Hey, American here! While I’m not a doctor and do not currently nor have I ever worked in the field of healthcare, I do know a fair bit of history. You’re correct in assuming that operating theaters (operating rooms with spots for observers) are primarily an old school thing, they largely fell out of fashion around the mid-1900s. However, they are still used for (as you said) teaching purposes. Most hospitals do not have one and you’ll really only reliably find them in hospitals that have very skilled and renowned physicians (MAYO Clinic, Cleveland Clinic, etc) nowadays. However, they often are just an operating room with seating on the second floor, but only qualified physicians who are learning a new procedure are allowed to attend.
To conclude, a fun fact for you: surgery used to be viewed as a spectacle around the mid 1800s so the term “operating theater” actually comes from the days when regular members of the public could pay to sit and watch doctors operate on their patients. In fact, there was at least one case of two surgeons (one of which was Dr. Robert Liston at the University College Hospital in London) racing to see who could perform a procedure faster (the idea of which wasn’t without merit given without anesthesia operating as quickly as possible greatly reduced pain and improved the odds of survival for the patient). The procedure was to amputate the patient’s leg and Dr. Liston, in the rush to complete the operation as quickly as possible, cut off his surgical assistant’s fingers as he was cutting through the leg and slashed a spectator’s coat while switching instruments. Both men died of infection in their wounds and a third (uninjured) spectator died of shock because he believed he had been stabbed. That is the only recorded surgery which had a 300% fatality rate.

I had 4 or 5 surgeries at our local Hospital and none of the surgery suites had the observation balcony, but they were all big enough for 30 or 40 people and still have room to operate. My first surgery there was 4 or 5 med students watching the surgeon attempt to reattach the nerves and muscles in my right thumb and forefinger. I was cutting PPE sheets with a bandsaw and the saw caught something and bowed the band and it ripped the sheet out of my hands and yanked my right hand into the saw. Thankfully the workgloves were thick enough to kepp the sawblade from cutting both of them completely off.
