The fact that nobody talks about the book Secret Testosterone Nexus of Evolution, speaks volumes about how people are stuck in life.
I was working 60 hours a week in an er during Covid. Felt super run down and couldn’t catch up on sleep. Asked my doc to check my t and is was 4. Found out I had a rathkes cleft cyst on my pituitary and started trt and had it removed. My life is now amazing:)
TRT has been a life saver for me.
@jamcinty can I ask how do they check the Pituitary gland?

@@eljonnymac I've just found out my T level is 5! I'm only 36 yrs old, doc just said take Testosterone but I want to solve the root cause which I think is my Pituitary so will ask him for Clomid instead to kick start it, and if no improvement will ask for MRI like you (thanks to your reply I just found out 30% of people with low T have a rathke's cyst)
@@joneslive586 I was 38 when I was diagnosed and my doc checked my FSH and LH to see if it was primary hypogonadism or secondary before I started taking testosterone. I would try and get a referral to an endocrinologist. Good luck!
The more I learn about testosterone, the more confused I get. Biggest takeaway for me is throughly self-experiment and address lifestyle factors first. As I’ve heard Peter Attia say before, ”the most potent drugs we have are food and exercise.”
bro same. still got a long way to go but excited to keep learning.
@Nate D Now that’s the kind of explanation that my brain can compute. And I hope that’s a reflection of your ability to make the complex simple, rather than my lack of intelligence 😂. Thanks! 👊🏿
And 😴 sleep
There's no confusion at all. The only thing I find confusing is that people like yourself believe that exercise and food is going to actually increase your testosterone to a therapeutic level again. When you're testes stop producing then your body is trying to tell you something and it's not telling you to jump on a treadmill and stop eating the Big Macs. Good luck with that plan of yours oh and let us know how it works with a truthful blood test before and after.

I am 34, excellent health and in great shape. But energy was low for many years. My total was in the low 300s for years. Finally started 100mg cypionate 1x per week. It's day 4, and I felt better than I have in a decade - maybe ever. Important to find a good doctor, though. Not only labs, but physical, EKG, echocardiogram, etc. Messing with your hormones is no joke. But it can be done safely and hugely improve your life. Best of luck dudes.
How did you go about finding a doc? The only clinics in my area are these crazy monthly fee clinics with telemed visits only..
I'm going to be 39 this year. Been having the symptoms 2 years ago, last August I was on low 400s, finally found a doctor with with and started me .3ml of 200mg once a week.

Try to increase injections if you can. It sucks doing more injections but I think its becoming consensus that smaller more often dosage is preferred.
The euphoric feeling wears off after a few months.

I think its great now that doctors can actually help us aging men get trt to make us feel better and give us energy to keep doing the things we love to do. Im 50 years old and im a very active person - i was just hitting the wall was so tired and sore all the time. I got tested and my test levels were under normal. I got given the lowest dose which is just a testogel to rub in my skin daily. With in weeks i was feeling great. I noticed it the best in my running - before my legs would give up before my cardio would. Now my legs work for miles and miles. My recovery has improved immensely also. I can still stay active and do the things i love. I believe if men dont get this it puts them in the grave quicker.
I'm 32 had levels of 208. Doctor is putting me on the TRT protocol and looking forward to getting my life. I eat health, weight train 5 times per week. All men should get checked.

@@knife1406do you know why you had low testosterone? I used to have high-normal amount now it’s extremely low all of a sudden
The clinic i am with does .125 twice a week. I am going to tell a very true unbelievable experience that is very important as it relates to TRT and its biggest issue Estrogen. I have been on TRT for 5 years. I do not need it no excuse but i wanted to try it and love the benefits. I however did have a ton of issues with Estrogen early on. I currently do not take any Anastrozole but at one point i had to take over 1mg a day, yes you heard correctly. I thought i was going crazy because when i started TRT i only had to take .125 twice a week and even then it was low for about 24 hours after i took the Anastrozole. What i didn't realize is if you take Testosterone but do not work out at all your Estrogen will go sky high. It wasn't in my head. I know what high Estrogen and low Estrogen feels like. Low Estrogen is bone crushing literally and figuratively. However very high Estrogen feels like you had six espresso shots and your on a first date with a pretty lady. Its very uncomfortable so obviously if you feel this bad and take Anastrozole and then feel great well it doesn't take a PHd to figure that out. Once i resumed working out vigorously 4 weight training and two moderate cardio sessions i noticed i dramatic difference a massive chage in Estrogen levels. Oddly enough now my Estrogen is too low. I do not take Anastrozole anymore. If i dont work out i have zero doubt it wont creep up again. So i think this is extremely relevant even ground breaking news or information. I mean exercise dropped my Estrogen more than 1 mg a day of Anastrozole. Thats insane! And trust me i am a very skeptical data driven individual. I thought it was impossible to need 1mg a day of Anastrozole. I hope even a few guys on TRT see this because if you increase the frequency and intensity i strongly believe you'll be able to lower or stop Anastrozole all together. I will say i have 7% bf currently so im sure that helps but again this came with the vigorous workouts. I did notice an almost immediate change when i started up again. The reason for stopping was a injury otherwise it i have been a life long athlete in one way or another. My dose is currently 160mg but has been in-between 140-200mg. Im 38 5'8 170lbs 7%. I do subcutaneously eod with an insulin needle.
Very interesting
I'll add the other end of the spectrum. If my estrogen is in the normal range, I feel like crap, with no labido. I dont lift that hard. (2x per week). I was cutting the .125 anastrozole in 1/4 and taking 3x per week. Still felt like it crushed my estrogen. With no inhibitor I'm in the 50 to 60 range and feel great. As a side note, I have always been a "horny" guy. So maybe high estrogen is my normal state.
This was very interesting! Thank you!
Go the gym bud! Its the best thing u can do for urself. 💪
@@hopshlooken In my opinion 50-60 is not a high number at all if you testosterone is sufficient and you have a good testosterone estradiol ratio. BTW is this the regular estradiol test or the sensitive one. If it's the regular probably this number is close to 30-40 which is perfect since regular test often overestimates estradiol (have seen the opposite though too)
I've just had full blood work done with my doctor after a home blood test from a male health clinic told me I had 10.8 testosterone. I cleaned up the diet, got more sleep and upped my training, retested a month later - 8.2 .. however it didn't tell me my free testosterone

What is your age? Do you exercise? Do you regularly have high blood pressure? I was on 200mg weekly (age 30) but I felt it was giving me too much high blood pressure, and the doctor told me to cut back to 100mg as he originally tried to get me to do (stubborn me thought 200mg was my sweet spot) but I got discouraged and just quit all together. First time I’ve heard positive things about DHT. Good to hear
DHT is incredibly important. The feel good energy, libido, strength, and mind/muscle connection come from DHT.
I started at 47 with 125mg Cyp 2 x per week which is high as per TRT protocols. Was 4 x above upper limit according to bloods. Years later, dropped to 75mg 2 x per week + 2iu GH per day. Test now sitting at 2 x above higher level according to bloods. Very happy with results and blood tests. Due for a CAC Score and Echo-cardiogram sometime this year coz bloods don't really tell you whats happening. Last CAC Score was Zero and echo was perfect. (Not recommending TRT or HRT. Do your due diligence.)
What’s the point of the echo?
TRT has been a game changer for me. 54 year old.
When you say game changer can you elaborate? I'm your age and been dealing with fatigue
Five years ago I went on TRT (70), at that time I had a PSA of 1.6 with a healthy prostate. Excellent discussion. When you check both your websites, it’s understandable you don’t give opinions/ advise/ consultation to patients that are not physically present as patients. You do also say, which is 100% correct that TRT clinics usually get it wrong or just interested in selling more pills. So how do we as men concerned about these issues actually get the correct advise… or do we have to do our doctorates to advise ourselves. This is a honest questions as it’s very frustrating, as all we want is educated honest advise, of course we are willing to pay for it… but we want real correct advise… how do we get that?
Exactly my concern too
Go to a good doctor instead of TRT Clinics.

Same question. I’ve seen a few PCP’s and recently a urologist. They won’t help me. The example patient Dr. Attia described is almost exactly me. My free T hovers around .7-1% of my total. @@micbrock most regular doctors really have no idea what they’re doing with TRT. They will just shoot you up every two weeks and test your blood every six months or so. I would definitely think about getting to a real, reputable clinic.
As far as SHBG goes, look at taking 10 mg of boron once per day. It definitely lowers that.
Hormone replacement therapy in men, particularly testosterone therapy, is becoming popular among younger individuals who are primarily focused on cosmetic effects. However, it is important to prioritize lifestyle factors such as sleep, exercise, nutrition, and social connections before resorting to hormone therapy. Factors that affect testosterone levels include genetics, estradiol, insulin, and thyroxine. Normalizing free testosterone is crucial for determining the need for testosterone replacement therapy. Clomid, an oral fertility drug, can be used to stimulate testosterone production, but its use should be carefully monitored. Anastrozole may be used to control estrogen levels if they become too high. Testosterone replacement therapy should involve smaller, more frequent doses to avoid side effects. Care should be taken to avoid the incompetence and overuse of hormones seen in some testosterone replacement therapy clinics.

I have been on 200mg of Test Cypionate per week since I was 40. I am 47 now. It was one of the best decisions that I have ever made. I had 2 test, 2 weeks apart before I started. My total was 207 and 211. The benefits are obvious. My labs are checked every 3 months. I have a script of anastrozole. I never use it. My estrogen stays in check. I give myself 100 on Monday mornings and 100 on Thursday evening.
What company are you using? I am just looking into trt clinics online and I am overwhelmed at the options.

I have had to donate several times to get it down to 16. Feel great, don't know which way to go. Opinions are all over the place.
Amazing podcast as always. Andrew, how should a healthy individual think about the naturally declining testosterone production due to age from performance and longevity perspective? Does it makes sense to try to keep up with peak testosterone production as long as possible? For example: we are conducting monthly measurements and noticing 15% decrease from peak (age 30) at age 40 but still being inside the reference values. Thanks.
Igore, cime li se bavis kad ti treba ovo? Samo se interesujem :D
Being at “low, normal” is not necessarily healthy or beneficial. They even lowered what they considered low normal now. A lot of guys at the low normal range feel terrible all the time. I would definitely go and look into it and possibly get yourself set up to “high normal”.
@deebee4575 I do not trust the general community, they've changed hormonal standards, obesity standards and more. And there's a lean towards de masculinizing as well
Iinject 57mg every other day. I was on 86mg every other day but my doc lowered my amount. When I was injecting twice weekly by day three I was crashing hard, so I had to switch to every two days. Being on 300mg a week I felt awesome. Cutting to 200mg weekly I feel good it is an amazing antidepressant, wound and injury healer, pain reducer and of course gym recovery aid. I take Clomid 50mg and Anastrozole 1mg three times a week.
For supplements I’m taking Vitamins D3, K2, Berberine and Red Krill oil.
Before starting TRT my total test was 170, free test was around 3 and my estrogen/estradiol was in the 30’s.

@@lolmouf Not necessarily true at all. A lot of physicians use a range for all males at any age or weight. That doesn't mean it's the appropriate range for you specifically. If you're 23, sleeping well, eating well, and consistently working out, you shouldnt be at the low range of ~300 ng/dl. 600+ would be more ideal in that case imo.

I'm 41 and work 50-60 hours per week with a needy wife and 2 toddlers. The honey do list never ends. My sleep sucks and i try to find time to work out but its not consistent. Always tired and feeling rundown. Kaiser straight out says you dont need TRT and simply orders a sleep test requiring an uninterrupted 8 hours of sleep which is impossible for me. I need to find a good doctor in the los angeles/ orange county area that will run my bloodwork and will be honest with me on whether or not I even need TRT.
I’m 42 year old and I’ve been on 1cc per 100mg every other week for 6 months now. My testosterone level was 120. I feel somewhat better the same week of the injection, but then get really fatigue feeling during the second week, give it take a few days. My initial symptoms were fatigue, irritable, no sex drive, erectile disfunction, and really dazed. I’m not sure what tunnel vision is, but I catch myself just staring at someone during conversation, or driving, or watching tv, but I can’t focus on anything. I have to kinda literally shake it off, and make myself come to to focus again. I was put on an anxiety med before this, but my symptoms got much worse, except for the irritability. I’m going in for a follow up this week, so I’ll see what my dr has to say. I wanted an mri to rule out the pituitary, but he wouldn’t order one. Seems like it’s just easier to prescribe something… I’ll probably try to find an endocrinologist if nothing changes. I hear and read these stories of men getting there life back, and I’m longing for that myself. I was always a really active person, but I have to force myself to do anything now.
Can you follow Dr Attia’s instructions? Take smaller doses 2-3 x week? Every 2 weeks is very outdated and always leaves guys feeling terrible during week 2.
1000x see an endocrinologist. They just know so much more about this and can vary your treatment. I had a pituitary tumor than removed much of my gland, and before they figured that out it was ruining my life (I had other symptoms that made it clear, it could have absolutely nothing to do with your issue which is why you need an endo).
A lot of hormone doctors and specialists I have talked with are against the 1 dose every two week. You hit a high concentration quickly and then it is all down hill from there for two weeks until your next shot. Whereas if you take the same dose and spread it out evenly over the two week period, you are still getting the prescribed amount and your blood levels stay much more stable. This should prevent those crashes. Talk with your doctor about this.
You for sure need to spread that dose out. Need to be poking every 3 days.
Totally agree with the others on increasing your frequency. Its now very outdated. I do every other day. It sucks at first but you get used to it.
I’m 32 I just started test and clomiphene 50 mg . Hope it helps me
These guys seem to know there stuff
I am 56 and just now looking into the subject. I barely got this info. Lots to learn.
TRT is definitely something to consider. I know older guys in the gym who take it, and they have insane fitness levels. Also you get the sex drive you had in your early 20s back as well.
@@Jay-bc7kh But is it worth it, if you risk all the bad stuff for your heart etc? I don't know much of TRT, but it sounds like a mild form of steroids or smoking for staying slim.
Omg I’m on finasteride and I never heard this take before. It’s helped my hair so much and my sex drive is still higher than ever.
Hello fellas! You guys are both dynamite in your field and I appreciate every ounce of information you guys are willing to convey to us all. I have a question however. There is some speculative at best suggestion that elevated testosterone leads to prostate cancer. In your experiences, would you consider this factual? If so, Would you consider it causative or a correlation?
+1000! (I did not see your post before making mine above. 😉)
No, its looking like it does not but having low t may contribute to it, the body needs t and if its not at the right levels that is not normal other health problems start. infact some give t with prostae cancer and have seen no rise in cancer See Testosterone: Myths & Facts
When i first checked my T, i was at 80 at age 35, so spoke to an endocrinologist i think it was and she had me exercise and eat healthier i and did, and after 6 months my T came up to 94, she kept saying its coming up so she refused to diagnose me or put me on anything, even though insurance wouldn't pay she finally agreed to put me on the cream, which is a nightmare, didnt feel any difference, still foggy headed, lethargic, and it brought up my T to 120-130. She still would not diagnose me and cream was too expensive because insurance wouldn't cover the high cost until i was "diagnosed", i ended up going to a clinic, which like they said, have their own issues. That was a few years after giving up on the endocrinologist, still exercising and eating well, i was only at 218. Now days, 39y old, I am on i think .4cc syringe of testosterone every 4 days, 80mg? dont quote me, dont know the type, and regardless its been a life changer. HOWEVER, i noticed if i smoke pot, or eat a lot of sugar, there is almost no noticeable benefits to the shots. But if i dont smoke everyday and limit my sugar intake, HOLY SHIT, just naturally lose weight, my muscles and veins seem more refined, libido is higher, energy levels are borderline hyperactive disorder, but there is minor back acne and i recommend taking some aspirin before bed because snoring becomes a real issue as well, fair warning, your partner might not like the snoring but somewhere told me to try a blood thinner like aspirin and again, helps. It sucks, but like Dr Huberman said in other podcast, pretty sure he mentioned this, THC, for some people, not all, but for me, it must just neutralize testosterone or something, because, no joke, i have to repeat this, if i smoked everyday, and was still active and eatching healthy, i would bloat up and gain weight and my T levels were awesome, I think based on the science, my body on THC converted the T into estrogen very efficiently is all i can figure out, bottom line THC fucking destroys T in my body.
So too much testosterone is bad unless it's free testosterone unless you also have too low estrogen unless you have too much estrogen unless you also have too low DHT it unless you have too much T4 unless you are on a TRT cycle. Got it, very simple.
Exactly. Know what you’re doing and don’t rely on the docs.
It’s the worst thing I’ve ever done. I’m 43 and it felt so good feeling like I was in my early twenties again. I only exercised to keep up the muscle mass I already have. My entire body was so hard. The first 6 months my libido was incredible. Then it just died! Yet my blood work was great! 9 months after I started I had to quit. The only problem with that is that no matter how much I exercise now, I’m soft. And believe me the depression post use is definitely there.
43 here, just looking into it, why did you have to come off?
@@danielhoward786 I’m sorry it took so long for me to get back to you. Ultimately it was because it caused acne. It doesn’t with most people but it runs in my family. High testosterone levels can ignite the oil glands. When I was younger I took a medication to stop the acne. The medication basically turned off my oil glands. It turned mine back on. Don’t let that scare you off if you are interested in trying it. Of course you may already be on it. I think it’s different for everyone and you just have to try it for yourself.
I took 140/wk of bi-test (prop cyb blend).
They gave me crazy energy and anxiety.
I lower the dose all the way down to 80 mg a week splitting the dose to every other day to equal a total of 80 for the week and I went from intramuscular to subcutaneous.
Nothing changed. I still had anxiety and incredible struggle to sleep.
I had erections randomly all day long like a teenager. Anyone who thinks that would be nice, just know in your late 40s it's not something most will actually want after it happened. Also my balls shrunk like raisins in 10 days from 1st injection.
I quit and my testosterone is lower now than b4 i started
If my doctor would have prescribed it, it may have been different but Kaiser won't do it and I went to a TRT clinic and the response to my problem was to recommend I take deca.
So it was about three and a half weeks in after getting that reply that I just quit TRT called tricky.

Do I consult an Urologist or an Endocrinologist to evaluate all these factors for starting TRT?
You did not address this issue on this episode (maybe because it is geared to men who are not quite yet in the prostate cancer age range??), but yes, a discussion on the catalytic effects of the various types of testosterone (and it's counter-valent compounds/hormones) on prostatic carcinoma growth, would be welcome.
Given that I am told (incorrectly??) that if any man lives long enough, he WILL get some form of prostate cancer, and also that testosterone (non-specific, they do not get into forms/'types' at all) is like nitromethane for cancer cell growth in the prostate, it IS quite relevant.
Given that I have survived stage one, low Gleason score, prostate cancer, and the above being pounded into my head, I am VERY wary and reluctant to even think about any form of testosterone replacement therapy, and I am sure that I am not alone in this, given the preponderance of this cancer in our populace.
This has been written about extensively. Google it.

How do I become a patient of Doctor Attia ?
Yes, I also wish that I lived in his location in order to become a patient, since he is into endurance sports on an exercise physiologist's level (NOT simply just about 'bulking up' as so many others on here want to do, exclusively, i.e.; "more plates means more dates" LOFL), and would understand exactly what I am trying to tell him as relates to my cycling.
UNLIKE my current primary care doc, who is used to treating people my age (67) who (mostly) are already on 10-15 meds (and he wants to put them on MORE!) weak, decrepit, COPD and cardiovascular disease ridden, obese, and sickly.
Dr. Attia seems to be more geared towards preventing problems ahead of time, rather than merely trying to treat the symptoms of those problems AFTER the fact, as is the 'standard operating procedure' in U.S. medical practice.

I started on tongkat ali and fadogia so I had my numbers rechecked before I started after having implemented these changes for a few months and it's still only at 200. I'm really hoping that the tongkat and fadogia will help but if not, should I try TRT? If I were to ever stop, with my natural ability to produce it be jacked up forever?
Test can be reduced from fatigue as well. You might be exercising too much. What is your bf%? If you’re at a high (30%+) bf, then your test could be aromatizing into estrogen.
I’m 32 and on TRT. It’s an effective treatment, but it should be a last resort, particularly if you want to have kids.
The consensus is that once you start, you should plan on taking it for the rest of your life. Best case, you take it for 10 years and come off. You’ll still have total test of 170 if you fully recover.

Holy crap, you spend way too much time in the gym. Slow down buddy. Days off are just as important as days in.
It's tricky when, like me, you deal with a huge amount of stress due to an anxiety disorder, and cortisol devours testosterone - but testosterone also counters cortisol, therefore decreasing stress. I already do everything I can for my anxiety disorder, but it's not enough. But nobody will prescribe me testosterone.
I'm on the low end of normal for my age, but I'm not sure what 'normal for my age' means. But he's saying here 'upper end of normal' without qualifications, so I'm assuming that means for your age. My libido has crashed in the past few years, after passing 40, and I have ED. I didn't have this problem a few years ago, despite my anxiety disorder being just as bad. I eat well and exercise, but I have a terrible sleep cycle. However, that poor sleep cycle is the result of anxiety, so...yeah.
