How to interpret your SIBO breath test!

- Unless you're with a SIBO expert, you could be interpreting your breath test incorrectly. This is what happened to me and I got down to 86 pounds so I made this video so that this doesn't happen to you!

Jake thank you so much for this video! I did the SIBO breath test last month and it came back very positive via my gastric motility specialist (t lactulose breath testing in October 2021 which returned showing an early, excessive rise in detectable hydrogen gas well above 20 ppm within 90 minutes.)
Finally getting some treatment and relief!

Excellent video thank you, really useful. For the first graph you displayed, did you mean to highlight the S6 point on the graph? Because you were talking about the 90min mark which was S5 (or I may have misunderstood)
The problem of doctor competence is a serious issue. I have saved lives because I made it my job to protect loved ones from doctor carelessness and incompetence. For example, I make it my job to stay with loved ones when they need to go in for a procedure.
Yes! Good on you for doing that and I’m sure your loved ones are very grateful to you for that. There are so many factors at play. Things that are their fault and many things that are not. For me personally, hard to know how fair I’m being sometimes but hey that ain’t my biggest worry at this moment lol 😂
Hi Jake, I'm really frustrated and need some guidance with results of my recent Lab Breath Test and I'm hoping that you may be able to help. I'm in Australia and I don't think the breath test is very widely used here and certainly mainstream doctors don't seem very conversant with it.....however I went ahead as I was pretty sure the symptoms I have are consistent with SIBO, I've been doing quite a lot of research on the subject in the last few months.
however when I saw the numbers on the Lab report and the comment of "Normal result NOT consistent with small intestinal bacterial overgrowth" I was somewhat underwhelmed....one question I have that I can't seem to find an answer for in a lot of the interpretations I've found is .... do you need to have a Methane reading over the 3ppm as well as Hydrogen of 20ppm or can you have one or the other on a Zero reading.....in my own case the Base Line reading was 0 for everything, but after having 20g of lactulose my Hydrogen reading at 20mins was 1, but at 40mins it had shot up to 34ppm and just continued to rise in more than 20ppm increments but Methane numbers remained at 0.
I queried the Lab but they stand by their result and would not tell me their testing parameters or cut off numbers. Hoping that you could perhaps shed some light on this.

Hydrogen ~ 84 ppm [90 minutes]
Methane ~ 11 ppm [90 minutes]
Is that hydrogen dangerously high -- what should I do?
Thank you so much for this informative video. I did my first trio smart back in June of last year. Was positive for Methane. My GI put me on medication. Didn’t see much results. Got retested after 6 months and methane levels were still elevated ( even worse than my first test). He now has referred me to dr Pimentel. But I’ve been wanting to see dr Rahbar so bad. I know he doesn’t accept any insurance. How was your experience with him. How long was the wait until you were able to schedule your first appointment with him? I’d really appreciate any tips before I call his office to schedule my first appt.
Dr. Rahbar is a fantastic doctor but you're right he does not take insurance. The Cedars-Sinai team really know their stuff about SIBO/IMO but they really stick to that lane. I don't remember exactly but it didn't take too long to see him. There are other docs and naturopaths that know how to treat SIBO properly that are less expensive than Rahbar but you gotta do your homework and make sure they are truly SIBO experts

This can be the result of an unhealthy gut from antibiotics or other drugs which greatly disrupt the biome of the intestines. The mainstream way of treating this is, you guessed it... antibiotics. One way to prevent overgrowth is taking probiotics and eating foods that are healthy during and after antibiotic treatment. Stay well.
Great video. ❤💯👍 Thanks for spreading the knowledge. It's sad to know that there are so many people who believe their doctors that what they have is an IBS and low FODMAP diet and antidepressants are the only treatments available 😥 And to people who are looking for an answer to the last question: If you don't have TrioSmart test and regular 3h lactulose test is not showing H2S (no flat line) but you do have H2S symptoms then can order a stool test like (16s rRNA from Ombre labs, Biomesight etc) and look for sulfur reducing bacteria (main culprits Desulfovibrio and Bilophila wadsworthia). If you have it and numbers are high then you probably got it. I think A.Siebecker also used to use one of OATs (Organic Acids Test) markers for H2S indication but I can't remember atm what it was.
I know!!! So many GI doctors don't even "believe" in SIBO. Takes way too long for research to reach to bedside. Thank you for your comment. I am pretty sure that Dr. Siebecker and Dr. Pimentel do not rely on stool testing for H2S SIBO since his research shows that there is absolutely no correlation between the bacterial microbiome of the small intestine and the bacterial microbiome of the stool. His team did a great paper on this and that's really going to shift how we think about the GI microbiome. Most studies on the GI microbiome just look at stool and that's not indicative of the GI microbiome
@@BartonellaBabe Thank you for your comment. I know that large intestine and small intestine microbiomes are very different and no doctor should diagnose H2S or any other SIBO from a stool test. However, if nothing is helping and someone has many H2S associated symptoms but can't access Triosmart and regular test or, the regular test is showing H2 or H2 and CH4 looking at stool can indicate if there are any hidden H2S bad boys causing havoc at least in large intestine 🙂
@@jurrga7487 oh yes I see what you’re saying! What would the treatment of hydrogen sulfide in the large intestine look like? I only have done reading on small intestine so I don’t really know about this topic
@@BartonellaBabe Well there isn't much info on H2S in small or large intestine as you know. For the most part the same things that work for SI overgrowth would work for LI as well (medications, herbals, prebiotics and adjustments to diet). But it's individual and may also depend on things of what species of bacteria causes it. Let's hope M.Pimentel finally releases new info on H2S next year 🤞
It is not necessarily true that the same treatment that would work in the large intestine would work in the small intestine. As you well know Xifaxan is the main antibiotic used in the small intestine but studies have shown it to be mainly active in the smal intestine and minimally active in the large intestine which is why it is used as the small intestine can be targeted without disrupting the biome of the large intestine. However, that is the only antibiotic I know of that is shown to work that way so otherwise yes any treatment for H2S that would work in the small intestine other than Xifaxan should have a similar effect in the large intestine.
i just got my results for hydro, metha and fruc, and none of them are above 6 argh. still yet to do hydro sulp though- i asked about sifo but the naturopath didnt really take it into consideration frustratingly!
Well a methane of 6 with symptoms (especially constipation) can be an indicator to treat! And that is very frustrating. SIFO is definitely a bigger deal than some of the sibo experts think. There is a patient in our group who has improved so much (sleep, behavior, psych stuff) just with SIFO treatment. Still has a long way to go but it’s been pretty amazing

omg it's a real song. I'm feeling so hopeless right now. Treatments worked but then I backslid. So I came here to listen to you be positive and informative. Giving up is not an option.
Now I had negative 16s for yeasts, and also negative culture. Many in the naturopathic circles would say it's hiding out in biofilms. What's your opinion on that? Did you have positive tests for yeast?
Haha yup it’s a real song! Backsliding is very depressing and I know it well. I’m assuming this was a stool test for yeast? Stool can’t tell you what’s going on in the small intestine. They are two separate worlds. That’s why SIFO is a clinical diagnosis. There is no lab test for it currently. I am getting better taking an anti fungal and I have had die off the entire time titrating up and I didn’t have yeast in my stool

I know this is off topic but I did some round about testing for CBS and MTHFR gene mutations. They both say Hetero AG. What does this mean? Am I positive for the genes?
I honestly don't put a lot of stock into individual mutations. There are so many genes and so many mutations and so many enzymes that do more than one thing. Then there are things like SIBO, bartonellosis, etc. So it all has to be addressed and I think many practitioners can get too in the weeds with all the combos of SNPs. That being said, metabolic pathways are super important and can work function efficiently or inefficiently, dependent on more than just genes. If you don't have the cofactors for an enzyme, it isn't going to work optimally
