  • Persistent Postural-Perceptual Dizziness (PPPD or 3PD) is a chronic condition characterized by non-spinning dizziness and unsteadiness provoked by environmental or social stimulation. These exercises decrease sensitivity to stimulation and improve tolerance for daily activity and motion. I include recommendations for making the exercises more comfortable and manageable.

    SOLVED …. So I had all the symptoms of PPPD. I’m male, 50 years old. Spend long hours in from of computer screens. Symptoms started about 2 years ago. Feeling of confusion, brain fog, unable to concentrate, poor memory. Then mild dizziness when walking especially corridors and the supermarket. Symptoms got worse, depression and very emotional. Jaw pain at my TMJ and incredible neck pain. Had MRI, CT, ear pressure test, eye test, changed glasses, dental checks etc. All cost me a small fortune. Tried stretches, yoga, meditation etc. nothing worked. THEN… by chance I had reason to close my ears with my fingers… symptoms reduced. So I wore earplugs and …. Symptoms reduced. Kept them on 24/7 for a week to get through the day. I still didn’t know why symptoms reduced. Took the ear plugs out it was like letting the devil back in. With the earplugs in whilst driving and pressed down on the plugs and symptoms reduced even more. Was convinced it was ears. So I watched a video on doing ear massage and stretching. When I pulled and stretched my ears, away from my nose line, and held them there for a minute, it was quite sore, the symptoms had reduced massively!! The pain in jaw / cheek had gone but my inner ear was very sore as I kept doing this all day. The next day all symptoms had gone by 90 percent but has very sore inner ears. I am now on day 3 and no dizziness etc but still quite sore inner ears and haven’t stretched them since. Please try this and reply if this worked for you. Go easy on the ears tho. Very gently pull and stretch them away from the side of your head. Please do reply if this works or not.

    Your exercise, walking and looking at object on floor and then out a window in the distance reminds me of my first bad dizziness episode. I was walking in the woods with no established trail. Naturally I had to look up and down frequently to avoid obstacles. After 20 minutes of doing this I became very lightheaded. This kind of situation still results in symptoms.

    • @MovementFunction
      @MovementFunction  2 роки тому +10

      Bruce, your experience is quite common. These exercises intentionally stimulate the vestibular system, which brings on symptoms. The idea is to retrain the brain in order to interpret motion better without causing dizziness and to get used to the movement so that it no longer feels strange. The goal is to do enough to achieve these results without overdoing it. Just a few reps and a few minutes at a time, then rest and do it again the next day. All the best!

    Dear Beth
    I got my first vertigo back in May in my sleep. I had no other symptoms for about 2 weeks and then another vertigo in my sleep. Again no other symptoms other than mild balance issues for about a few days. I visited a VT and she suggested that I have BPPV. First test showed a horizantal nystagmus and she did BBQ roll. 3 days after the BBQ I had another vertigo in my sleep. I went back to her the next day but this time the test triggrered a severe vertigo with cercular nystagmus and she did epley. I was unable to walk right after the epley cause I felt so sick that day. My symptoms were improved in about 8 days at almost 90%. 4 weeks later while sitting at the beach on a hot humid day I had dizziness. It was not vertigo but severe rocking right left sensation. I hardly was able to walk to the ferry station. Since then I am having balance issues and dizzy when lying down in bed. I use 45 degree pillow. This morning I was dizzy when I woke up.
    I told my VT that I might have VM rather than BPPV and she suggested very possible since I suffer from migraines for the last 7 years. My dizziness while walking (sensation of rocking right left, imbalance and fear of going to fall) is PPPV developed after VM episodes? If yes, would that be safe to bike?
    Thanks

    • @MovementFunction
      @MovementFunction  Рік тому

      Tan Bass, based on the severe rocking, imbalance and fear of falling, I would not recommend outdoor biking at this time. If you miss riding your bike and want the cardio conditioning, you might consider putting your bike on a trainer and setting it up on your patio, in the driveway, garage, etc.. Basically, somewhere on a flat surface outside where you can feel fresh air moving, maybe in front of trees or other greenery. That would be much safer and still helpful for the physical and mental benefits of riding your bike.

    Thanks for the video! I have PPPD for the last 2 years and with VRT I went from being ridden to be functional even though I have daily symptoms and still can’t do many things.. is it normal that now all the things that are difficult for me trigger fatigue more than dizziness? For example if I go to the grocery store or take a walk or if for 5 minutes I do some squats or lift some weights I get very very tired that I have to lie down and often take a nap. Since the beginning of this condition these triggers give me immense fatigue around my eyes and in my head

I’ve done months of acupuncture, i did vestibular therapy for a few months which didnt do anything. Nothing helps and it makes me job (im a construction worker) incredibly difficult and nobody has any clue that 80% of the day i feel like i just woke up on anestesia from surgery bc all the drugs im on and im STILL dizzy constantly, feels like im walking on a boat or jumping up and down constantly.
    This condition is a silent killer bc it makes me suicidal, im never been more miserable. I just want it to stop! I cant take it any longer and NOTHING works 😩
    I’m going to try this but i went to a clinic for months that specialized in “vestibular therapy” and all it did was make my symptoms worse but never got better… and i work 12 hours a day at 400+ feet elevation walking on pipes on construction sites.. its so awful, and i already lost 6months of pay last year bc i couldnt work.. i could barley walk let alone perform strenuous job duties on a construction site for 10-12 hours a day

      Jason, I wish you the very best in your healing and health. I hope these exercises are helpful for you to decrease your dizziness. Be sure to monitor your symptoms while doing these exercises. Doing too much can certainly make symptoms worse, so try the 'remote control' suggestion I included in the video to be sure you're not pushing too hard. It's normal to feel a little more dizzy while doing the exercises because they intentionally stimulate the vestibular system, but it's important to not push too hard and to rest and recover following the exercises. • @MovementFunction
      @MovementFunction  2 роки тому +1

      Rohit, this is an excellent question. The answer is highly individual, which is partly why you're seeing mixed views. With the combination of vestibular issues you are dealing with, it's very difficult to anticipate how you might respond to provocative movements, especially extended sea travel over a number of days/week(s). What I say is that truly anything IS possible. We have a great deal more power to change things in our bodies than we realize. Since you are passionate about cruising, don't give up on it! If possible, I suggest testing out your response to being on a boat little by little. If you live in an area with boat access, go out for an hour... then a few hours... then a full day... etc., and build up until you're ready for a cruise. If not, you might try buying a tilt board, BOSU ball or something like that to test your response to ground movement AND to build up tolerance. I love the BOSU ball. It's expensive and there are other brands, so shop around. I'm not sure what is available in your area.
      Here's the Bosu ball on Amazon: www.amazon.com/Bosu-Balance-Trainer-65cm-Blue/dp/B00AQ4F19K/ref=sr_1_4?crid=2U888IC1HY8Z2&keywords=bosu+ball&qid=1643068290&s=sports-and-fitness&sprefix=Bosu+ball%2Csports-and-fitness%2C172&sr=1-4
      The balance tilt board is also a great tool and more affordable. Here's the balance tilt board on Amazon:
      www.amazon.com/PACEARTH-Professional-Challenge-Stability-Accessory/dp/B08DHWJ5TL/ref=sr_1_8?crid=1NDA8L94I0QN2&keywords=balance+tilt+board&qid=1643068414&sprefix=balance+tilt+%2Caps%2C183&sr=8-8.

      @MovementFunction  Рік тому

      Hi Laura. I suggest taking a break from the vrt exercises for about a week and let the BPPV settle. When you return to the exercises, start at a less intense level than where you left off, for example, do fewer exercises and fewer reps at a slower speed, less movement, etc., and build back up gradually. Hang in there, you'll be back to the vrt soon. :) As for your question re: PPPD and BPPV, sometimes it's difficult to know if one vestibular condition is causing another or if they are concurrent conditions. Especially with PPPD and BPPV. I'm sorry I can't answer your question more definitively. I wish you all the best!

    Hi Beth,
    Thank you for being such a gracious and thorough teacher and for explaining so well and so patiently each exercise. 🙏😘
    I live in Paris and was finally diagnosed with PPPD last June, after many months of various medical exams and total disarray, as nobody was able to find the reason of all these symptoms I had been suffering from for many months, until I went to see a physiotherapist specialized in vestibular rehabilitation!
    Soon after I started my rehab online on his website, I discovered your videos and started to use them as well, as I felt more comfortable with you!
    One question: the symptoms disappeared almost completely after a 3 week rehab, but COVID obliged me to stop completely for 2 months, and now the symptoms are coming back, so I'm very slowly resuming my rehab. Does it mean that I can never interrupt rehab even when symptoms disappear ? Do I have to practice forever?!

    • @MovementFunction
      @MovementFunction  Рік тому

      Lubna, this is an excellent question. The only accurate answer I can give you is that time will tell... In general, some people are able to fully recover without continuing the exercises while others need to do maintenance exercises. In your case, you had an illness that likely involved sinus congestion and inflammation, which is a common irritant to the inner ear, so it is understandable that you'd need a rehab period (hopefully short!) for vestibular function following an upper respiratory virus. All the best!

I have visited many many doctors in different countries but no one seems to find the cause. Unfortunately this has now caused me anxiety too which prevents me doing many things.
    I have been doing your exercises for 2 months which seem to help but I am still not fully dizziness free. I was born with hearing loss on one side and is now getting worse so I am nearly deaf so I wear a hearing aid. Some doctors says this is the cause of my dizziness which I doubt. I am a barber which requires me to focus a lot and look down therefore it might be because of my neck? As I had a MRI scan which showed I have sight slipper disc there.
    I am just wondering if I keep carry on doing your exercises could it make me completely dizziness free or not cause of my hearing loss?
    I appreciate your help and your reply.

      Kaka, For many people with chronic dizziness, it can take several months to achieve maximum results and best outcomes. Since the exercises are helping you, keep doing them. I can't say if your symptoms will completely resolve and whether hearing loss is a factor in your situation. Regarding your neck, I suggest checking out the cervicogenic dizziness video I posted and my other videos addressing cervical spine dysfunction. All the best to you!

      muvish, I think all of the exercises in this video would be helpful for you. Sometimes moving in the same direction that causes your dizziness is helpful for getting used to the movement and sometimes it aggravates the dizziness... Sometimes moving in other directions helps your body get used to movement overall and decreases the sensitivity to looking down... Every body and every person with 3PD is different. Since you felt fine after doing the exercises alone and felt dizzy after adding the run, it is likely the run was too much or the combination of the run and the exercises was too much. Try doing the exercises and running at different times of the day and see how your body responds. If you still feel very dizzy, decrease the distance of your run and see if you can tolerate a shorter run.

      Zuzanna, thank you for your comment and for subscribing to my channel! I'm so glad that the exercises in the vestibular series have been helpful for you and you've noticed improvement. With the walking exercises, have you been doing the other variations, including moving your head up/down and in a diagonal direction as well as side to side? You might also try varying the distance of the object you're focused on, so look at something close for a few head movements, then far, and then medium distance, etc. Also, this one is new: Try tilting your head side to side (ear to shoulder) while looking at an object at varying distances as you're walking. Another new very advanced exercise is to walk with your eyes closed and turn head side to side, up/down, diagonals and tilts (Be sure you do this in a safe space where you won't trip or run into anything). Start slow and walk just a few steps, build up gradually while maintaining balance. This is a challenging one - I have a hard time doing it. Finally, have you had your vision checked since May? All the best to you!

      @@MovementFunction Thank you for writing me back so quickly. Yeah I had my eyes checked vut from very begining doctors are saying its issue with my vestibular system. VNG showed pver 50% loss in my affected ear so I feel it's the root of everything. Now about exercises I already do this that when I walk and look at something at distance i try to not only move head side to side but i also try to tild it or move up and down but never tried with closed eyes so maybe i will try it from now on. I have a question about this other thing you recommend me though. This fixing my eyes on targets at different distance: close medium and far. While i do so i should be standing still and just move my head in this all directions you wrote before and just fix my eyes on point. Or i should walk toward this distant point and move my head but "stop" at some points for a short while and perform movements there too. I didnt catch that one. Yeah in all i feel like head movements are most triggering for me just feels weird moving my head, everything goes crazy then. i dont know what excerises may be especially beneficial for fighting with it :/

    • @MovementFunction
      @MovementFunction  2 роки тому

      Zuzanna, I'm suggesting that you continue walking and choose different targets at different distances without stopping. Do several head turns while focusing on one target then change targets and continue the walking head turns (or nods, diagonals, tilts, etc.). For example, you start walking down the street and look at the house closest to you, do 5 head turns or as many as you can before you pass the house. Continue walking and choose a house as far down the street as you can see. Do 10-20 head turns, then choose a target at medium distance and do more head turns. Continue walking with changing targets and doing head turns. Vary the number of head turns for additional variety. This is a high-demand coordination exercise as well as vestibular exercise.

      Rita, these exercises typically do not do much for Meniere's but they also will not make it worse, so it might be worth the effort to try them and see how you feel.

    When you have pppd why are the balance suddenly bad?

    • @MovementFunction
      @MovementFunction  Рік тому

      Amelia Runge, balance is affected by PPPD because the messages the vestibular system sends to the brain about where we are in space are affected. The information is not as fast or accurate and this leads to imbalance.

      Rushana, there could be many reasons for headaches and fatigue and I can't say for sure what the cause is for you specifically. For many people with PPPD, the constant dizziness is exhausting and stressful, which might cause or contribute to headaches and fatigue. I suggest further exploring these symptoms with your doctor(s).

      Alex, this program is more specific to PPPD while the vestibular rehab series includes a broader selection of exercises to address various vestibular diagnoses. There is some overlap. To complete the most comprehensive program, I suggest starting with this video and then adding in the exercises from the beginner vestibular video .

    • @MovementFunction
      @MovementFunction  2 роки тому +2

      Abi, yes it is normal to feel more dizzy after finishing the exercises but the increased dizziness shouldn't last longer than 20 minutes. And you shouldn't feel completely miserable. So, if the extra dizziness lasts more than 20 min or is severe, back off the exercises by doing few reps, fewer exercises or moving slower. You might also try taking breaks between the exercises and/or breaking up the exercises into shorter sessions to decrease the intensity.

      I find letting myself return to baseline after each exercise really helps my overall healing. Let’s say I’m going into the exercises and my dizziness is a three that day. For me, I will do the exercises until I reach a seven or eight and then let myself settle back down to whatever my baseline is that day before continuing.

    mam, is swiming good for pppd??

    • @MovementFunction
      @MovementFunction  3 місяці тому +1

      Swimming can be helpful if it is relaxing and comfortable. Be mindful of head movements though... especially if head movement provokes vertigo. It's likely best to start with breaststroke, where your head stays relatively stationary. Very gradually try other strokes, keeping movements slow and steady to start. Don't swim alone. Be sure that there is someone with you who knows about your condition and will be able to help you if you need it.
