was sent home from A&E with suspected gastritis, given Lansoprazole (PPI) and guess what, a month later I'm in agony again. Bloating and everything. I also have no gallbladder so this info was a real lightbulb moment for me. Thanks Doc, I with the NHS had more like you.

      I had a follow up with my Gastro Consultant after an Endoscopy etc due to having chronic gut issues for the last few years, they’ve put it all down to IBS and I asked if it could it be bile reflux due to my Gallbladder removal in 2018 to which he replied, oh I didn’t know you had it removed 🤦‍♀️, this is so worrying to think he hadn’t read my notes properly, I have no faith in the NHS anymore

    They tell you none of these things when you have your gallbladder removed. Mine was removed 7 weeks after my last pregnancy, 17 years ago. Never know when I’ll be running to the toilet. 🤷‍♀️ But already had IBS issues before it was removed. Not much has changed, really…🧐

      Probiotics in fermented foods such as a spoonful of kimchee, saurkraut, yogurt, etc. to balance gut flora is an important part of digestive and immune health. When I don't pay attention, I rebalance with the above or take a coated pearl from Shaklee.
      My liver is short a few enzymes of a full deck. It showed up in pregnancy with bilirubin and a little jaundice in my eyes. So, if there's a fatty or protein rich food once in awhile I use EZGest which has a variety of enzymes for different foods, or if my tummy is blowing up from acid, on occasion, I take Stomach Soothing Complex. It is a blend of ginger, peppermint and anise. Within 10 or 15 min. the distention goes down and the burping releases that painful gas.
      Things like carbon dioxide processed decaf coffee was an instant message. NO. A coffee guy at the local Grower's Mkt. said it turns the beans gray.
      Also, marshmallow root tea coats and soothes the intestines if they're inflamed. Good for IBS, etc. And a Uti or bladder infection. The mucilage coats the bladder walls and colon. Much better than meds and surgery.
      Remember: What comes in a box, can put you in a box. Let food be your medicine.

    Had this happen to me after I had my Gall bladder removed when I was 21. I had small stones that kept getting stuck in the ducts and cause excruciating pain where I could hardly breath. They never tell you there will be any side effects. After about 6 months I started having horrible smelly gross diarrhea everyday couldn’t hardly function had to always be by a bathroom. After about 10 years a gastroenterologist figured it out that it was to much bile. I started taking Colesevelam 3 to 6 pills every day. It binds your bile so it doesn’t reach your large intestine it was like an instant miracle. It has been 30 years and It still works. I have to take it everyday if I miss a single day I will be sick the next day. I am just so thankful for something that works I was sick for so many years.

      Doctor's (who work for institutions) can't give you advice that is not out of "their playbook"
      Example: lots of colon issues in my family. Told the doc maybe I should get a colon scan now that Im 44, he said no we do them at 50+ unless certain conditions are present. So even if he thought it was a good idea, he can't unless I have the appropriate symptoms first. His institution won't let doc do what he thinks is best, he has to follow a set of guidelines. The guidelines probably minimize liability for the institution.

    I had this problem for a long while... which created an internal hemorrhoid problem. Terrible! I started drinking ACV and both issues were resolved. Now, I just have a belching issue, which I'm working on with the help of your vids. Thanks, Doc!

      Wow!! You’ve described exactly what I went through and did to remedy the issue. Acv and lemon everyday. 2 keto meals ad 18 hrs of fasting a day has helped immensely. Thanks to Dr. Bergs videos!

    Psyllium husk, Bentonite clay and activated charcoal seem to help a bit. Still haven't been the same since I had my gallbladder removed five years ago. It's been especially much harder for me to gain muscle no matter how much I work out and eat.

      Thank you for this comment. I had my gallbladder removed in 2019 and was fine until a year later and I have lost strength in my proximal muscles and gained 50 pounds. I have been blaming it on stress, thyroid/cortisol hormone imbalance, I didn't make the connection to my surgery or malabsorption.

      I just recently was out on Colestipol. I have the SAME issue since 1998! After a month and half of taking 1 gram at night with water I feel so much better! Cured? No, but I’m not spoiling myself! It’s been terrible living without a gallbladder 😭😭😭

      Mine didn’t flare up until 13 months post GB removal. It was a week of yellow diarrhea. Then cleared up for a year and it is now back…not sure how to fix it.

    That diarrhea after gallbladder removal is a but kicker! I have to drink cholestyramine powder with water twice a day to live a normal life! And still cut back on fatty meals and eat small portions 🙄

    I’ve been having terrible issues with my digestive system this year. It started out with pain in my right side accompanied by diarrhea and then loose, watery stools which leave behind either a distinctive yellow residue on the toilet paper, or nothing at all. Extreme bloating and gas is also an issue.
    I can’t eat anything high in fat like nuts, avocados, red meat etc without experiencing chronic diarrhea many hours later. Chocolate and dairy also seems to elicit the same reaction.
    I’ve also lost quite a bit of weight which seems to be mainly muscle from my glutes, plus a lot of aching in my glutes and pelvis which intermittently pops up and then disappears.
    I found out that taking digestive enzymes with betaine hydrochloride helps with every meal, but I bought some double woods Tudca the other week and found that after two days of taking 2x 250mg tablets on an empty stomach in the morning. Resulted in extreme diarrhea on the evening of the second day.
    After this reaction I’m starting to think it might be Bile Salt Malabsorption. Has anyone else been through something similar to what I’m describing?

      @@giselleaberdeen3697 Avoiding trigger foods, taking digestive enzymes before meals, and taking charcoal to ease the excessive gas. Bromelain and taurine seemed helped to firm up my stools as well, but I’ve since run out.
      I’m far from fine, but these supplements seem to help take the edge off it somewhat.
      As for stopping the yellow colour, nothing seems to help, it’s been constant throughout 😕

      @@Libertycapuk Honestly it's the same for me, the colour is so puzzling. I've had tests done, especially for bile/gallbladder stuff but everything comes up negative. I'm really curious about taking digestive enzymes before meals as I think that might also help me. Which ones do you take? And by charcoal do you mean the activated charcoal powder? That might be something I could try for the pain I get afterwards. Just praying to get my life back, really ... Xx Thanks for replying.

      That is true. Symptoms overlap.
      Thought I had SIBO but it probably was sludgy bile.
      It is important not to jump to conclusions

      @@shawnmulberry774 I do think I have SIBO. Antibiotics (never used to) and anything with sugar in it causes diarrhea, I have rashes breaking out in my and around my mouth which can be treated with zinc oxide cream. I've read medical white papers of patients who had this and were treated with certain antifungals and improved. To date I can't get any doctor onboard with helping me try this, hence my symptoms are now so much worse. I also had to stop taking probiotics because they were exacerbating the problem. I can however, eat organic plain whole milk yogurt. Go figure.

      @@ateachableheart2649 yes probiotics and fiber make SIBO flare up. You have to try to starve the bacteria. But if yours is aggressive you may need antibiotics for this purpose. There are doctors who work with SIBO but you probably have to pay out of pocket. Best of Luck

    Blast! I've had some kind of IBS or SIBO since I moved to Florida. This video explains this, finally some light on the subject! Thank you!! Is keto okay though? You mentioned lowering fat.... and yes, veg in general help very much. BTW monolaurin helps my version of this (SIBO??), as does Saccharomyces Boulardi, if anyone is interested.

      I had mine removed almost 17 years ago. For many years I threw up bile many mornings. It was hell.
      It Finally went away. But what lead me here to this video is I've been fasting most of the day for over a week and ingesting much less calories than usual and I know bile is building up bc my stomach has been griwling and hurting from not eating. And...the associated symptom arose today.

      Had gallbladder removed a year and half ago....so now since May I lost 12 lbs...had diahhrea for weeks and weeks and my GI doc put me on Colestipol to help...it does help a little but I'm trying to eat alot and still not gaining any weight back...it is so depressing and I feel weak...we did a bunch of tests, endoscopy and colonoscopy which was normal...now geta CT scan of liver and pancreas...this is hell honestly, I have okay days and bad days, it causes depression!

    Thank you for directing me to this video. I was taking Cholestyramine Resin- doctor said it absorbed excess bile.
    It helped somewhat.
    Dr Berg. Thank you for this video. I do eat the foods on the list and I cannot tolerate greasy foods so I avoid them.
    My biggest battle is the eczema that has burned me - endlessly and without relief- some birth.
    I fear I will develop colon cancer as my ancestors did.
    I am seeking all help that I can afford but I feel my body has suffered for so long that the damage may be done.

    Is there a benefit to growing veggies in different metal buckets?
    Like cast iron, copper, zinc, will the veggies contain more of these minerals?
    I know if you use some soil that is right by the sea, there is an increase of iodine in the veggies.
    Maybe it's only carrots this all works on?

    Thank you so much for the great info. My gut problems stem from a resectioned small intestine 8 inches removed. ( dead tissue ) bc of a Volvulus that attached to old scar tissue. I have been prescribed Cholestramine ( using for 3 years. ) to stop over production of bile salts. I want to find supplements and changes so that I can stop the cholestramine reliance. The nutritional deficiencies are showing up bc I can't absorb the good fats. I am gluten free and alcohol free.

    I used to randomly get diarrhea 20-30 mins after evening meals, occasionally with bad cramping and burning rectum (sometime very painful). Also very sleepy after ANY meal. Tried going gluten free for over 5 years... seemed to help at first, but symptoms returned so I gave up and went back to just eating normal garbage / processed food. Still had occasional evening diarrhea and many mornings 3-4 loose dumps.
    I'm in the early stages of trying a keto type diet and seem to have less diarrhea (if any) and take about 2 dumps every morning and a lot less sleepiness after meals. Sugary foods and diary also began to really mess with my head (sinus problems, eye/vision problems, dull ache/pressure in my head, brain fog, lethargy tinnitus, itchy inner ear) which clear up when stick to keto. Thinking I might have chronic fungal sinus infection and metabolic disorder.

      You might have an autoimmune condition, because you describe exactly my symptoms before I found out I had it...you will need to do an ANA test, followed by ENA test to narrow it down. Don’t go on steroids, but deal with it by keto diet, gluten free, and avoid lectins ...antiinflammatory diet basically. It’s much improved, but has to be managed constantly.

      Gluten gets blamed for many things but usually it is the grain in the bread or the yeast. Look into yeast overgrowth, my daughter had it and her journey sounds much like yours.

    I've been watching your UA-cam videos on Bile post gallbladder surgery, I've been having diarrhea issues for years and it's been worsening in the last 2 to 3 yrs... Just had an endoscopy and colonoscopy with no significant findings, now trying to get a stool sample into the lab... Your videos have really made sense to me and my situation... I was wondering about EPD, could you do a video on that? I so much appreciated your videos... Also, with my issues, I've been concerned about started a keto diet, because of the fat intake...

    I had a tumor removed from where the small intestine joins the large one, approx 18" of bowel removed... I feel sometimes food just goes straight thru me and I have Gerd and acid reflux

    Thank you so much for the information about getting more fibers. After waking up, I have normal bowel movement. then after next bathroom, it becomes diarrhea. This often happened during my keto diet last year. Too much fat created more bile juice. I love vegetables. It sounds like I not eating enough even with two servings. Any help welcome!

      vegetables are alkaline, when you eat them add some acidity to see if this helps. perhaps some lemon water will balance the ph level in your stomach. the acid production in your stomach may be a bit low so something citric. should help. wish you good health!

    Leafy greens give me serious stomach problems. I have to avoid most salads and any leafy greens. Years of trial and error helped me figure this out. I don’t understand why though.

    Dr. Berg - could being born without a gallbladder be what contributes to chronic yeast overgrowth and issues with weight loss, diabetes, etc.?

    After having part of my intestine removed (Crohns), I now must make some metamucil mixed with oatmeal the first meal of the day, to avoid the problem mentioned in the video. Having to take some oatmeal/metamucil probably isn't the best way to break an occasional 48hr fast, but we have to play the cards we are dealt. The metamucil/oatmeal is effective in removing the excess bile.

      You could use oat bran instead of the metamucil

    Thank you for this video Dr Berg! Can you do a video to clarify whether or not one can take bile salts if having Sibo and fatty liver? I’ve seen a video that says you shouldn’t take the bile salts if you have Sibo and a video that said you Should take the bile salts if you have Sibo.
    Can you please clarify? Thank you

      Agreed. This needs to be clarified. One video says take bile salts and another says not to.

    Hey Dr. Berg, I've had IBS for the last 30 years and the thing that has given me the most relief when my "stomach" hurts is eating a box or 2 or 3 or Kellogg's Raisin Brand with whole milk. This works almost every time. About 3 servings a day along side or in place of normal food. FYI I guess. Have a great day. Thanks for all the info.
