The explanation as I see it is she doesn’t take her medications so is driven to commit suicide and thus causes this accident. “ did anyone die? , i didn’t take my pills”. The earlier parts of the video are her brain trying to process the event & this trauma but she is still so dissociated from it. Real life keeps peaking through hence the painting on the wall and coded actions from others. There are so many parallels such as the guy hitting this head on the pillow which is really the guy hitting his head on the airbag but her mind keeps replaying this ( again going to the lyric keep repeating self hating phrases). The paramedics throughout the video pulling her back down as she ascend to heaven, giving her oxygen, etc. Even the the lg promo can be interpreted as a “sine from above” ( which is the instrumental from another Chromatica track which is played when the camera cuts to this sign) saying LG or lady Gaga life’s good. She’s ends by saying she doesn’t want to die. There’s soo much more

She says at the end of the video, "did anybody die?" And she also says, "I don't want to die" at the end, so I think the song and video are about her as Lady Gaga, and her as Stefani Germanotta. Like she is almost fighting the two identities that she has built. She says in the song, 'Holding on so tight to this status, it is not real, but I'll try to grab it to keep myself in beautiful places.' Lady Gaga is the flamboyant, fearless side of her, and Stefani, the girl from New York who nobody knows- and the girl at the end who is saved by the creativity, she was saved by Lady Gaga. So I think she is saying that she has been running away from Lady Gaga, but ultimately she needs her creation myth, Lady Gaga, to survive and find meaning in life. She needs to hold on to the status to keep herself in beautiful places, as well as needs to pop a 911 (her medication) 'paradise is in my hand' to calm her down. I also take medication for my mental health, and I panic a lot, and my body spends a lot of time in fight or flight mode which is exhausting and hard to explain. Her lyric 'my biggest enemy is me ever since day one' is something I have said to many therapists so many times, and to hear her say it in this enormous pop song is hard-hitting. This video is amazing! This song is about her antipsychotic pills she used to take during a period (I didn't get if she still need some but I think she's done with that because when she pulled this video out she said it's a way for her to put this hard period to the past from now) probably related to her post traumatic stress And the video is inspired by the armenian movie The Color of the Pomegranates which is a very experimental movie with a lot of meaning and this movie is a masterpiece (but not easy to understand) that inspired many recent movies However the plot twist, all the costumes, the new element she brings as the true artist she is, etc. are all her own ideas but the fact a car accident was chosen as a trauma is probably the idea of the video director that did an incredible job! The concept of “what is reality, who and where are you” is a major theme in the album. This video illustrates the dissonance between objective and subjective reality... it’s abstract enough to interpret a lot of stuff out of it which is awesome.

Thanks for this reaction! We love that Gaga’s art and music has a profound meaning. You pretty much got most of it correct. She’s raising awareness about what can happen to people who struggle with mental health. It’s basically about an antipsychotic pill she needs. When dealing with PTSD or being in a traumatic state your mind can disassociate from realty and start creating different scenarios in your head in order to cope. In the video she’s using an car accident as an example and she was on the verge of dying and shows how’s her mind registered the tragic scene around her. Everything she saw in that reality was in some form in her imagination, dream, hallucination, purgatorty.. etc what ever it was, and even though the paramedics were trying to save her, in her mind they were trying to kill her, which also tells us how the negative thoughts we have sometimes aren’t real. When she ascended to “heaven” in her dream as a metaphor of her dying the man aka paramedic pulls her down not giving up on her, the man banging his head was in the man hit by the airbag, the women with the mummy, was holding her dead child, the location was a billboard, the police, bystanders, were all in her head and so on.

I'm not sure if it was a suicide attempt or not, but in her recent interviews she talks about having suicidal ideations daily the past couple of years. If you rewatch the video again, there is a brief less than a second frame when the paramedic brings her back down to earth where it flashes to reality and she wakes up. It is very quick and right before the have her in the red blood outfit. The video is based on the 1969 film The Colour of Pomegranates. The movie poster flashes on the screen as they scan the theater before flashing down to the police officers directing traffic. This video is genius and her best one if you ask me!
