Sorry, I already uninstalled the mod so I can’t any time soon. I can tell you there are two in the building in which you fight Obediah, one on each side path, there’s one over near the farron follower camp inside a weird alcove thing guarded by one of the drunkards, another is near the cave where you fight chaos amalgamate, guarded by a drunkard and two or three minions, and the last one is to the left side of the place in a kind of sunken little area guarded by a drunkard and some guys. If you explore the whole area they’re all pretty easy to find, just be patient and look around.
I went there after exploring the entirety of the level and the worm popped up many times and did like one attack and then left. Maybe you need to trigger him in every spot? I would say just try exploring most of the level and then go to the spot at the end

why does the worm disappear at 40% hp? Do you have to kill him somewhere else to finish him off?
He doesn’t show up anywhere else in the demo but I assume in the full release he will appear again in a later part of the level
How did you get the carthus sandworm to spawn? It doesn't show up for me

If you managed to kill him in the field when he keeps popping up, he will not spawn
