  • Persistent Postural Perceptual Dizziness, known as 3PD, typically occurs after a disruption to one of your sensory systems, usually with a vestibular disorder, but can even be from a significant stressful life event.
    For example, if you have an episode of vestibular neuritis, you may recover fine, but if your symptoms change in quality and become more constant, you may be developing 3PD. What are some signs that it's 3PD and not something else?
    symptoms worse with standing, better with sitting or laying down
    symptoms worse with busy visual environments like a crowded areas, grocery stores, or complex patterns like gingham
    symptoms worse with motion
    symptoms last hours, or can be constant, but may be better or worse at any given time

    This is me. I was diagnosed with Bilateral vestibular loss & the 3pd came after. I have constant rocking, swaying & balance problems. I feel like my head is on a spring, wobbling about in every direction . I do get some relief when lying down. I really struggle With any kind of stimuli..I’ve also noticed it does it with sound too. To much listening to people talk also makes me feel very unwell. I do have a sound sensitivity.. I get a lot of motion sickness!!. The slightest tilt of my head/ body makes me feel sick!!! Thank you for making this video clear for people to understand.. it’s not easy to contend with and I sympathise with anyone who is struggling with this disease. It’s very debilitating!!!!!

    I was wondering if you could explain what is ment by rocking/swaying? Is it the feeling of your head feeling that it's swaying? Or that your actual body is swaying?
    I developed some symptoms similar to this but I have not been checked or have not done any scans to ensure it's 100% PPPD. My doctor seemed to think it was sinusitis since I have pressure around the areas where you usually develop it. She says this can cause unbalance and dizziness, which I have currently.
    No problems walking though and I don't experience problems visual problems either like pattern issues or heavy crowded environment.
    Hell I even play Valorant with no issues.
    Should I consider this being PPPD? Or is my doctor right and should I lean more into it being sinusitis? Getting checked for that in 2 weeks.

      hmmm, hard to say if you have it or not, but I think most people feel like their body is rocking/swaying, sometimes it's a hypersensitivity to the movement, as we all have very slight shifting of our weight to keep our balance, just most of us ignore it, but those who are hypersensitive to it may feel like the movement is exaggerated. Sometimes vestibular migraine can cause sinus pressure as well, or you could have a combo of issues, sinusitis with vestibular migraine and 3PD. I would ask your ENT what he thinks about the possibility of other issues contributing alongside the sinusitis.

    I’ve been having symptoms if 3PD since Jan 1. I’m glad the room doesn’t spin. I am lightheaded, sensitive to lights, complex visual input, too much activity go on in small places, it’s hard to climb stairs. Picking things up off of the ground is destabilizing. I have changed my diet to lower possible inflammatory causes. Gluten free, lactose free and low sugar. I’m going yoga and exercises from vestibular therapy.

    Hi Dr, you’re the first person that mentioned busy patterns, for example Sees candy checkers black & white floors throws me off, also bright light, bright colors

      Have you had any symptoms of rocking/swaying with standing?
      Common are sensitivities to busy environments like the checker patterns, casino floors, lighting or light contrast.
      Also common is a visual motion sensitivity with experiencing dizziness watching moving objects, either with busy TV shows/cars going by, driving past trees, looking at all the items on grocery store shelves, or being in a crowded area.

    I developed vestibular Neuritis about 6-7 months ago. Severe spinning for a week. I have had a persistent spaced out feeling since. I developed severe anxiety for about 2 -3 months. I got on lexapro for 2.5 months and my symptoms virtually went away. I came off the SSRI and the spaced out feeling came back as well. I longer have anxiety thankfully. Would this be considered 3pd in your opinion? Thanks

      Very possible it could be 3PD! It’s common to have a rocking sensation that is worse with standing or walking and better when sitting, have more symptoms with busy visual environments, and anxiety. You may also want to see if your neck is contributing though (cervicogenic dizziness). Let me know if you have any other questions!

    I had BPPV and shortly after having treatment I have started experience fussy feeling between my eyes and lightheaded. Grocery stores cause me to become off balanced and go into a panic mode. Watching TV when screens change a lot causes light headed and my eyes to do flicking motion. Is this common after BPPV?

      not as common after BPPV as it is after vestibular neuritis or with migrianes but any kind of disruption to the vestibular system can trigger 3PD and difficulty for the brain to process all of the sensory information going to it. With the right exercises it will get better! Keep working on it!

    I believe I have this. Over a year ago I went to an ENT because of tinnitus. I was diagnosed with Menieres. Within this past year I have had 8 steroid shots in my right eardrum - an operation where the doctor look in my inner ear and put meds in there. Then October 2023 I had surgery where the doctor put a shunt in my inner ear. In the discharge papers it said that dizziness was to be expected. So all this time I thought I was recovering from surgery. At work looking at 3 monitors and down at my keyboard really made me dizzy. I couldn’t go outside or to any stores without my head being so overwhelmed. I am now on short term disability and waiting for an appointment with a balance rehab center to try to get my quality of life back. This has been so hard.

      Ah I’m so sorry! That sounds awful. When you go back to therapy have them see if your neck is contributing to any of your symptoms as well!

    Ive had 3pd now for over a year, its been pretty awful to be honest. Currently doing vestibular exercises + on low dose of sertraline to help with dizziness etc. I work as a self employed gardener and this had made working really difficult. Thanks for the video. 😉

      @VertigoRecoveryDr Currently waiting to see Neurology. I did see a chiropractor last year and went for many visits. He thought possible issues with c2/c3. When he adjusted my neck there was always instant relief from dizziness but it usually came back within hours.

    Thank you for your video! This exactly describes what I've been experiencing since the beginning of 2023. It took me a long time to figure out how to describe this to my caregivers (mostly, that it is NOT dizziness or vertigo as typically described). Mine is definitely worse the longer I am standing and walking. Long hallways seem to be especially challenging, causing me to feel like my legs are working harder the further I go. So weird! That aside, it's a huge help to simply know what to call this bouncy-house floor feeling. I believe mine began after an episode of Covid.

      haha I love the description of "bouncy-house floor feeling"! I may use that... Covid can definitely affect how the body and brain process sensory information. Have you tried vestibular physical therapy? Are your symptoms improving? Any other covid symptoms still lingering? I hope you're feeling better!

    How does nicotine/alcohol affect this condition

      Great question! Nicotine I’m not too sure about, I’ll have to look that up! Alcohol can affect how the vestibular system functions, changing the density of the endolymph (fluid in the inner ear), but how or if it affects 3PD, I’m not sure. Alcohol can do a lot of things, one can be disrupting sleep which can prevent you from recovering from chronic conditions like 3PD. When trying to recover from it, you need every advantage you can get.
