A lot of us do understand. There is a Facebook group of thousands physicians doing keto+IF together. Who doesn’t understand is your insurance company and the hospital corporation/public office. Try fighting with the VA about please don’t give my patients 12 ounces of “grape drink” along with low-fat pancakes and syrup. Even if I used my own $ to bring the patients food (unfair to expect that of me) that’s not allowed either. I’ve had arguments with federal govt dieticians who prepare the patient meals that I need to raise them to 50 gram carb meals and then give insulin. You can eat the best kind of food just restrict carbs including sugar to 50 grams and and try not to eat more than your usual and keep four hours between meals. Some veggies are a must lots of water. Just to give you a flavor this is a sample of my days eating. The breakfast is coffee made with two tablespoons of heavy cream with little water and one toasted piece of sour dough bread with butter. Lunch some berries peanuts walnuts and some milk. And then an early dinner with butter chicken with little rice and bowl of salad with olive oil and avocado milk smoothie. So what exactly do I eat? What is this magic? I mean, people ask. And I am down over 60 pounds and have kept it off.
1.- Whole Foods. Reduce your consumption of franken, hyper processed foods to a minimum. Why? Added sugars, multiple forms, sugar alcohols, and other crap. No, you do not need it.
2.- no seed oils. They have oxidative properties that are not good for you. This, outside restaurants, is almost non-negotiable. This reminds me, a lot of processed franken foods have seed oils because they are cheap.
3.- while I stopped eating bread over a decade ago, I am in that ten percent that is actually sensitive to it, I don’t eat rice, tortillas, quinoa…I can rarely do legumes. As in rarely.
4.- as a T2D no potatoes, yams or the sundry of root veggies, even if I like them. If you are not a T2D you might be able to enjoy on ocasión. However, your grandmother was right, it goes to the hips. So keep them to a rare treat.
5.- no pasta, I don’t care what form. It’s a shortcut with the chickpea pastas for example. If you are not a T2D, you can enjoy on ocasión, but carefully watch the portion size. Make your own tomato sauce, it’s stupidly easy, and yours will not have added sugar to it.
What I eat…fish, at least four times a week, chicken, lamb, occasional beef, other lean meats like rabbit…and also duck, which is not lean, but is delicious.
Full fat milk products, farm grass fed and all that.
Plenty of above ground veggies, including bock Choi mixed greens, sprouts, crucíferos veggies.
Nuts (read ingredient labels, no nuts in seed oils or matodrextin. Just nuts, thank your.
      Hard cheeses
      Eggs.
Yes, we do have bacon from time to time.
Olives, olive oil, balsamic vinegar, apple cider vinegar (there is research the ascetic acid can help with glucose control. It’s tantalizing, and will not hurt)
Fruits are limited to berries. In time I will experiment with other low GL fruit. I am not in any rush.
I try to keep carbs low.
Eat mindfully. Learn when you are hungry, and when you are not. No snacks, unless you are a T2D and need to treat a low blood sugar. This is a hard rule for me. You do not need to eat seven to nine times a day. Fun fact, I treat mine with cheerios…because this heart healthy cereal, sarcasm, is essentially sugar and I prefer the texture to the sugar gummies.
At this point I also do intermittent fasting every day. I rarely have actual breakfast, because I am rarely hungry. When I do, I tend to eat more as welll. There is actual research on this as well.
I exercise, but not because it will help to lose weight…it really does not. But it has a slew of other benefits and have added resistance training to further reduce the insulin resistance.
What is hysterical is that until the 1960s the diet component was pretty standard advise…then Ancel Keys happened. That could be a rant on its own. This epidemic is a forced error on our part.
Oh and I forgot DO NOT DRINK YOUR CALORIES

Also…it should be touched on…even briefly…what the difference is between refined grain (white bread) carbs and celery (green vegetable) carbs. One very simple explanation relates to water content and fiber content. I acknowledge Gary’s greatest gift to the discussion is his deep dives into the scientific history and cultural context of what has happened over the past century. “Good Calories Bad Calories” opens with this amazing story about Eisenhower’s heart attack in 1953. If you're metabolically healthy, calories are a good enough approximation to control weight, however it does not truly represent the real mechanisms going on inside that actually manage weight. Calories are a pure contingency because they are not themselves the reason why calories in vs out might end up working out for some people. They simply correlate loosely to the general changes in the diet that need to be done to trigger the underlying mechanics of weight management in metabolically healthy people, and the less metabolism is healthy the less using a calorie model has a chance to work.

I maintain a largely ketogenic diet because that works for me and I have to address years of eating too many carbs. But when we say that carbs are the enemy, carbs are bad, protein and fat are good that makes a great narrative but it isn't entirely accurate.

Gary, your point on people not testing their assumptions; I had the same experience in my industry of personal training. I personally am overweight despite the lengths I have gone to, and in regards to exercise, I did everything that the experts and the very large muscular guys said, and it didn't work. One day, full of frustration, I wrote myself an exercise program, then I asked myself, what is the goal of some point, and then I asked asked myself, why do I think doing this thing (always taught as how to achieve that goal) will give me that outcome? So then I would do an experiment to see if that action would lead to the actual result. I started doing experiments in other words. My findings were jeered at on public forums but several times, 5 yrs later, some well known person in the industry would come out with what I said and everyone would talk about how good this person was. I started to see the same phenomena in everyone around me. So I found it very interesting but no unexpected to hear you talking about it here.

Telling us that we do not get fat by what we eat but because of mainly high insulin , then end up telling us to restrict calories intake to lose weight ?,
Tying up overweight with too much insulin is a good strategy to encourage overweight to put blame on insulin and to then take the necessary steps to reduce Insulin production by eating less overall calories mainly from refined processed carbs , sugars , seed oils and fats.
Good strategy to put the blame elsewhere.
The body usually don't produce insulin to make us eat , it produces insulin after we have eaten carbs and sugars. It is this excess carbs and sugars that the body then stores as fat.
So it directly points to calories in = calories stored as fat.

When I've had oatmeal for breakfast, for example, I was hungry within two hours. It just doesn't stick with me. It's hard for me to imagine how having a diet high in carbs would work in my case. From what I understand, eating a lot of starch isn't that great for health. Grass-fed and pasture-raised meats are far more nutritious and satisfying. But whatever works for you. Since I learned about regenerative agriculture, I gave up on the idea of a vegan or even a vegetarian diet. I don't think they help the planet much since animals are absolutely necessary to maintaining healthy soil and giving us the nutritious foods we all need.
