    Wow! Great video. I’ve never thought about the stages of the keto diet this way but it makes total senses to vary the amount of fat you’re eating depending on if you want to burn your own fat once fat adapted vs needing to eat fat as a fuel source once you’re lean.

    I went hard keto in 2020. 6 months later I had lost 50 lbs , my hypertension, joint pains, and all pharma.
    I was under 20g/day carbs forever and never experienced anything like so-called “keto flu”.

      I gradually ate fewer carbs as I gave up certain foods that were bothering me, so I never had keto flu. I did have one episode of oxalate dumping.

    Thanks.I feel better listening to you two doctors.I feel mt best on a ketogenic diet.Whenever I go back to carbs my blood sugar rises again and I gain back all the weight.Some educators teach that a ketogenic diet is not sustainable.I just feel it is the best diet for me.

    Oh my! This was SO helpful to me! I have been low carb since January with time-restricted feeding, intermittent fasting and lost 35 pounds. Then I hit a plateau. I still stay away from seed oils sugar and processed food, but got lax on healthy carbs and lengthened my feeding time to 8-10 hours. This greatly helped me understand what I am doing wrong. Thank you!

    Agree. I am 3 1/2 months on keto. Lost all skin tags and benign skin growths. Lost 30 lbs. No hungry during the day. I also only eat between 4:30 am and noon👍. Feel great.

    Ketones are not an accident or a by product. They are a required fuel and substrate mainly for our huge brains. Your liver produces ketones and glucose under the catabolic action of glucagon moderated by insulin. Being in a fed ketogenic state is our natural intended state. The problem is constantly consuming glucose blocks fat burning and ketosis. Your liver makes all the glucose you need. Any you eat is extra and is a problem of storage and disposal for the body leading to chronic disease and obesity.

    I love my new keto way of life. Lost 56lbs in 6 months. However, I did experience a horrible keto flu/rash. My chest still has the dark marks left behind although it’s getting a bit lighter now. It is very real for a lot of people but since we are all different we will have different experiences. I’m happy I fought through and didn’t give up on this.

    As always, it depends on what you're trying to achieve. I got anti- inflammatory and pain modulating effects within 3 days of cutting out the carbs, but I'm clearly NOT fat adapted at that stage. I gave up on Dr C when he strayed heavily into psychology, referring heavily to his perceptions of what was wrong with his upbringing, and overstating things for dramatic effect.

      Since then, my levels of insulin in the periodic bloodworks during this time were:
      3.31 (April)
      1.44 (May)
      4.61 (August)

    Thank you! This is why we use the GKI system. The ratio between blood sugar and ketones gives us an idea of how much insulin we are producing.

    After fighting with unstable blood sugars for 55 years I've learned a few things, plus more recently from these 2 brilliant docs. Keto flu in my experience is from dropping blood sugars when levels of insulin production stay high, its natural adjustment lagging behind the drop in carb intake when an individual starts a low carb diet. Almost wrecked my car more than once when this has happened, and have suffered plenty of other instances where most of my higher brain functions go offline. This time I know to gradually reduce my carb intake (and track it to show myself what I did) over the course of maybe a week because for me dropping from, say 125 grams of carbs/ day to 20 is just too much of a shock to the system.

    Dr. Cywes is great 👍. Learned a few nuance items from some of his past videos. Especially were he had some patients add back in higher amounts of carbs.

    Having ketones is an impirtant part of a keto diet. Ketones have very important functions not only as nutritional metabolites but also important cell signaling molecules. Both is impirtant. I agree with getting the ketones from a ketogenic diet being vastly suoerior.

    I made the switch from SAD, to keto to carnivore and DEFINITELY had keto flu AND an horrible rash. GI upsets also, which I suspect was a disrupted gut microbiome. P.S. This is one thing Bart Kay and Rob Cywes agree on...making the changes over the course of several weeks.

      Thank you, Doctors, for your work! The keto experts guide the public fabulously! Other keto doctors include Dr Boz, Fung, & Berry who all teach us great info. Kelly Hogan continues to research and share core keto info with subtle tweaks to improve people’s health. I wouldn’t trust an unknown doctor due to their shocking lack of nutrition training and food pyramid disaster. It comes down to continuing to eliminate foods that hurt/poison you. Folks have found great health through keto/carnivore. ❤❤❤

    Every time I try to “be normal” and eat any low carb veg in any quantity larger than a quarter cup for a whole day, or, a tablespoon at each of my two meals per day, my stomach immediately blows up into a rock hard lump, I get nauseated, and I experience intestinal discomfort for a day or two. Sometimes the gas is so bad that it makes me want to cry…. and I’m a grown woman who can withstand a lot of pain.
    Dairy doesn’t seem to do this to me. I can eat meat, of any kind of eggs, fat of any kind, and dairy in the form of small amounts of cheese and butter/heavy cream, and the very occasional handful of macadamia, pistachio, walnuts, or pecan nuts. And I seem to be able to handle spices as long as I’m careful about blends. That’s it.
    I’m grateful to know what I can eat though…it took years to figure it out.
    I can’t wait for studies to be done on how our DNA may or may not influence our tolerances and intolerances. I think that knowledge will be very interesting. Maybe not terribly useful, but might at least help guide our choices? Who knows?

    In 2018 I lost 65 lbs., reversed insulin resistance & fatty liver on Keto, but had trouble with electrolyte imbalance even though I was eating vegetables daily. I had to supplement with electrolytes. I never had the benefit to find a doctor with experience in ketogenic eating that I could work with.

    I don't think i've ever had a keto flu, But when I first tried five years ago I did get a herx rash that didn't go away till I started eating carbs again. It was all over my chest and neck and lasted over a month

      I never had it. However, three friends of mine all had it really bad, like half their bodies! Glad I dodged that bullet.
      I looked it up and read about Asians were more susceptible. My three friends were Japanese and Korean. Interesting.

    When I did my first ketosis I had keto flu and it was terrible. I had to reset and try again, but with additional electrolytes (magnesium, potassium, sodium) supplements. Each time I re-enter ketosis I had less & less flu symptoms. Now I don't get it at all.

    Im struggling with the question of can you get the benefits of fat adaptation on a relatively low carb diet or do you need to be carb free? Clearly, if you are someone used to eat 400 grams of carbs a day and cut back to 100 grams most ppl will lose significant weight. Clearly in that case eating a low but not zero carb diet is beneficial despite not being in ketosis. So, assuming one can maintain the low carb approach and not go back to their former levels it seems to me a low carb diet can be healthy. Am i wrong on this?

      I think you're right. The recommendation that I've heard is less than 50g of carbs a day, ideally sub 20g if you can manage that. As for a zero carb diet, at that point what else can you eat but meat and pure fat like butter or tallow or lard? That's a carnivore diet and not everyone wants to do that.

    I definitely got the keto flu ~ I was just lowering my carbs to less than 75 g and I felt awful!! 3 years ago I did not know about taking electrolytes but I did use trace mineral drops in my water but it didnt help~ so I just finished my 21 day commitment and went back to carbs/ I eventually very slowly reduced my carb intake cutting out bread then sugar then all grains/ until I was eating 20-30g of carbs ,started using electrolytes then I got a rash which took about 2 weeks to go completely away~ IVe since eliminated fruits and most veggies but still do dairy and Im still learning my way to a better way of eating and living!!

      I think that is because he eats a LOT of meat! If you overeat protein the body converts the excess into glucose as protein cannot be stored for later use.

    I'm always curious about how Keto and fat adaption affect humans with no thyroid gland. I had mine removed 10 years ago due it trying to kill me with cancer. Not a very nice thyroid gland, needless to say. 😊 I've been ketoviour for 11 weeks now. The weight loss has been slow. Any thoughts?

      @rebeccamiller6050 Hello Rebecca, Slow weight loss might mean some insulin resistance still around. Or some inflammation, too. Or lot's of chronic stress (cortisol --> glucagon --> blood glucose goes high --> insulin resistance). Low quality sleep can induce an unbalanced glucose metabolism / insulin resistance too.
      Do you have any blood work from before you sarted keto? Any glycemia + insulin measurements that would allow you to calculate your HOMA / QUICKI indexes? (They will show your level of insulin resistance / sensitivity).

    I never got any of these withdraws people get. I was weak the first few days adjusting to ultra low carb and one meal a day at first. Been on it all year minus a few short breaks.

      By eating fat and protein and reducing carbs . It just happens naturally.

      You are right. Here is my n=1. After about 2 months of regularly high ketones 3 or more I had a precipitous drop, 2 or less. From what I gathered by listening to these experts my body became fully able to utilize the ketones and took them out of the blood. Regardless I kept eating the same and kept getting good results. I hope this helps.

      I measure my GKI, Blood Glucose & Ketones almost daily.
      My typical gki < 3 mostly

      Ketosis is being fat adapted. I don’t understand your question. I’m genuinely curious because the only two ways to get there (that I know) is:
      1. Don’t eat anything until in ketosis.
      2. Don’t eat any carbs until in ketosis.

      @@BuzzMoves365 Fat adapted is when you cells/ body readily burn the ketones. There is a marked difference when you become fat adapted. It took me a couple of months. I measured and saw preety good drop in my blood levels.

    I started a keto diet and had awful keto flu for 2 weeks but I stuck with it. I lost 40lbs in 8 weeks. I ate mostly green leafy vegetables and chicken, fish, beef, eggs. I maintained a low carb lifestyle and kept the weight off for over 2 years. As soon as I injest carbs I gain weight back quickly. I get the keto flu every time I switch between carbs and keto diet. Seems like I have a hard time transitioning to being fat adapted. Menopause has also made losing weight difficult now when I am on keto, which was not the case when I was pre-menopausal…sigh

    Notice he doesn't say "food addiction doc" That's
    because he understands that the problem is carb
    addiction. Carbs are the problem. When you next
    have one of those people on who dwells on "food
    addiction", have them talk to Dr. Cywes.

    Can you address the issue of weight when a person is taking heart medications? My weight loss has come to a screeching halt no matter what I do….I just had HA and am also inactive…can a person take meds and lose weight?

    I got into Ketosis (about 1.5 on ketomojo) without ketoflu, burned fat like crazy and was not hungry. I think hunger gets confused with carb addiction.

    it would seem to be a correlated psychological and physiological question. Some people can be moderate users or mild users of drugs and alcohol and it not really affect their life. The odd consumption here or there on special occasions has no long term effect. Whilst its not positive it can be seen as permissible. Others, one drink means they have begun the circle downwards that leads them automatically to excessive consumption. For them a complete change of lifestyle avoidance of drugs or alcohol is critical to the health and well-being. It's all nothing. The real difficulty is not kidding yourself as to which group you belong to.

      In most regards, maybe not. But there are benefits, especially since ketones are also a signaling molecule. It helps your body get used to using ketones for fuel! Especially for those with Alzheimer's, it can make up the missing energy

    Doc, congratulations on finally talking and learning about ketosis. Maybe you can now learn to define "keto diet" as a diet that induces the production of ketones by the body (endogenous and not exogenous).

    It's good at first, do it for a year or two. Go carnivore for a couple and then go animal based. Meat, fruit, dairy, nuts, etc

    Ketosis is good to be in but not all the time. You should cycle in and out of ketosis to be regular.

    Exogenis ketones are not that beneficial in the long run if ur body is not already producing ketones naturally from ur diet. Its better to have natural ketosis then using external stuff

    Cywes has an ego problem . . . if you watch him for any length of time you'll see it clearly. Many of his presentations are done with the pure intent of getting attention. One of his favorite ways to do that is to challenge the popular. This is one of those challenges.
    Additionally, Cywes is in sales mode. He is actively pushing for patients with every video. Checking one's insulin REQUIRES an MD so it's no wonder he's pushing his insulin theory and down playing checking ketones.

      Chronically high insulin causes insulin resistance right? Hyperinsulinemia is Chronically elevated insulin, is it not? Maybe i am missing something here?

      By all reports he seems not to have had a weight problem, i put it down to a quality Jewish diet. Maybe the gospel that mentions his obesity was cut out when the editors created the New Testament.

    Ketosis is not the presence of ketones in the blood, ketosis is a metabolism/a metabolic state. "Fat adapted" is a description of the ketogenic state. He cheapened the definition of the word for clicks just as food companies cheapen it to sell processed foods labeled keto-friendly.

      I don’t get the difference. When ketones are in your blood (not too much) aren’t you in ketosis?

      Ketones are always in the blood, the actual amount will depend on how much fat you are oxidizing at the time.

      @@bonnieschmidt5882 not necessarily. An easy way to think about it is ketosis is the body making its own ketones from fat. Simply taking in ketones from a product or eating processed food labeled keto will not put you in that state. Your insulin has to be low enough (otherwise you're in ketoacidosis) and your glycogen stores from carb intake has to be depleted, plus other physiological factors I'm sure I'm missing before your body starts making it's own ketones.
