Pilots should have a special training when they play as a firstofficer along side with very senior Captains. Most of the cases they get scared of speaking against captain even when they are breaking the SOP.
In Southeast Asia (India, Pak, Bangladesh, SL, Nepal etc.), the managers think that they are owners of their employees. They will go to any length to spoil junior's career if he/she speaks speaks against. So, even after training, they will be scared.
There is training involved and we do study a subject called "human factors" to raise awareness and prevent such issues. But the sad reality is as long as "seniority" and "ranks" exist such incidents can never stop.

CRM was supposed to address the authority gradient in the flight deck with first Officers mandated to speak up if the aircraft is operated outside the parameters of the Flight Operations Manual. In this case the first officer should have assertively called every violation eg. sink and bank angle etc and if immediate corrective actions are not taken by the captain she should have taken control. This can be very stressful but it is the correct and lawful thing to do. In my 27,000 hours back when I was an FO I had to take over twice on captains who were about to put us in severe jeopardy. In both cases I was berated by the captain, so when we had arrived at the destination gate I requested the engineers pull the CVR and send the contents to the flight department. I submitted flight reports explaining what had occurred . The wash up was both captains were stood down, with one later being fired and I received a letter of commendation for one of the incidents .

As a Bangladeshi, I can say that this tragedy is still a red mark on our national flag. On the day of that incident I was in my office doing my regular tasks. And when we came to know about this incident, it was really difficult for us to concentrate on our work. In Bangladesh aviation history, this was the only tragic incident that happened till date!!!! I remember this crash and couldn’t understand how that could ever happen I almost think the Captain was committing suicide . That is the only thing that could explain such shockingly bad airmanship. The plane crashes and that’s pretty much it. Would be good to know the output of any investigations that took place. What corrective actions/fines/changes in operating procedures etc came about as a result of this tragedy? Who was held accountable and how do we get renewed confidence that flying this airline is now safer because of those who went before us. Keep up the good work!

It was beyond a pilot error. That "captain" was a full-blown psychopath and a complete danger to the public, unfit for flying. That horrible guy used his Bangladeshi male privilege to keep the female first officer quiet and in "her place" so she could not properly redirect the plane and correct that crazy captain's actions. Shameful. There was a special place waiting for that captain after he crashed that passenger plane and killed himself and others.
A Lecturer of our University was on this flight with her husband. After the flight crashed, her husband managed to get her out, but he couldn't make it. Her husband was also an alumni of our university. It was a tragic incident for our university.

bro please do a video about Air India flight 182 that took of from Canada and was on its way to London,the plane a Boeing 747 was carrying 329 passengers and on its way to London something exploded inside the plane which caused the plane to break and get completely destroyed and all the remains of the plane fell into the ocean and no one survived.
