When do the ultra, holding down X and the ultra continues normally, when you get to the last movement that is pressing X, and even got down the X button what you'll be pressing Y while you release the button X. This is how it works :)

On the hardest difficulty, they break any medium length combo, unless they are fainted. As for the perfect all game it is probably with a invincibility cheat, or save state.
