    When the brain is still adapting post acute phase, I’ve been told that overstimulating the nerve can cause a flare up. So right now sound triggers me to have a dizzy episode. The visual vertigo was treated well with therapy. But if I hear competing loud background noises, within 10 minutes I start getting dizzy and depending how loud or how long I was exposed to sound, my dizziness can resolve in an hour or it can trigger dizziness and visual vertigo again for days. I’ve been told this is normal progress but do you think this sounds more like vestibular migraine? Can vestibular migraine start as a result of recovering from neuritis? I’m 14 weeks post acute attack, 10 weeks into vestibular rehab. I feel about 95% normal except for this sound sensitivity.

      Yes a vestibular neuritis can affect how the brain processes sensory information, including sound, but what you're describing seems like dizziness is being triggered by loud noises. Typically with migraines you will have a migrainous episode and will have light/sound sensitivity during the episode (not always, can have light/sound sensitivity outside of episodes). When someone describes that loud noises are triggering their episodes I usually think of perilymphatic fistual or semi circular canal dehiscence. When sounds trigger your dizzy episodes, what do you experience/feel, how would you describe the dizziness? Spinning, lightheadedness, head up in the clouds, etc...

      @@VertigoRecoveryDr At first the dizziness was triggered by visual stimulus and caused an imbalance, falling to the left, nausea, light headed and a spinning sensation. After VRT this has resolved. Then I noticed environments with white noise or loud background noise triggered a disorientation, a quick spin sensation but not repeatedly just like everything quickly spun to the left, and like my brain was scrambled. Trouble concentrating. Not light headed from sound. If I lay down it resolved within 60-90 mins. I then have intense left eye fatigue for a day or so. Vision is clear but my eye and temple area get very sore inside like a spot I can’t reach. Not a headache. Thank you so much for your response. 🙏🏻

      typically dizziness from a semicircular canal dehiscence will last just as long as the noise is happening for, as for a perilymphatic fistula it may be possible that there are prolonged symptoms, but also very possible that it's triggering a migraine. I would definitely recommend you find an ENT that specializes in vestibular disorders. Ask about VNG, oVemp, and cVemp testing, which may help to rule in or out some of these disorders.

    more nonsense and the disclaimer "it might just be age" somehow born n '50 we never knew anyone who had this. I agree I had tinnitus in late march early april from a sinus infection so severe the congestion displaced MY RIGHT EAR DRUM.which restored to natural position after aggressive intervention alto it lelt me wth systemic buzzing all over body I was 73 and never had such a thing happen with all the colds viruses and other infections. in late march to april '24 i had an episode of vertigo so severe I was carted away in ambulance to Langone for 3 DAYS. It was driven by inner ear issues from my left ear as they could see only my left eye was moving when triggers occurred. Absolutely no connection to any auto immune disorder i have none no association with accident or exposure to loud noise. Had three brain scans nothing which would explain issue. Of course i spent endless amounts of tIme researching from both years and have been taking lots of the supplements said if deficient might trigger both I am not diabetic and have a very healthy varying D3 level for oerr 20 years of 65 to 90 an a B12 at 870. Glucose perfect AiC a miracle SO WHAT GIVES ? Infuriated none of these jerks have any idea of what is dirving so many conditions affecting increasingly younger people every single year.

    Meclizine is a bandaid to help with symptoms. It doesn’t get rid of it. It leaves you extremely drowsy.

    Thank you for your video! This exactly describes what I've been experiencing since the beginning of 2023. It took me a long time to figure out how to describe this to my caregivers (mostly, that it is NOT dizziness or vertigo as typically described). Mine is definitely worse the longer I am standing and walking. Long hallways seem to be especially challenging, causing me to feel like my legs are working harder the further I go. So weird! That aside, it's a huge help to simply know what to call this bouncy-house floor feeling. I believe mine began after an episode of Covid.

Have you tried vestibular physical therapy? Are your symptoms improving? Any other covid symptoms still lingering? I hope you're feeling better!

    I had BPPV and shortly after having treatment I have started experience fussy feeling between my eyes and lightheaded. Grocery stores cause me to become off balanced and go into a panic mode. Watching TV when screens change a lot causes light headed and my eyes to do flicking motion. Is this common after BPPV?

      not as common after BPPV as it is after vestibular neuritis or with migrianes but any kind of disruption to the vestibular system can trigger 3PD and difficulty for the brain to process all of the sensory information going to it. With the right exercises it will get better! Keep working on it!

    This is very helpful, I've had a vestibular problem since getting the covid booster shot in February 2022. I have found that if I carry something a little heavy on my right side, like a laundry bag for example, I feel a little off suddenly. I realize it's originating from my left side. I don't do this exercise anymore but it seems like I might have to.. Does this condition ever fully go away?

      I have a lot of patients that are able to get to where their symptoms aren’t bothering them with day to day activities, but there are some people that continue to have persistent symptoms. The vestibular system has connections throughout the body (vestibular spinal reflex is one example), while x1 viewing may still be a good option, think about incorporating whole body activities like balancing on something while doing the exercise or yoga poses like bird-dog, etc. As a disclaimer, consult your physical therapist before doing any new exercises as I am not aware of any other conditions you may have.

    Thank you so much for your content. I was recently diagnosed with bppv and I'm finding two weeks in I'm still feeling dizzy when I turn my head/sensitivity to light. I'll be going to vestibular physiotherapy next week (self refered). I am however, wondering if there is more to my history though.. I remember one time when I was 35 I was highly stressed and I went deaf for about a day then it slowly resolved. I'm now 46 and am currently peri-menopausal. Last year after swimming laps I ended up having a migraine with visual aura.. this has happened a couple of times since wothout exercise and my eyes were checked; NAD. Here and there, I have loud ringing in my ears and believe constant tinnitus.. does this sound like vestibular migrane or menieres at all to you? Thank you for the free e-book btw..

      Thanks for your kinds words! It can be very difficult to discern between migraine and meniere’s. It may be worth it for you to see an ENT who specializes in vestibular disorders and have an audiogram and VNG test done. Meniere’s typically has low frequency hearing loss in the affected ear. You can also ask the ENT if they do oVEMP and cVEMP testing which may help differentiate. I would probably lean towards vestibular migraines but let me know what your vestibular therapist finds! I hope you feel better soon!

    I have severe b12 deficiency and borderline iron and ferritin can that also be a reason? Im dizzy 24 7 even if i dont move my neck. My head feels so heavy i get migraines and eben in my wheelchair feel like falling. Being in bed is making circulation worse but im unable to do anythng without medication i need some supressant so i can tolerate tests. I stopped doing general checkups. Gut issues from antibiotic overuse

      Low iron/borderline and b12 deficiency should be something your physician can supplement you with. If your iron is borderline it is important to find out why (ie: diet/pathology) it's something that is important to continue to get regular check up and or a second opinion. The word Dizzy is subjective and to have the best chance imo to begin to be treated properly getting on top of any mineral and or vitamin deficiencies are key.

      Multiple antibiotic use can deplete your gut of good bacteria that supports a healthy microbiome.. in my opinion I know it can be frustrating, but you have to continue to strive and seek out those that can help you be your own advocate and get some answers.. may you find the support you seek 🙏 excuse the unsolicited advice

    Hi Dr. Thanks again for a great video. Ive previoulsy consulted u about how to manage bppv as i cant tolerate epley. I have MS in a wheelchair with neck and core issues and any maneuver triggers seizures and vomiting. I have chronic ear pressure in one ear with pain tinnitus dizziness and a higher fever but no diagnosis. Neuros say its not central. Ents cant find peripheral. In bed i sleep upright and i started wearing a cervical pillow. I feel head pressure on my pillow upright without head movement and i wake up from vertigo. How is meniere diagnosed what does vng really show? Another q i have my tooth cavity trigger vertigo? Its on the side of the dizzy ear but im worried that dental work will trigger bppv. Cervical probs are there too. A strong sinking feeling in bed with palpitations.

      Ah I’m so sorry you’re going through this. Meniere’s is diagnosed based on a cluster of symptoms and can be tricky to differentiate from vestibular migraines. Typical meniere’s presentation is with vertigo episodes, tinnitus, pressure/fullness in ear, and low frequency hearing loss on that side. An audiogram can show the hearing loss, and it’s typically done with a VNG test that looks at how responsive the vestibular nerve is which can show if you have a hypofunction. A VNG will also look at any central signs from tracking certain eye movements. Not sure a cavity could trigger dizziness, but probably can refer pain. Sounds like some of you neck may be contributing to your symptoms, has anyone evaluated your neck? I would also follow up with a physician regarding the fever.

I was in er today and they mentioned it sounds like meniere and that it has to be diagnosed so we can start treatment control ths dizziness and do therapy. Does dental work trigger bppv or actually cause bppv or its only assuming that the position on the chair for a long time might? I mean does getting a tooth filling or extraction cause bppv from the work itself? I can also tell the dentist to adjust the chair. Another q Dr. Lying in bed with or without slighg head tilting if im sleeping upright i get drop sensations and my heart starts racing..does menieres cause weird inner vertigo feeling in the brain and even the spine lying down to the side? I was told my probs are vestibular but it seems its more than 1 diagnosis. And the big disc bulge in my neck can contribute to vertigo but not 24 7

      @lararose9106 dental work itself doesn’t usually trigger BPPV, if people have BPPV, then the position the lay in at the dental office may often trigger it so you can ask if they can adjust the chair to accommodate. Glad you got checked out! Vertigo can often trigger a sympathetic nervous system response or trigger anxiety.

      That could be just migraines, you may want to investigate further into what might be causing your migraines or make some changes to decrease how often you get them. It’s also possible to have 3PD along with migraines that may be contributing to the problem, talk with your medical professional about how to better manage those two! Calming your nervous system and pacing appropriately are going to be important!

      Dr. Why do i feel so dizzy if i lock my wheelchair and better moving. I have to make slow movements with the wheelchair i cant sit still. Also as im typing i have severe ear pressure and pain and dizzy even though im not moving my neck and im sleeping upright with pillows. I do have a tooth prob and fever in the ear but its a weird dropping feeling in my brain.

    I have done the Semont manoeuvre for my VBBP now I still have some symptoms ( dizziness) when I move my neck, I am scared to stay supine I feel super scared, any thoughts?

      Could be a couple of things, either you fixed your BPPV and the dizziness you are experiencing is more due to cervicogenic dizziness (typically the quality of the symptoms in this case would change from a spinning type to a wooziness or different type), or you haven't fixed your BPPV and need a different maneuver. Is the dizziness you are experiencing the same quality as it was before the maneuver, or has it changed? How would you describe your symptoms now?

      VNG can help rule things out or look at how much loss someone has had in the case of a vestibular neuritis. It’s helpful when someone’s symptoms aren’t clearly pointing to a specific diagnosis.

    I believe I have this. Over a year ago I went to an ENT because of tinnitus. I was diagnosed with Menieres. Within this past year I have had 8 steroid shots in my right eardrum - an operation where the doctor look in my inner ear and put meds in there. Then October 2023 I had surgery where the doctor put a shunt in my inner ear. In the discharge papers it said that dizziness was to be expected. So all this time I thought I was recovering from surgery. At work looking at 3 monitors and down at my keyboard really made me dizzy. I couldn’t go outside or to any stores without my head being so overwhelmed. I am now on short term disability and waiting for an appointment with a balance rehab center to try to get my quality of life back. This has been so hard.

      There’s a link between low vitamin D levels and an increased risk of BPPV, possibly because it affects how those crystals are formed and they are more likely to get displaced because of that. Low estrogen can lead to low vitamin D levels so I would say it’s a possibility!

      Interesting! I’ve always thought of increasing blood flow to the brain as improving tinnitus. I’m not sure, I’m more on the exercise and musculoskeletal side of thing than supplements but I’ll look into it and see if I find anything!

    Excellent video. I saw myself in every word you said. I actually started out being treated for stenosis of the neck for 3 years. Neurologists, MRI's CT scans.etc. until about 6 months ago I woke up with the room spinning. Called my ENT went in for an appt. had ear testing ( normal). The audiologist didn't notice any back and forth eye movements when laid back. Scheduled a VNG ( normal) for whatever they look for with that except a slight weakness on the right side. Prognosis labyrinthitis. No meds no exercises, let it run it's coarse, it's a virus. Dizziness came back a few weeks later, only lasted a couple days. It has returned twice again in the past 3 months but short term. I found this video very informative, more than my doctor ever informed me. My one question I have is can cervical stenosis bring on neuritis or labyrinthitis rather than the other way around? I am wondering now if I had the neuritis/labyrinthitis all along and the stenosis had nothing to do with it. Except I had no spinning until 6 months ago. Just curious about how much Covid could have affected this issue. When my dizziness ( not spinning) started Covid was something they weren't even checking for. Could this be long term affects of Covid virus? Ya. dizzy, hospital, meclizine go home.

      So sorry you’re going through this! I don’t think cervical stenosis can cause vestibular neuritis or the other way around as one is a virus and the other is a structural problem. COVID can definitely lead to vertigo, I haven’t looked into the research ok if people are developing vestibular neuritis after COVID, but that would be interesting! People are definitely developing dizziness with post COVID syndrome. If you had a pre-existing neck problem, then develop a vestibular neuritis, I’d say you may be more susceptible to developing cervicogenic dizziness. All good things to look into and address!

    Hello dr. I'm 21 yrs old , 2months ago i was diagnosed with VN, now it's been 2months still I'm not fully better. I still feel dizzy when i go outside or noisy environment, i can't ride bike, I can't do many things, I'm not feeling normal, anxiety and many things. I'm taking betahistine tablets for dizziness but not so effective. I'm very tired and frustrated kindly tell me how much time it will take to fully recover.

      It’s going to be very different based on the severity of loss to the vestibular nerve, differences between each person, and how you have been doing your rehab. Could be anywhere from 1 month or 6, although I know people who have never fully recovered. But you should be able to get back to being in busy environments and riding bikes!     I hope you can help. I visited a ENT yesterday after vertigo and nystagmus/tension headaches. Dix Hallpike negative, roll test negative. But at home I discovered the bow and lean test and when I bowed and twisted my head slightly, horizontal left beating nystagmus started. Leaning also provoked more slight nystagmus to the opposite side. I moved onto the BBQ manouvre assuming it was horizontal. But I only get nystagmus when I roll onto my front and lift my my slightly. Not in any other areas or positions of the BBQ manouvre. Could this be a different location?

      Frustrating! Usually when I have someone roll onto their stomach with the BBQ roll I have them tuck their chin, are you lifting your head? How long are your symptoms lasting? 15 seconds? Or longer like a minute or more in those positions?

      ​@@VertigoRecoveryDr It was very strange, I've had BPPV before, this time I'd woken two nights with nystagmus on waking, which settled after 20 seconds or so. My ENT consultant tested me and couldn't get it to reoccur. Even though there was slight nystagmus when I leaned forward when sat. Slowly recovering it seems, with a heavy dose of anxiety to boot :( Well done on the very informative videos and content, they are so helpful!

      People describe dizziness differently, but typical people experience the spinning sensation when there is nystagmus. Nystagmus can be caused from BPPV, vestibular neuritis, meniere’s, sometimes migraines, central disorders, etc. Typically the rocking/swaying/on a boat is described by people experiencing 3PD or mal de debarquement. Often people can experience spinning in the acute phase of vestibular neuritis, but then transition into developing a swaying sensation from 3PD. I hope that helps!

      @@Lovemy911 My daughter has had it since January about as long as you, and she has never had the spins just feeling like she’s on a boat, floaty, fullness in ears in the past few weeks. Her ENT said vestibular neuritis but she thinks she may even have vestibular migraines because she’s had many migraines in her life before the onset of this. Bppv is spins and menierres

    Hi Dr.. could you please make a video on antidepressant withdrawal dizziness.. it is very very common and such a prolonged issue.. ive read that it is due to decrease of serotonin in the vestibular nucleus complex in the brain.. Cud u please elaborate on the issue.. what helps.. how to power it through.. does vrt has a role in it or is counterproductive.. is staying more and more active recommended or rest is best...please make a video on it as there is literally no information on this subject

    Hi Dr. How are u can u plz share ur experience with vertigo patients who have autoimmune diseases like myself? I have MS and my vertigo still doesnt have a diagnosis. ENts say its central while neuros say go to an ent. If it is purely central does it respond to therapy? Other than bppv what causes strong floaty sensations in bed on all sides? The drs strictly said i shouldnt do manuevers and that they wud trigger seizures from my phobua and that bppv will go on its own. Im wondering why i cant sit still in a car or chair like upright i get very dizzy

      With MS, it’s likely that your vertigo is central, I would defer to your medical professionals, especially if they don’t recommend doing maneuvers to avoid seizures. The brain could be inefficiently processing sensory information leading to that floaty feeling. Practicing grounding techniques and activities to promote parasympathetic nervous system (rest and digest system/relaxed and calm state) could be helpful.

    Does it always come with an onset of bad symptoms or vertigo? My onset came slowly and I never had a real vertigo attack. Testing does show my right ear has a 63% weakness compared to left ear. I did have shingles about a year ago. I dont have spinning dizzy. I have more of an imbalance feeling. Mainly on right side when i move head or eyes closed. I also dont think I was sick when symptoms occured 2 months ago.

      It can come on slowly, just most of the time a vestibular neuritis is a sudden onset. Your brain typically compensates for the spinning dizziness but you may still have lingering imbalance, that’s where vestibular rehab comes in! Not everyone experiences cold or flu symptoms with a vestibular neuritis, most of the time it’s just an onset of dizziness.

    Hi dr. Ive txted u before and learning alot from ur vids. I have ms and chronic vertigo snce covid a couple of years ago. My ataxia lead me to be in a wheelchair. No proper diagnosis was found. Ent says neuro neuro says ent. Im having a severe inner ear infection snce weeks now and got bppv and 2 rounds of different antibiotics still high wbc and crp levels. Im freaking out about the bppv part im not allowed to do any repositioing because i get seizures from the vertigo phobia. Bppv from ths infection whats the chance it will go if left untreated im in a wheelchair cant do epley and watch my movement i have to get up in certain ways i cant change

      Oye! So sorry you’re having all this! May go away on its own, there is a theory that they can dissolve in the endolymph, but I’m not sure if that actually happens or how long it would take. Would anti-anxiety meds help prevent the seizure during the re-positioning? Maybe something to talk to your ENT about.

      Will shorten the recovery time and reduce risk of developing chronic vestibular conditions! So yes, affects it in a positive way. Just remember to pace them appropriately to not overwhelm your brain too much, you need to take breaks to allow your brain to adapt.

      that would be a better question for an ENT, but they may be helpful depending on what is causing your vertigo, possibly more helpful for meniere's, but some people with mild to moderate cases of vertigo, no matter the cause, have reported improved symptoms with betahistines, although some people don't notice any improvement at all.

      I actually don’t know that answer, I do know that tooth pain can refer pain to the TMJ, cause headaches, and other issues though. If I come across something relating teeth to vertigo I’ll post about it!

      Hi dr. I suffer from chronic unexplained dizziness since covid last year and as a result ive been in a wheelchair since a year. I have ms but neuros said its not from ms. And ents dont know y. I sleep sitting up straight but the other day i lyed down flat and got spinning vertigo in the right ear. I went to the hosp and found out i have a fever fluid in middle ear and a very high wbc count so the dr. Said its an ear infection. Im spinning on all sides even sitting up in bed. I cant do the epley i vomit and get a severe panic attack to the point i get tremors and relapse ftom ms. I have 2 more days for the a ntibiotic still no imrpovement. What if bppv doesnt go after the infection subsides? Theres noway i wud do epley.

      Hello Dr Vertigo and all the viewers, Mr. Kevin Smith i am getting back to you after 5 months. So i finally know my diagnosis of verrtigo and diziness and it's hard to be found and mentioned on youtube. My case has similarities to PPPD etc. but it's not. I visited ENT specialist that focuses on vestibular system just like you recommended me, thank you by the way. After 5 hours of testing with various methods like optokinetics and so on, doctor performed Cervical Vestibular Evoked Myogenic Potential (cVEMP) Test. My cVEMP test shown assymetry, 67 % in right ear. We then tested for what is kind of ""level of unpleasant noise", where we found that in my right year, i can tolerate only 65dB and in my left ear i am fine with 80dB, i was told that healthy person can tolerate much more, so again assymetry found. CT brain scan revealed i have a "hole" in my upper vestibuar canal. There we go. diagnosis is superior semicircular canal dehiscence (SSCD) or "Third window syndrome", my right upper canal has a hole. I was told to avoid noise, so basically when i am doing anything that includes noise, i wear construction job's earplugs, such as washing dishes or walking on street. My therapy is queit places and silence, also i cannot talk loudly because it makes me dizzy. Well there we have it, my options is of course that or it can be fixed with surgery. Problem is that in my country, in middle of Europe, there is no neurosurgeon that does this kind of surgery because it's relatively new diagnosis. If it gets worse i will consider going more west like Germany or so, to find a surgeon. Thank you for your guidance and i would love if you can make some content on this syndrome, maybe there are other people living amongs us that were hopeless, lost, in panic and scared of their life, completly paralysed by anxiety.@@VertigoRecoveryDr

      Great question! Nicotine I’m not too sure about, I’ll have to look that up! Alcohol can affect how the vestibular system functions, changing the density of the endolymph (fluid in the inner ear), but how or if it affects 3PD, I’m not sure. Alcohol can do a lot of things, one can be disrupting sleep which can prevent you from recovering from chronic conditions like 3PD. When trying to recover from it, you need every advantage you can get.

      Sure! Some people recover fine, but I will say that you will recover faster and reduce your risk of developing chronic problems like 3PD or cervicogenic dizziness by participating in VRT.

    I have a few questions. 1. What type of specialist/ doctor would I need to see for diagnosis and treatment in my area? 2. I have had a history of vertigo for almost 40 yrs with no real treatment, my doctor says, oh you have vertigo. I can't close my eyes without swaying (like rinsing shampoo in the shower, not good) Walking I tend to drift to left or right. I never really noticed until my husband and children were laughing about being careful. If I am headed North, I will drift east and then drift west ever so slightly. I tend to drift towards person next to me, hence the joke. 3. I have had tinnitus for the last 5 years, maybe longer. As long as I have background noise, i can manage, ok, bedtime is killer. It sounds like crickets outside in summer. Some days are worse than others. I know previously I have had issues with tinnitus, but it went away or wasn't as bad as it is now. I really would like to see someone but my G.P. just ignores it. It is to the point I can't. Do I need to go to an ENT doctor or some type specialist? Any advice would be greatly appreciated.

      I would look for a vestibular physical therapist to help with your balance; and might be a good idea to see an ENT who specializes in vestibular disorders. You can ask for a VNG test to see what your vestibular nerve function looks like.

      The research I’ve read has mostly not recommended it because the side effects won’t outweigh any potential benefit. I’m assuming after 72 hours the inflammation has probably resolved and you’re left with the damage to the nerve, so if there’s no longer that inflammatory process happening there’s probably not anything for the steroids to do. I would defer that question to an ENT who specializes in vestibular disorders though.
