You hit the nail on the head mate, hauntingly beautiful song, by the way the short haired woman used to be a blues singer and the darker character used to be a bananarama singer back in the early 90s. They are actually reviving shakes spear sister and are going to do some more songs

The first two verses lyrics are applicable to a loved one speaking to an attempted suicide

In certain circles, the general consensus is that this song - - and video, especially - - is about addiction. As it begins, the man is unconscious, apparently from an overdose.
As his life hangs in the balance, Marcella Detroit serves as the physical manifestation of his addiction, using phrases such as "You must only think of me" and "When your pride is on the floor, I'll make you beg for more".
Siobhan Fahey represents the Angle of Death (or at the very least, Death itself) with her lyrics speaking about the afterlife.
As the 2 of them are physically fighting over his fate, he emerges from his unconsciousness, and ultimately, chooses to "stay" with is addiction (Marcella Detroit’s character). At the end of the video, you can clearly see that Siobhan's character has accepted his decision but knows she'll eventually return to escort him to the afterlife at some point (probably the near future).

The girl who played the devil was drunk during the shoot. And she was normally the lead singer. They got so much notoriety from this song, she quit the group, and they didn't speak for over 20 years.

The actual lead singer with the dark voice (Siobhan Fahey formerly of Bananarama) didn't quit the group but fired her colleague with the high voice (Marcella Detroit) PUBLICLY at the Ivor Novello Awards in 1993. But before that the band released another four singles: "I Don't Care", "Goodbye Cruel World" (re-issue), "Hello (Turn Your Radio On" and "My 16th Apology". From 1993 Shakespears Sister was a commercially unsuccessful solo project, until Siobhan reunited with Marcella in 2018 to make some new music. But now it seems to be all over again.
