- The National Transportation Safety Board (NTSB) in the US has said that a mistake by an air traffic controller caused a near-collision in Texas last year.
An investigation was launched after a FedEx plane and a Southwest Airlines jet came within about 170 feet (52 m) of each other, before the FedEx Boeing 767 was forced to fly over the Southwest 737-700 to avoid a crash in poor visibility conditions.
Investigations determined that an air traffic controller had cleared both planes to use the same runway, having expected the Southwest plane - which had 123 passengers onboard - would have already departed before the FedEx plane landed, given his "expectation bias" that Southwest planes were quick to depart.
The incident - one of at least half a dozen near-miss incidents in the US last year - raised concerns about aviation safety and the strain on understaffed air traffic control.

Why is it we have to get this sort of news from the BBC and SkyNews Australia? What happened to “news” reporting here in the US?
News reporting in the USA is state controlled so as not to disturb the mindless masses listening to stations like CNN and other garbage-producing news outlets.

23 near misses out of a few million flights is not a concern. Cars are far more deadly.
The more aircraft in the air, the more likely there will be a collision. A.I. needs to be developed soon to take back control of our skies.

As a Boeing 737 pilot I don’t believe the controller deserved the blame in this case. The NG requires a 30 second run up on the runway at 70% N1 in icing conditions. You always tell the controller when you need to do this so they can provide the spacing. The Southwest crew was told that traffic was on a three mile final and they still chose to line up on the runway and perform a 30 second run up without telling the tower controller at an airport that doesn’t have ground surveillance radar. You have to have better situational awareness than that.
That’s tuff when you have 5 arrivals coming in all at 3-5 mile spacing. You may be waiting a while.
This keeps happening more and more in the US, i'm honestly worried that soon there will be consequences
I wonder if the mass retirement of senior aviation staff during COVID has contributed to the uptick of these near misses.

They are almost exclusively in airports in the southern states. Those run by Christian Nationalist, Trump loving, Right wing republicans. Seeing my friend and her colleagues in action, I wouldn't wish this job on anyone. The level of concentration and speed of communication at times felt like they're speaking a different language.
There needs to be fewer flights. It is as simple as that. We, as customers cannot expect to have such frequenty flights everyday. Also once a plane lands it should wait 1 hour before the next destination and after all safety checks are completed.

I was watching a breakdown of this, and one of the factors that fed into ATC giving the takeoff and landing clearances in this close proximity, and the FEDEX crew accepting it, is they were Expecting the southwest plane to do a more immediate takeoff that they are apparently known for.
The FedEx crew were the only ones that weren’t clueless.
Well the Southwest crew couldn't have possibly seen the fedex. Was a foggy day after all

Media needs to relearn how to report on investigations that can't give results before they've completed the investigation.
And experts interviewees need to relearn how to say, "I'm sorry, that's a good question, but that's exactly what this investigation seeks to answer, and i' prefer not to speculate."
It's like that non story of the Titan sub where anyone in the know guessed the passengers were dead, but a search & rescue mission isn't going to announce circumstantial evidence (like the navy picking up a sound that's probably an explosion) until they've found physical proof, or else the window of survivability has been exceeded to the point there's no chance - and then because the search was still underway, Tv media invented this fake frenzy of dwindling air supply.

Runway incursion and misinterpretation or misleading ATC clearance could cascade into a catastrophic crash in US airportsm. In retrospect, it boils down the the shortage of ATC controllers nationwide regardless of the decent salaries.

I was on an American Airlines flight from Madrid to Philadelphia on May 12, 2024. The weather was clear and little wind. As we were landing, at an altitude of 300 feet (91 meters), we abruptly pulled up, dramatically climbed to 3000 feet, and did a go-around maneuver. When we landed, the pilot apologized for the delay and stated it was due to "traffic on the runway," which I assume means another plane. The whole incident was absolutely terrifying.

The US allows landing clearance to be given in advance while the runway is still occupied - that’s the dangerous issue. Nobody should be cleared to land until the runway is clear!
This is happening pretty much weekly at the moment, the system is broken and you'll get a real bad accident if it doesn't improve

southwest man......ive flown southwest since they were a texas only plane. not a single accident in their history. (well they had one plane overshoot a runway but minor injuries). please dont now become just like UsAirways. please please please. i love this airline so much. i willl actually cry if i see this plane come down....😢

Take off and landing are the most dangerous moments of any flight. There’s a shortage of ATC, pilots, and airline workers in general. These people are overworked and underpaid. Be kind to them when you’re traveling

The root cause is that professionalism sucks these days. Work and human life is not something many people take seriously - or their own mental health is so shot they can’t show up like they used to.
Infinite money is a prerequisite for infinite safety. A simple traffic signal in the US can cost over half a million. At an airport we are talking about millions. Many Americans are already very concerned about the federal debt. And even then, technology isn't perfect. I'm not sure what specifically she is alluding to, but runway status lights are one thing. But they are a MAJOR project. This means runway and taxiway closures with construction and delays.
The pilots share responsibility too. When they were cleared for takeoff, the controller stated, as required, that there was traffic at three mile final. The SWA 737 also has TCAS, which depicts nearby traffic and relative altitude on the navigation display. Assuming it wasn't inoperative (rare), they had a visual representation as well. Between the takeoff clearance and when they actually started their takeoff roll, was a solid minute. That is an absurd amount of time. Especially with how SWA pilots typically fly. At three mile final, that fedex was at roughly 1000'. Landing is in around a minute. The SWA pilots did not use common sense and experience. It's possible that they were waiting to hear that the cabin was ready for takeoff. They should have said they would wait if they weren't ready.
All that said: an aircraft on the runway has right of way. Southwest took their time, but it was the controller's job to verify the status of both aircraft. They didn't.
Lack of experience overly rely on technology. Not taking the ownership of the job.
More safety devices due to inadequate training of pilots and controllers. Disasters are only a matter of time.
When you cram more aircraft into confined spaces, things like this are more likely to happen. Add in the shortage of air traffic controllers which causes understaffed control tower personnel to be overworked and you have a recipe for a potential disaster. Just imagine the chaos that gods on daily at ORD, LGA, JFK, LAX,, SFO, ATL DFW...
Since this is BBC news. Does this technology exist at all UK airports? Does it exist at any UK aitports?

There are many technological aids to managing airplane traffic. Those, combined with human intelligence, work well. It is better to fix an actual problem, when it is determined what happened, than to imagine that a new system will solve all our problems. It won't.
Artificial Intellence, once properly configured, will resolve all air traffic control issues. Humans are the problem here, and from everything I have read about air traffic controllers, they are overworked and suffer from extreme fatigue.

The reporter asked a loaded question, the number of close calls is actually decreasing but the public is becoming increasingly aware of these incidents and the media is taking advantage of this interest by reporting on what the public demands. Of course we should strive to reduce the number of close calls to zero but we can only do that if we're aware of the situation and I'm disappointed that the BBC failed to properly educate its viewers. This is why legacy media is dying, why would I watch TV when I can watch enormously higher quality content on UA-cam from an actual expert that cares about educating their viewers?
Or maybe you dont know what you're talking about since 2023 had the highest rate of near collisions in 5 years.

Of course there would need to be some human oversight, and most importantly, regular A.I. maintenance check-ups. A.I. can do a better job and it is inevitable that it will eventually do just that.

Because it can fail and it's not fool proof. Not to mention The US has chronically ancient air traffic infrastructure - you should look at the funding reductions on infrastructure passed by local and state officials.
